Book excerpt: The National Institute for Occupational Safety and Health (NIOSH) was established by the Occupational Safety and Health Act of 1970 (U.S. Congress, 1970). Today the agency is part of the Centers for Disease Control and Prevention of the U.S. Department of Health and Human Services. NIOSH is charged with the responsibility to "conduct . . . research, experiments, and demonstrations relating to occupational safety and health" and to develop "innovative methods, techniques, and approaches for dealing with [those] problems" (U.S. Congress, 1970). Its research targets include identifying criteria for use in setting worker exposure standards and exploring new problems that may arise in the workplace. Prevention of occupational hearing loss has been part of the NIOSH research portfolio from the time the agency was established. A principal cause of occupational hearing loss is the cumulative effect of years of exposure to hazardous noise. Exposure to certain chemicals with or without concomitant noise exposure may also contribute to occupational hearing loss. Hearing loss may impede communication in the workplace and contribute to safety hazards. Occupationally acquired hearing loss may also have an adverse effect on workers' lives beyond the workplace. No medical means are currently available to prevent or reverse it, although hearing aids are widely used and research on other treatments is ongoing. Occupational hearing loss is a serious concern, although the number of workers affected is uncertain. In September 2004, NIOSH requested that the National Academies conduct reviews of as many as 15 NIOSH programs with respect to the impact and relevance of their work in reducing workplace injury and illness and to identify future directions that their work might take. The Hearing Loss Research Program was selected by NIOSH as one of the first two programs to be reviewed. Hearing Loss Research at NIOSH examines the following issues for the Hearing Loss Research Program: (1) Progress in reducing workplace illness and injuries through occupational safety and health research, assessed on the basis of an analysis of relevant data about workplace illnesses and injuries and an evaluation of the effect that NIOSH research has had in reducing illness and injuries, (2) Progress in targeting new research to the areas of occupational safety and health most relevant to future improvements in workplace protection, and (3) Significant emerging research areas that appear especially important in terms of their relevance to the mission of NIOSH.

Guided by the systems theory, to be realistic about the implementation, monitoring, as well as evaluation of outcomes of HCPs within the African mining context, this book is a call for a paradigm shift in the assessment and management of ONIHL and HCPs in African mines. This book, Occupational Noise Induced Hearing Loss: An African Perspective, equips researchers involved in the management of ONIHL and implementation of HCPs with evidence that allows for contextually relevant best practices in mine settings, particularly those located in low-and-middle-income countries (LMICs). This best practice is multidisciplinary in nature and engages all stakeholders in all relevant sectors, with the goal of adopting a preventive audiology approach to ONIHL rather than the compensation-oriented approach that is currently prevailing. This book is a research-driven contribution to the occupational health and safety (OHS) space, with ONIHL as a focus case study, and it provides contemporary, contextually relevant, and responsive evidence related to ONIHL and HCPs in LMICs with a very specific focus on the South African context. This book expansively addresses all aspects of ONIHL and HCPs in one volume, with careful considerations of complexities and challenges to HCPs implementation, applicable specifically to LMICs, although useful globally. The book offers potential solutions and recommendations for all challenges identified, having carefully and deliberately engaged with local evidence, local context, and local policies and regulations to ensure an Afrocentric contribution to the world of evidence.

Book excerpt: In September 1985, NATO sponsored an Advanced Study WOl'kshop entitled, "Noise-Induced Hearing Loss: Basic and Applied Aspects." Tne meeting was held in a mountain retreat near Lucca, Italy and was attended by scientists, clinicians, and public officials from 12 countries. This was the third in a series of such conferences organized by the authors. The first two were supported by the United States National Institute of Occupational Safety and Health; their proceedings were published as "The Effects of Noise on Hearing" in 1976 and "New Perspectives on Noise-Induced Hearing Loss" in 1982. The Organizing Committee approached NATO because it was felt that the problem of noise was common to all industrialized countries and was an especially serious problem for the military. Thus, the NATO sponsorship and the Italian site of the meeting were part of the Organizing Committee's plan to obtain an international and thorough repr'esentation on the problem of noise-induced hearing loss. The NATO meeting and proceedings followed the format of the previous two symposia with an initial focus on the anatomical and physiological disturbances resulting from noise-induced hearing loss. This was followed by sections devoted to studies of a more applied nature involving general auditory performance in noise, issues associated with the establishment of noise-exposure criteria, nonauditory effects of noise, and the interaction of noise with other agents.

The goals of the study were to measure the current noise levels which are present around employees who work within Company ABC's Number 2 napkin line and to analyze collected exposure data to determine the extent that the employees meet OSHA's hearing conservation program requirements. The review of literature included the background information of sound properties, physiology of the ear, hearing mechanisms, noise-induced hearing loss, noise measuring instruments, exposure limitations, OSHA's hearing conservation program requirements, and the hierarchy of noise control. An initial area-based noise survey was conducted to accomplish the research goals. The finding indicated that employees working within the vicinity of the Number 1 as well as the Number 2 napkin lines are likely exposed to noise levels which range from 86 to 95 dBA during an eight-hour workday and thus are subject to OSHA's hearing conservation standards. To reduce the exposure to excessive noise levels, several engineering, administrative as well as hearing protection device recommendations were provided to Company ABC.

Apart from this, there is the Malaysian perspective as written by some researchers from Malaysia. Then there are also effects of noise not only on humans but also in birds and fish. Methods to diagnosis and ways to prevent, with up-to-date researches and treatment are also discussed. Thus, it is a comprehensive one for the knowledge of all levels, professionals and non-professionals.
