Although researchers have explored the cause of breast cancer, data on the follow-up rate of abnormal mammogram results the knowledge and attitude of providers regarding abnormal mammography and follow-up are limited. The Stevens STAR model of knowledge transformation and Pender's health promotion model were used in this project as frameworks that integrate evidence-based findings into practice. The purpose of this project was to determine the knowledge and attitude of providers about current ACOG guidelines regarding follow-up of abnormal mammogram results, and to determine the impact of an educational intervention on the rate of abnormal mammogram result follow-up. Quantitative methodology was used for this project. The population selected included 25 providers and volunteers who work at the project site. When surveyed, 42.9% of respondents indicated that breast cancer screening guidelines from professional organizations were somewhat influential or not influential in their mammography referral decisions. The other 50% thought these guidelines were very influential. Limited data availability at the chosen project center due to a system flaw discovered during the project data collection process prevented evaluation of the impact of the intervention on follow-up rates. The project led to the adoption of an educational intervention that outlined the ACOG follow-up guidelines regarding both abnormal and normal mammogram results.

Factors, including health status, found in the cancer screening and treatment literature, are necessary in identifying variables which have the potential to affect a person's perception, and promote or deter follow-up. Protection Motivation Theory constructs utilized in this study are found in the literature to improve diagnostic health behaviors such as performing breast self-examination and complying with diagnostic tests. A non-experimental, descriptive, cross-sectional design was used to identify the barriers to follow-up after an abnormal mammogram by: 1) determining the noncompliance rate of follow-up mammograms among women screened at an urban hospital's mammography mobile unit in North Texas (October 1, 2004, to September 31, 2005) who were found to need further evaluation for suspected abnormal findings; and 2) identifying factors associated with noncompliance and perceived barriers to noncompliance. The sample consisted of 262 participants, 136 (52%) women whom the hospital reported had not returned for follow-up and 126 (48%) women who were reported to have returned. A logistic regression model was performed using follow-up as the dependent variable. The variables most related to follow-up were (1) number of mammograms in the last 5 years; (2) having health insurance; (3) having problems receiving abnormal mammogram results; (4) having problems receiving or making a follow-up appointment; (5) taking off from work for the follow-up appointment; (6) not having transportation to follow-up appointment; and (7) waiting a long time to receive the follow-up appointment. Non-compliance to recommended follow-up after an abnormal mammogram is a serious public health concern, since breast cancer screening can improve breast cancer outcomes only if prompt diagnostic resolution and access to state-of-the-art care is available to all screening participants. This study adds to the literature on predictors of follow-up after an abnormal mammogram, as well as the to the health disparities literature.

Thus, if 15% of the 48 million American women 40 years of age or older have mammograms, there would be more than 7 million abnormal mammography results each year. It has been estimated that 30% or more of women with abnormal mammograms fail to comply with follow-up recommendations. This proportion is disparate across racial groups, - such that women from minority populations are less likely to receive follow-up than white women. There is little known about why this disparity exists and a need to find out more in order to decrease the number of black women dying from this disease. This study proposed to look at this existing problem from a new perspective-that of the African American woman. The goal of this study was to improve the rates of follow-up in African American women after an abnormal mammogram result by understanding the variables that predict follow-up and developing an innovative intervention through community input that overcomes obstacles to follow-up. Thirty-nine women were interviewed about their health practices, particularly their knowledge, attitudes, and practices associated with mammogram screening. Findings indicate that mot of the women are receiving regular care and are fairly positive about the importance of early detection.

Book excerpt: Problem: There is a lack of knowledge about women who are screened for breast cancer, have an abnormal finding on mammogram, and then do not return in a timely manner for diagnostics and treatment. Lack of follow- up likely constitutes delayed treatment and poorer outcomes. Delays may result in later entry into the health system with advanced disease, more extensive and expensive care, burdening resources. Late stage breast cancer likely results in poorer health outcomes or early death. Purpose and Aims: The purpose of this research is to describe contextual characteristics at the health delivery level as well as individual characteristics of women with abnormal mammography, and their association with use of follow- up health services. Particularly, this research examines the differences between women who are early and late responders after an abnormal mammogram. Population: The participants were a convenience sample of 380 women who participated in mobile breast cancer screening. A subset of women with inconclusive or abnormal mammogram findings was the focus of analysis. Methods: This research utilized a descriptive design with quantitative data collection through participant survey at mobile mammogram screening events in multiple urban and rural Arizona sites. Participants requiring further health care were followed by chart review. Analysis of correlations with the outcome variable: time to first follow- up appointment for recommended health care in women with abnormal mammograms was conducted. Findings: Data indicated the time to the first follow- up appointment ranged from 1- 110 days with follow- up for 77.4% of participants within 60 days, 6.5% within 60-90 days, and 16.1% without follow- up after 90 days. Significant relationships between contextual and individual characteristics and follow- up were found. Categories included organizational health system characteristics of geographic location, clinical breast exam and shared case management; individual characteristics of beliefs including value of health care; finance including out of pocket costs and perceptions of financial assistance; perceived needs including breast symptoms; and satisfaction with the last health visit and mammograms in general. Implications: Recognition of barriers to follow- up after breast cancer screening is important for development of interventions to improve outcomes and has implications for screening and treatment management programs, community health centers and private practice. Health disparity related to screening without adequate options for access to health care is ethically untenable. Nurses are well positioned to reduce barriers to health care.

When coupled with appropriate treatment, early detection can reduce breast cancer mortality. At the request of Congress, the Food and Drug Administration (FDA) commissioned a study to examine the current practice of mammography and breast cancer detection, with a focus on the FDA's oversight via the Mammography Quality Standards Act (MQSA), to identify areas in need of improvement. Enacted in 1993, MQSA provides a general framework for ensuring national quality standards in facilities performing screening mammography, requires that each mammography facility be accredited and certified, and mandates that facilities will undergo annual inspections. This book recommends strategies for achieving continued progress in assuring mammography quality, including changes to MQSA regulation, as well as approaches that do not fall within the purview of MQSA. Specifically, this book provides recommendations aimed at improving mammography interpretation; revising MQSA regulations, inspections, and enforcement; ensuring an adequate workforce for breast cancer screening and diagnosis; and improving breast imaging quality beyond mammography.

Care Without Coverage examines the real consequences for adults who lack health insurance. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Most minority Americans born in the 1950s have vivid memories of 'separate and unequal' health facilities. By the 1980s, overt and blatant barriers to care were uncommon. In spite of the progress achieved, recent studies continue to provide evidence that minority Americans experience differential access to health coverage and to some health procedures. To investigate these differentials, contributors to this volume were asked to examine the health care experiences of nonelderly Hispanics and African-Americans within a nationally representative data source: the 1987 National Medical Expenditure Survey. Through this effort, the authors document the extent to which barriers to access persist and provide insight on possible explanations for variations in access. This volume will provide policymakers, practitioners, and advocates with an objective base of important information to guide decision-making about health care policy.
