There is increas ing recognition of species differences and concern that studies in animals may be misleading if unquestioningly applied to the human. A dramatic example of this occurred in the early his tory of the study of the circulation of the brain. Galen of Pergamo (131-201 AD) described a rete mirabile or "marvelous network" of blood vessels at the base of the human skull that he must have derived from observations of certain animals. This vascular structure was supplied by the carotid arteries which, after penetrating the cranium,"are divided into a large number of very small and thin branches in the region between the cranium and the dura matter. Then . . . intersecting one another they give the impression of having forgotten their way in the brain. But this is not the case. In fact, these numerous arteries rejoin and unite like the roots of a tree trunk. . . . " The authority of Galen's writings dominated scientific thought for about 1500 years. His description of a rete was unques tioned by Leonardo de Vinci, who included it in his anatomical sketches. William Harvey's remarkable observations led to his definitive account of the circulation of blood.

Book excerpt: This presentation describes structural and functional properties of the cerebral circulation that are unique to the brain, an organ with high metabolic demands, and the need for tight water and ion homeostasis. Autoregulation is pronounced in the brain, with myogenic, metabolic, and neurogenic mechanisms contributing to maintain relatively constant blood flow during both increases and decreases in pressure. In addition, unlike peripheral organs where the majority of vascular resistance resides in small arteries and arterioles, large extracranial and intracranial arteries contribute significantly to vascular resistance in the brain. The prominent role of large arteries in cerebrovascular resistance helps maintain blood flow and protect downstream vessels during changes in perfusion pressure. The cerebral endothelium is also unique in that its barrier properties are in some way more like epithelium than endothelium in the periphery. The cerebral endothelium, known as the blood-brain barrier, has specialized tight junctions that do not allow ions to pass freely and has very low hydraulic conductivity and transcellular transport. This special configuration modifies Starling's forces in the brain such that ions retained in the vascular lumen oppose water movement due to hydrostatic pressure. Tight water regulation is necessary in the brain because it has limited capacity for expansion within the skull. Increased intracranial pressure due to vasogenic edema can cause severe neurologic complications and death. This chapter will review these special features of the cerebral circulation and how they contribute to the physiology of the brain. With the aim of improving communication between several disciplines and technologies, we started to organize since 1987 in Bologna, Italy, an international Symposium on heart brain interactions to be held every 3 years. Our intention was to gather prominent clinicians and researchers from outstanding cardiologic and neuro logic institutions actively involved in the study of heart-brain interactions. The ambitious goal has been to fit different pieces of information like in a puzzle. This book originates from the contributions presented at the 2nd Sympo sium which was held in Bologna on November 30-December 1, 1990. The book is subdivided into three sections: I cardiovascular consequences of cerebral damage, II cardiogenic cerebral ischemia, III cerebrovascular and coronary artery disease.

The heart and brain have electrophysiologically active cells, which may respond in similar ways to diseases and various interventions. Finally, many drugs affect both the brain and the heart, and drugs used for heart diseases usually have side effects on the brain and vice versa. With today's increasing subspecialization in medicine, we feel the time has come to present a book that integrates basic and clinical aspects of cardiology, neurology, cardiovascular surgery, and neurosurgery. We hope this cross fertilization will broaden horizons and advance both understand ing and practice. This book is based on a symposium held at the University of TromS!/l, Norway, 24-27 June 1987, organized by Ilmar A.
