Du fait d'une couverture diagnostique générale encore insuffisante, près de la moitié des patients dont le SAOS nécessiterait un traitement ignorent leur pathologie. Il existe possiblement une morbidité associée au SAOS, liée à l'absence de diagnostic et/ou de traitement de ces patients. Le but de cette revue est de faire le point sur les données épidémiologiques et l'impact péri-opératoire du SAOS, depuis la consultation d'anesthésie (dépistage), jusqu'à la gestion per-opératoire. Existe-t-il un fondement suffisant pour statuer si le SAOS est lié à des complications postopératoires et nécessite une gestion de l'antalgie et de la ventilation spécifique ? Les évènements respiratoires sont les principales complications observées en post-opératoire. La physiopathologie de l'aggravation du SAOS reste encore peu définie, et l'absence d'essai interventionnel disponible suffisant ne permet pas d'affirmer qu'un traitement spécifique est requis pour ces patients. La consultation d'anesthésie est, en plus d'être un moment pour dépister une pathologie cardio-vasculaire, une occasion de suspecter le SAOS. Ceci est important car la simple suspicion de cette pathologie permet d'anticiper les éventuelles difficultés de la prise en charge péri-opératoire. Nous proposons une étude (Anesthésie, Arythmies, Apnées : 3A) qui a débuté au CHU de Grenoble et permettra une approche physiopathologique par un dépistage du SAOS après la consultation anesthésie et l'analyse des anomalies électrocardiographiques dans un contexte de routine clinique post-opératoire.

Book excerpt: Il s'agit d'une étude observationnelle, prospective, monocentrique qui s'est déroulée au CHU Grenoble-Alpes (CHUGA) de juin 2015 à juillet 2018. 1104 patients ont été screenés en Consultations Centralisées d'Anesthésie (CCA) du CHUGA de juin 2015 à janvier 2018. Au total, 120 patients inclus ont bénéficié du dépistage N0, 15 patients ont été exclus en per-protocole et 13 patients dépistés « SAOS sévères ». 62 patients ont été classés dans le groupe « SAOS » et 30 patients dans le groupe « non SAOS ». 65 patients ont réalisé le suivi post-opératoire N1 et 52 patients le suivi N3. L'hypothèse principale est de mettre en évidence une augmentation de l'IDO, des troubles du rythme et des complications cardio-respiratoires en post-opératoire (le 1er et 3ème jour) chez les patients atteint de SAOS par rapport aux sujets sains. Nous nous attendons donc à retrouver une augmentation significative de la fréquence des anomalies du rythme cardiaque en lien avec les épisodes de désaturations nocturnes (IDO) des patients SAOS, associé à une aggravation de la sévérité du SAOS (IAH) en post-opératoire immédiat (N1) et au 3ème jour (N3). Secondairement, notre étude permettra d'évaluer l'efficacité d'un moyen simple de dépistage du SAOS en consultation anesthésie, d'analyser la prévalence du SAOS non appareillé et de ses complications cardio-respiratoires postopératoires, et de définir l'impact des différentes prises en charges anesthésiques (analgésiques, curares) sur l'aggravation post opératoire du SAOS.

Sudden Infant Death Syndrome, Obstructive Sleep Apnea, Ondine's Curse and various respiratory complications of general anesthesia are not so rare; as a matter of fact they happen much too frequently. The idea of organizing another symposium dealing with breathing in sleep and anesthesia has been discussed almost immediately after we said "good bye" to the Organizers of the excellent Paris meeting "The Regulation of Respiration during Sleep and Anesthesia" (R.S. Fitzgerald, H. Gautier, S. Lahiri eds., Advances in Experimental Medicine and Biology, vol. 99, Plenum, New York 1978). Taking into account the impressive amount of data that have emerged during the last few years, we have decided that we shall meet and discuss them; we hoped also that the publication of the scientific material might be useful for everybody interested in the physiology and pathophysiology of breathing, anesthesia and sleep. So we met in Warsaw under the auspices of the European Society for Clinical Respiratory Physiology and the Polish Academy of Sciences, we discussed vividly many fascinating papers presented by our Colleagues from Europe and America and Plenum Press has published the proceedings. I hope that the final result will satisfy the reader.
