Objectifs : Un retard de relaxation identifié par les vélocités de l'anneau mitral, associé à des profils de remplissage apparemment normaux en Doppler conventionnel serait en faveur de pressions de remplissage plus élevées. Matériel et méthode : Cette étude préliminaire a été conduite chez 10 patients à fonction systolique normale, devant bénéficier d'une intervention de revascularisation coronarienne avec circulation extra corporelle (CEC). L'enregistrement per opératoire des vélocités Doppler des flux mitraux, veineux pulmonaires et des vélocités de l'anneau mitral en mode pulsé ont été obtenus et associés à la mesure directe simultanée de la pression auriculaire gauche moyenne (POG), à l'état de base péricarde ouvert puis lors de variations de précharge standardisées grâce au circuit de CEC. L’objectif de l’étude est de tester l’intérêt de la mesure non invasive de la réserve de flux coronaire (RFC), par écho-doppler transthoracique, à la phase aigue de l’infarctus du myocarde antérieur (IDM) revascularisé. Méthode : Chez 48 patients consécutifs présentant un premier IDM antérieur, une mesure non invasive de la RFC et une échographie cardiaque standard ont été réalisées dans les 24 heures suivant la revascularisation par angioplastie. La RFC était définie par le rapport de la vitesse du flux diastolique en phase d’hyperhémie, obtenue par injection intra veineuse d’adénosine (140 μg/kg/min sur 2 minutes) sur la vitesse du flux diastolique à l’état basal. La récupération de contractilité était définie par une amélioration de 10% de la FEVG, le remodelage ventriculaire (RV) par une majoration de 15% du volume télésystolique (VTS) lors du suivi, et les évènements cardiaques par la survenue d’une insuffisance cardiaque, d’un décès de cause cardiaque, d’une revascularisation ou d’une récidive d’infarctus sur le même territoire. Résultats : Trois analyses ont été réalisées pour étudier l’intérêt de la RFC sur les différents critères (récupération de FEVG, remodelage et évènements). Les patients étaient âgés de 58 ± 13 ans, ils avaient des caractéristiques cliniques et des traitements identiques dans les différents groupes. Le niveau d’atteinte de l’IVA n’était pas différent entre les groupes, pour chaque critère. Le 2D-strain permet la quantification des déformations myocardiques et de la torsion. Peu de travaux sont disponibles sur l’apport de cette quantification pour l’étude de la viabilité post infarctus. But de l’étude : Le but de notre étude était d’évaluer la viabilité myocardique en phase aigue d’infarctus du myocarde (IDM) par l’étude qualitative et quantitative des déformations myocardiques et de la torsion du ventricule gauche (VG) par le 2D-strain.au cours d’une échocardiographie transthoracique (ETT) de repos. Matériels et méthode : 41 patients admis en USIC à l’hôpital de Compiègne pour IDM antérieur aigu et traités par angioplastie primaire ont été inclus de façon prospective (âge 61 ± 14, 39% sexe féminin). Tous les patients ont bénéficié d’une ETT de repos dans les 24 heures suivant l’angioplastie primaire, et lors du suivi (3 à 6 mois). La fonction systolique était évaluée par les méthodes classiques (FEVG Simpson biplan, score de cinétique segmentaire indexé (WMSI)) et le 2D-strain. L’analyse par le 2D-strain incluait l’étude des déformations myocardiques quantitative (strains longitudinal global (modèle à 18 segments), segmentaire, et territoire IVA) et qualitative (durée de l’élongation longitudinale systolique (L/S) exprimée en pourcentage de la durée de la systole, et raccourcissement post-systolique (PSS)), associée à une analyse de la mécanique de torsion du VG. La viabilité au niveau segmentaire était définie comme la normalisation de la cinétique segmentaire lors du suivi. A l’échelon patient, elle était définie comme la normalisation de la cinétique d’au moins 50% des segments avec trouble de cinétique initial. Résultats : Le nombre de segments avec troubles de cinétique à la phase aigüe variait de 4 à 9 selon les patients avec une moyenne de 7,7±1,2. Sur un total de 738 segments, 706 (95%) ont pu être analysés pour évaluer la fonction longitudinale. Parmi les 316 segments qui avaient des troubles de cinétique à la phase aigüe, 147 (46,5%) avaient une récupération lors du suivi et 169 n’en avaient pas. L’analyse qualitative (L/S) des déformations myocardiques apparaissait plus sensible et plus spécifique que l’analyse quantitative pure pour la recherche de viabilité. A l’échelon segmentaire, L/S, PSS, et le strain segmentaire étaient reliés à la viabilité en analyse univariée, tandis que seul le L/S était un prédicteur indépendant de la récupération en analyse multivariée (valeur seuil de 31%, sensibilité = 72%, spécificité = 73%, p 0.001). A l’échelon patient, l’âge, la rotation apicale (Ar), la torsion VG, le L/S, le WMSI, la FEVG, et le pic de troponine étaient reliéreliés à la récupération de la fonction systolique en analyse univariée, tandis que seuls Ar (valeur seuil 5.5 °, Se = 82% Spe = 74%, p = 0.01), et L/S ( 4segments avec L/S 31%, Se =83%, Spe = 74%, p
