Book excerpt: Sensible au contexte d'éducation thérapeutique qu'implique cette maladie chronique, nous avons étudié la transmission des messages des internes ainsi que leur contenu en jargon médical. Notre travail porte sur la communication verbale interpersonnelle dans la relation médecin-patient. Matériel et méthode : Démarche qualitative avec double recueil de données : par des consultations enregistrées suivies d'un questionnaire sur les messages émis et reçus, et par un focus group d'internes. Résultats : 9 consultations enregistrées, montrant une prédominance d'occupation de l'espace verbal par les internes (60% en moyenne). Les questionnaires ont révélé l'importance de la différence entre volume d'information émis et reçu. Un focus group de 10 internes a retrouvé les thématiques des messages des consultations. Discussion : L'analyse des messages a montré un jargon régulièrement expliqué et reformulé, une absence quasi-totale de vérification de compréhension, et l'utilisation régulière de banalisation et dramatisation. Le focus group a isolé beaucoup d'obstacles à la communication : liés aux internes, aux patients ou divers. Les internes ont jugé nécessaire d'améliorer leur expérience, le temps alloué aux patients, de vérifier la compréhension des patients et d'éviter le trop-plein d'information. Conclusion : Le jargon médical, bien que présent, ne semble pas le principal obstacle de la transmission de messages. Les futurs médecins gagneraient à laisser plus de temps de parole aux patients, leur donner moins d'information et vérifier leur compréhension.

Un risque accru de fracture a été observé chez les patients diabétiques, alors que la DMO est 5 à 10% plus élevée chez les patients atteints de DT2 par rapport aux sujets sains. Nous avons utilisé la tomographie quantitative périphérique à haute résolution (HR-pQCT) pour caractériser les paramètres micro-architecturaux corticaux et trabéculaires, chez les DT2 comparé à des sujets sains. Nous avons conduit une étude cas-témoin nichée au sein de 3 cohortes prospectives, réunissant ainsi 507 participants, hommes et femmes inclus. Chaque cas était apparié à 2 témoins, sur le sexe, l'âge et l'IMC. Les hommes DT2 ont une densité minérale osseuse augmentée de façon significative au niveau du col fémoral (0.83 ± 0.13 vs 0.79 ± 0.14 g/cm2). Après ajustement sur la médiane d'évolution du diabète, on observe une aire totale au niveau du radius et du tibia diminué de façon significative. Parmi les femmes, au niveau du radius, l'aire corticale est significativement plus grande chez les DT2 (44.35 ± 9.26 vs 41.17 ± 10.05 mm2), de même pour la densité corticale qui est plus haute (826.6 ± 70.21 vs 804.37 ± 72.72 mg HA/cm3). Book excerpt: During the 1970s and early 1980s, the Middle East and North Africa were perceived as being exceptionally successful, but now the region is viewed as a resounding economic and social failure. Islam is not only a religion, but also a political and social project. A major pretext of this work is to demonstrate how the tensions within Islamic movements feed directly into the economic, social, political, historical and religious arena of the region, and vice versa. An introductory chapter sets the context of the book. The core chapters of the book comprise an in-depth examination of the varied forms of oil revenue abuse. For examples, the past mismanagement of the tremendous wealth provided by oil. Following Islamic beliefs, revenue from oil should not finance wasteful consumption, but used instead for public welfare. Abstaining from interest calculations, there should be a case for keeping more oil in the ground. Indeed, oil has also stifled industrial development, and with declining oil revenues, the conflict between civilian and military priorities intensifies. While western interests have promoted arms spending, high population-growth expenditure reinforces the reality of the count-down to the post-oil era upon the Middle Eastern and North African oil exporters. So far the governments seem unwilling or unable to adapt and react. Furthermore, in the past oil has been used as a substitute for democracy. While the large oil revenues of the 1970s and early 1980s strengthened the position of autocratic rulers and weakened the private sector,repressive regimes have made Islam a source of criticism and opposition for the Western world. Following on from this, the book then looks forward to the problem of uniting the divergent interests in the spheres of oil and Islam into a cohesive whole. The book proposes that ideally Islamic governments would synchronise the depletion of oil reserves with investment in new productive assets. Islamic governments could also find ways to combine private, domestic and foreign interests in the oil industry. The main readership for this book will be policy-makers and professionals involved in development issues for Middle Eastern and North African affairs, and those with an interest in oil politics and Islamic studies.
