Marta looked solemnly out into the starry night. The car seemed to be hovering motionless in the sky. A strange black sky, with no ground visible. Earth was invisible far below them; it could not be seen through any of their windows. They had flown out of the shadow of Earth, thrown by their retained rotational velocity. Now Roger would keep the sun under them, charge the hull with electricity to ward off the solar wind, and pray. The thermo-electric elements kept the car cool enough, but the floor and walls were warm to the touch. The windows were edged with flame from the sunlight below. ?Where are we?? Marta asked. Roger cleared his throat. ?We?re nearly halfway to the moon, sweetie.? Luna was visible off to the right, as if on the horizon. ?Oh.? She considered the stars thoughtfully. Finally, unexpectedly, she asked, ?Are we going to be like the tigers?? Startled, Roger laughed. ?Sure. You like that one?? He used a light tone, to see if she would turn playful. Marta nodded, meeting his gaze solemnly. Sobering, Roger recited: ?The mind desires to roam, The soul aspires to soar, But Earth?s a fettered prison Without freedom?s ancient lore. My seeking mind turns outward To tropic jungles and the night, Where cold-eyed tigers freely prowl To hunt and dream and smite.? Marta said sleepily, ?It?s always night out here,? and touched her nose to his. He kissed her cheek. ?Do you know one about buffaloes?? she asked, cuddling. ?Uh, yeah, I know a song about where buffalo roam. Or used to, at least. Shall I teach it to you?? ?Yeah!? ?Okay.? Roger paused to get the lyrics straight and as he did, he reflected that buffaloes had been confined to zoos and parks even longer than tigers. ?Oh, give me a home?.? As he sang and cuddled his daughter, Roger brooded over his position and course. He had been brooding over the course for days. They were approaching turnover time for travel to the Moon or Ell Clusters, and still piling on gee. Luna, of course, was nowhere near his line of flight. He would pass near the L-4 Cluster as space distances go, but it would scarcely be visible. The transponder had beeped occasionally? it sounded off audibly when tweaked by questing beams, even as it responded. Now Roger shut off its responses. To have come out without filing a flight plan or with a silent transponder would have been a giveaway; the Patrol would have been all over him at once. Now, though, maybe he had left it on too long. As it was, the transponder had beeped every fifteen minutes or so since he had first been warned of his deviation from his LEO destination. They knew where he was, how fast he was accelerating, and in what direction. Where were they? Roger hugged his daughter, sang louder as she joined in the chorus. He couldn?t help glancing around at all the windows, expecting to see the prow of a CisLunar Patrol ship, evil menace incarnate, loom near them. If they caught him, he would lose his daughter. He could think of nothing else to do. He could only run.

We have witnessed a veritable explosion of chemical, physiologic and pharmacologic data on this hypothalamic peptide and the approximately 1500 agonist and antagonist analogs that have been synthesized. In order to track this expanding field, I was asked to organize an international symposium on basic and clinical aspects of LHRH analogs as part of the Reproductive Health Care: CDS Symposium held in Maui, Hawaii, in October 1982. This meeting brought together a number of the leading investigators in the field. Much new state-of-the-art information was presented which I and my colleagues felt deserved a wider audience. Drs Vickery, Nestor, and Hafez consented to undertake this task. Upon review of the literature, it was apparent that there was no recent text which fully covered the breadth of developments in the field. Accordingly, the editors decided to use the symposium as a nucleus on which to build a singular, comprehensive state-of-the-art analysis of this rapidly growing discipline, and the application of such knowledge to reproductive medicine. As exemplified by the various areas of expertise provided by the individual contributors, it becomes obvious that the scope of the subject matter, while relating solely to a well-defined chemical class (LHRH analogs) and a circumscribed physiologic and pharmacologic entity (reproduction), has expanded enormously.

Book excerpt: Successful cancer chemotherapy relies heavily on the application of various deoxynucleoside analogs. Since the very beginning of modern cancer chemotherapy, a number of antimetabolites have been introduced into the clinic and subsequently applied widely for the treatment of many malignancies, both solid tumors and hematological disorders. In the latter diseases, cytarabine has been the mainstay of treatment of acute myeloid leukemia. Although many novel compounds were synthesized in the 1980s and 1990s, no real improvement was made. However, novel technology is now capable of elucidating the molecular basis of several inborn errors as well as some specific malignancies. This has enabled the synthesis of several deoxynucleoside analogs that could be applied for specific malignancies, such as pentostatin and subsequently chlorodeoxyadenosine (cladribine) for the treatment of hairy cell leukemia. Already in the early stage of deoxynucleoside analog development, it was recognized that several of these compounds were very effective in the treatment of various viral infections, such as for the treatment of herpes infections. This formed the basis initially for the design of azidothymidine and subsequently many other analogs, which are currently successfully used for the treatment of HIV infections. As a spin-off of these research lines, some compounds not eligible for development as antiviral agents appeared to be very potent anticancer agents. The classical example is gemcitabine, now one of the most widely applied deoxynucleoside analogs, used for the (combination) treatment of non-small cell lung cancer, pancreatic cancer, bladder cancer, and ovarian cancer.

Book excerpt: Adult growth hormone deficiency (aGHD) is the clinical expression of a reduced GH secretion caused by congenital or acquired diseases affecting the hypothalamus-pituitary axis. Once considered a rare clinical disorder, its prevalence is apparently increasing. Nevertheless, due to the subtle clinical manifestations, aGHD could be still underestimated. Thirty years of experience with recombinant GH (rh-GH) clearly indicate the beneficial effects of replacement therapy with amelioration of metabolic and inflammatory parameters, body composition, endothelial function, quality of life, and reduction of cardiovascular risk. Furthermore, the world of GH and aGHD is rapidly enriching: new information on GH physiology, regarding its metabolic role and pleiotropic activities, is spreading, thus even making inappropriate the same name of “growth hormone”. The definition of “functional” and “partial” aGHD is still unclear and debated, although data about partially impaired GH secretion showed alteration of some metabolic and clinical parameters associated with cardiovascular risk. Current guidelines about GHD diagnosis and treatment have been elaborated, but many questions remain debated. New tests for diagnosis have recently been proposed, and non-conventional indications for diagnosis and treatment deserve further investigations. Controlled trials on the beneficial effects on morbidity and mortality are still lacking and new formulations of GH are under investigation. Several questions are related to the age of affected patients (from transition age to ageing) and no indications are available on how long the therapy should be considered. Other concerns are related to a possible pro-oncogenic effect, especially in patients who develop the deficiency after a removal of a hypothalamic-pituitary tumor. The interrelations with other pituitary axes need further clarification since isolate GHD and multiple pituitary deficiencies may have a different spectrum of manifestation. The aim of this Research Topic is to furnish deeper insight to questions related to aGHD: from molecular pathways involved in the pathophysiology to diagnostic tools and replacement therapy.
