The first is Middle East Respiratory Syndrome (MERS), an infection related to Sudden Acute Respiratory Syndrome (SARS). SARS was the first new-found plague of this century. Zika virus, which is similar to yellow fever virus in being transmitted by mosquitos, is another of the recent scourges. Zika appearing for the first time in the Americas is associated with birth defects and a paralytic condition in adults. Lastly, illness due to hepatitis viruses were observed prominently during the second World War initially associated with blood transfusions and vaccine inoculations. Since then, hepatitis virus infections have afflicted millions of individuals, in some leading to an acute fulminating liver disease or more often to a life-long persistent infection. A subset of those infected has developed liver cancer. However, in a triumph of medical treatments for infectious diseases, pharmaceuticals have been developed whose use virtually eliminates such maladies. For example, Hepatitis C virus infection has been eliminated from almost all (>97%) of its victims. This incredible result was the by-product of basic research in virology as well as cell and molecular biology during which intelligent drugs were designed to block events in the hepatitis virus life-cycle"--

The 1970s saw euphoria about the defeat of one of humankind's oldest disease scourges with the global eradication of smallpox. To set against this, the 2020s are experiencing the pandemic ravages of new viral diseases, of which COVID-19 is currently the most potent. But it is only the latest of a succession of threats. A Geography of Infection explores the distinctive spatial patterns and processes by which such infectious diseases spread from place to place and can grow from local and regional epidemics into global pandemics. This resource focuses initially on the local scale of doctors' practices and small islands where epidemic outbreaks are slight in the numbers infected and in geographical extent. Such local area studies raise two questions. First, how and where do epidemic diseases emerge and second, why do more diseases appear to be emerging now? To approach such questions implies a shift in spatial gear from painting epidemics with a fine-tipped local brush to an expanded palette on which doctors' practices and small islands are replaced by regional and global populations. By the late eighteenth century, the British medical profession was divided between traditionalists, who attributed acute diseases to the interaction of internal imbalances with external factors such as weather, and reformers, who blamed contagious pathogens. The reformers, who were often “outsiders,” English Nonconformists or men born outside England, emerged from three coincidental transformations: transformation in medical ideas, in the nature and content of medical education, and in the sort of men who became physicians. Adopting contagionism led them to see acute diseases as separate entities, spurring a process that reoriented medical research, changed communities, established new medical institutions, and continues to the present day.

Twice he was called upon to serve his country in times of crisis. During the Spanish-American War he was a key member of the Typhoid Commission which investigated the outbreak of the life-threatening fever among army recruits in southern camps. During World War I, he was a member of the medical board within the Council of National Defense which contended with an unprecedented influenza outbreak. Vaughan's professional work included more than 250 published papers and some 17 books, many outlining laboratory techniques that modernized the newly evolving field of bacteriology.

But are these, in fact, entirely new agents, or mutated forms of "old" viruses that have evolved along with us for eons? Edgar Hope-Simpson could not have written this book at a more opportune moment. He is a firm believer in gradual evolution, rather than the sudden arrival of new agents. I suspect that he would also have a naturalist's Darwinian approach for the origin of AIDS. It has been a source of some amazement to me over the years how even the most innovative scientists conform to a current hypothesis. Pioneer thinking comes more easily to persons outside the scientific mainstream. Edgar Hope Simpson has always struck me as a modem-day naturalist of the classic style, observant and perhaps a little maverick in line of thought. Certainly, the central hypothesis propounded in this book will be controversial to many scientists. From his unique citadel, the Epidemiological Research Unit in Cirencester, he has carefully reexamined mortality data from old records as well as new.
