Book excerpt: Introduction : les recommandations pour l'anesthésie loco-régionale aux urgences datent de 2002, et se sont suivi d'un intérêt pour son développement et sa formation, avec un net contraste dans sa pratique dans les services d'urgence où elle reste peu fréquente. Nous cherchons dans cette étude à montrer la demande et le manque de formation persistant, ainsi que la forte incidence des plaies faciales, de mains et pieds, dans ces services en comparaison avec une faible réalisation de blocs anesthésiques qui auraient toute leur place dans ces indications. Méthode : nous avons réalisé une enquête par le biais d'un questionnaire pour évaluer la formation reçue et leurs aptitudes en anesthésie loco-régionales qu'estime les médecins interrogés ainsi que les internes. Nous avons aussi étudié l'incidence des plaies notamment faciales et des extrémités de membre sur une année. Ceci est associé à une étude plus précise sur la durée d'un mois, concernant la prise en charge des plaies aux urgences d'Angoulême, notamment sur de plan de l'antalgie et de l'anesthésie locale et loco-régionale, en corrélation aux traitement local de ces plaies, tout particulièrement les sutures. Résultats : Le questionnaire aux médecins des urgences d'Angoulême a montré une différence importante entre la formation présumée à l'ALR avec 4 soit environ 26,7 % des DR et 4 soit 36,3 % des internes se déclarant formés contre une pratique courante aux urgences aux nombres respectifs de 13 soit 86,7 % et 11 soit 100 % (30,8 contre 92,3 % en moyenne). Également, on a pu constater une demande à la formation en ALR aux urgences à la hauteur de 96, 2% des personnes interrogées. Sur une année étudiée, l'incidence des plaies de la face aux urgences d'Angoulême correspond à 28,54 %, celles des mains et digitales à 39,28 %, et celles des pieds et orteils à 4,73 % environ, soit un ensemble de 72,56 % par rapport à l'ensemble des plaies hors chirurgicales ayant mené à une consultation à ces urgences. Par ailleurs leur prédominance se maintient peu importe l'époque de l'année. Sur les 214 plaies faciales, de main et pieds, vues en Mars 2018 au SAU du centre hospitalier d'Angoulême sur le mois de Mars 2018 on observe sur cette période la réalisation de 8 ALR (3,74 % des cas), réalisées 7 fois sur 8 par un interne en médecine, et une anesthésie locale par infiltration sous-cutanée réalisée dans 80 cas soit environ 37,38 % de l'ensemble des plaies étudiées. Parallèlement, 127 sutures ont été réalisées soit 59,35 % des cas étudiés, avec une prédominance de leur prise en charge par des internes en médecine. Discussion : nous avons pu constater un niveau de formation hétérogène à l'anesthésie loco-régionale avec sa pratique déclarée comme régulière mais relativement rare sur le plan observationnel, en opposition à une forte incidence des plaies, notamment de la face, des mains et digitales, dont la suture est le traitement local prédominant, sur ces plaies qui semblent éligibles à une potentielle réalisation de blocs anesthésiques faciles d'accès et d'apprentissage, et d'un intérêt évident. Conclusion : le développement de l'anesthésie loco-régionale semble avoir sa place en médecine d'urgence, et est corrélé à une forte demande notamment au niveau de la traumatologie faciale et des extrémités de membres, qui s'accorde avec une possibilité de blocs anesthésiques dont les avantages sont certains en SAU. L'adaptation organisationnelle de ces services, associée à une formation accessible et obligatoire pour tous les médecins, ainsi que les internes, à commencer par les blocs faciaux puis digitaux, ainsi que les blocs des nerfs du poignet paraît être un bon point de départ au développement de ces techniques et de leurs avantages.
