Alcoholism remains one of the most challenging problems in medical care, with far-reaching medical, social and economic consequences. For example in the U. S. , estimates indicate that 18 million people have a serious drinking problem and that the total cost to the economy of alcohol abuse is $117 billion. Treatment of alcohol dependence and other alcohol-related disorders accounts for almost 15% of the total health bill of the United States. Despite the scale of the medical problem, biomedical research on alcoholism remains something of a 'Cinderella science'. Research funding from government and other bodies is relatively poor and the number of medical scientists working in the field remains small. The Organizing Committee for this NATO ASI, comprising Charles Lieber (New York), Timothy Peters (London), Mario Dianzani (Torino), Emanuele Albano (Torino) and Norman Palmer (perth, Director), were therefore particularly grateful to the NATO Scientific Affairs Division for their active support of this AS I, the first dealing with a topic related to alcohol abuse. We moreover hope that this support will continue. The theme of the ASI was an in depth discussion of the molecular events initiated by alcohol abuse that culminate in onset of alcohol-related disease.

Book excerpt: The papers in this book represent the proceedings of the Third International Symposium, which was held at the Addiction Research Foundation in Toronto in July, 1979. The purpose of this meeting was to bring together experts in the field from a wide variety of backgrounds in an attempt to gain some clarity and insight into the problems of alcohol and aldehyde metabolism. One might ask, "Why have such a meeting and a collection of research papers?" The answer is clear. The societal problem of alcoholism is, unlike many other health problems, growing instead of abating. Treatment efforts are largely ineffective (e.g., Griffith Edwards' classic work)', and fundamental research has yet to identify rational therapy based on sound mechanisms for this disease which effects 5% of the population severely and a much greater percentage to a lesser degree or indirectly. I have become impressed with the possibility that this lack of progress may be-in large part-due to a fundamental oversight on the part of investigators in the field of alcohol research. Simply, this possible problem could be stated as follows: an alcoholic is defined the same way as all other alcoholics. Attempts to identify possible subpopulations of alcoholics with defined and treatable diseases have been minimal. One could argue, however, that little evidence, with the exception of the gross psychiatric definitions (e.g., Jellinek) exists that there are different types of alcoholics. This argument is countered, first, by the dismal lack of progress made in this field by classifying an alcoholic equal to other alcoholics, and second, by analogy with cancer research. Now we know that "cancer" is only a generic term which defines a broad group of diseases, some caused by specific environmental chemicals, others by viruses, etc. Progress was only possible after suitable animal models were developed. More importantly, however, is the fact that once specific forms of the disease (i.e., once the diseases themselves were separated from the generic term) were identified, it was a relatively easy task to develop tests for early identification of some forms of the disease (e.g., the pap smear, etc.). This analogy can be extended to alcoholism's. First, let us assume that we have failed to develop tests for early identification of alcoholisms because the population is not carefully defined into subgroups. If we assume that subpopulations of alcoholics exist, we can again turn to the cancer literature for examples of how to proceed. Much progress in this field rests. On the careful development of strains of animals which differ in their susceptibility to certain carcinogens. By analogy, distinct forms of alcoholisms could be identified by the use of genetics (e.g., Goodwin). First, distinct phenotypes need to be developed for certain characteristics suspected to be important in alcoholism. Second, a test specific for early identification of this specific phenotype can then be developed. Third, longitudinal studies must be performed in humans to see if the animal work can be applied to man. Implicit in this rationale is the thought that progress is not possible by studying alcoholics 20 years after primary changes had occurred, nor even by studying pre-alcoholics until specific sub forms of the disease are identified. Thus, the need for fundamental research into the mechanisms responsible for alcoholism is required. Unfortunately, alcohol research appears to suffer some of the stigma of the alcoholic

Book excerpt: From the time of discovery, people have used both alcohol and tobacco in various forms all around the world. Alcohol abuse and cigarette smoking causes thousands of preventable deaths in the US each year. Several lung diseases such as COPD, asthma, respiratory infections, pneumonia, acute respiratory distress syndrome, cancer of neck, throat, mouth and lung are associated with smoking and drinking. Even though some studies have examined the role of alcohol and cigarette smoke in lung diseases, only a few studies have focused on the role of reactive aldehydes generated in the lungs of those who abuse alcohol and smoke cigarettes. In this study, we showed that reactive aldehydes formed in lung can attack protein and DNA to form various adducts. Malondialdehyde, a reactive aldehydes formed from both alcohol metabolism and cigarette smoking forms malondialdehyde deoxyguanosine adduct (M1dG) with guanine base of DNA. Our study showed that this adduct is formed in both human bronchial epithelial cells and in lungs of mice exposed to alcohol and cigarette smoke. In bronchial epithelial cells, M1dG adduct induced DNA damage. Our study also showed that use of dietary supplement such as diallyl-disulfide (DADS), a CYP2E1 inhibitor, significantly reduced DNA damage and M1dG adduct formation showing the important role of CYP2E1 in M1dG adducts formation. These findings suggest that CYP2E1 plays a pivotal role in alcohol-induced M1dG adducts, and the use of DADS as dietary supplement can reverse the effects of alcohol on M1dG formation. With regard to protein adduction, our other study showed that reactive aldehydes acetaldehyde and malondialdehyde, when generated in lung in large concentrations, could form hybrid adduct malondialdehyde-acetaldehyde adducts (MAA) to various biological proteins. Our study showed that this hybrid adduct, once formed, could induce inflammation in both cell and in vivo model systems. MAA reduced macrophage phagocytic, superoxide ion and nitrite ion release functions. Also, MAA induced pro-inflammatory cytokines TNF alpha and IL-6 from macrophages. But in the absence of scavenger receptor A (SRA), previously known to bind MAA in bronchial epithelial cells, the effects of MAA were diminished; suggesting an important role for SRA in MAA-mediated changes to macrophage function. These results further support our previous finding that SRA is necessary for MAA binding. In accordance with our in vitro study, our mouse study results suggest that SRA is important for SPD-MAA induced lung injury. In the absence of SRA, there were significantly fewer total cells and neutrophils in the BAL fluid when mice were intranasally instilled with MAA for 3 weeks. In addition, there was less binding of MAA to lung epithelium and decreased neutrophil influx in the lung parenchyma of SRA KO mice. These data demonstrate that SRA plays an important role in SPD-MAA-induced inflammation in lung as evidenced by the diminished inflammation in the absence of this receptor. Taken together, our data suggest that aldehyde adducts formation in lung is important and may provide an effective mechanism through which alcohol and cigarette smoke could initiate lung pathogenesis. Also, the role of CYP2E1 in lung aldehydes generation and SRA in MAA- mediated lung effects could be helpful in developing a novel treatment pathway for alcohol abusers who smoke cigarettes.
