Book excerpt: The U.S. population of older adults is predicted to grow rapidly as "baby boomers" (those born between 1946 and 1964) begin to reach 65 years of age. Simultaneously, advancements in medical care and improved awareness of healthy lifestyles have led to longer life expectancies. The Census Bureau projects that the population of Americans 65 years of age and older will rise from approximately 40 million in 2010 to 55 million in 2020, a 36 percent increase. Furthermore, older adults are choosing to live independently in the community setting rather than residing in an institutional environment. Furthermore, the types of services needed by this population are shifting due to changes in their health issues. Older adults have historically been viewed as underweight and frail; however, over the past decade there has been an increase in the number of obese older persons. Obesity in older adults is not only associated with medical comorbidities such as diabetes; it is also a major risk factor for functional decline and homebound status. The baby boomers have a greater prevalence of obesity than any of their historic counterparts, and projections forecast an aging population with even greater chronic disease burden and disability. In light of the increasing numbers of older adults choosing to live independently rather than in nursing homes, and the important role nutrition can play in healthy aging, the Institute of Medicine (IOM) convened a public workshop to illuminate issues related to community-based delivery of nutrition services for older adults and to identify nutrition interventions and model programs. Nutrition and Healthy Aging in the Community summarizes the presentations and discussions prepared from the workshop transcript and slides. This report examines nutrition-related issues of concern experienced by older adults in the community including nutrition screening, food insecurity, sarcopenic obesity, dietary patterns for older adults, and economic issues. This report explores transitional care as individuals move from acute, subacute, or chronic care settings to the community, and provides models of transitional care in the community. This report also provides examples of successful intervention models in the community setting, and covers the discussion of research gaps in knowledge about nutrition interventions and services for older adults in the community.

Book excerpt: The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) began 40 years ago as a pilot program and has since grown to serve over 8 million pregnant women, and mothers of and their infants and young children. Today the program serves more than a quarter of the pregnant women and half of the infants in the United States, at an annual cost of about $6.2 billion. Through its contribution to the nutritional needs of pregnant, breastfeeding, and post-partum women; infants; and children under 5 years of age; this federally supported nutrition assistance program is integral to meeting national nutrition policy goals for a significant portion of the U.S. population. To assure the continued success of the WIC, Congress mandated that the Food and Nutrition Service of the U.S. Department of Agriculture (USDA) reevaluate the program's food packages every 10 years. In 2014, the USDA asked the Institute of Medicine to undertake this reevaluation to ensure continued alignment with the goals of the Dietary Guidelines for Americans. This, the second report of this series, provides a summary of the work of phase I of the study, and serves as the analytical underpinning for phase II in which the committee will report its final conclusions and recommendations.

Book excerpt: Every aspect of immune function and host defense is dependent upon a proper supply and balance of nutrients. Severe malnutrition can cause significant alteration in immune response, but even subclinical deficits may be associated with an impaired immune response, and an increased risk of infection. Infectious diseases have accounted for more off-duty days during major wars than combat wounds or nonbattle injuries. Combined stressors may reduce the normal ability of soldiers to resist pathogens, increase their susceptibility to biological warfare agents, and reduce the effectiveness of vaccines intended to protect them. There is also a concern with the inappropriate use of dietary supplements. This book, one of a series, examines the impact of various types of stressors and the role of specific dietary nutrients in maintaining immune function of military personnel in the field. Nutrient Composition of Rations for Short-Term, High-Intensity Combat Operations considers military performance, health concerns, food intake, energy expenditure, physical exercise, and food technology issues. The success of military operations depends to a large extent on the physical and mental status of the individuals involved. Appropriate nutrition during assault missions is a continuous challenge mainly due to diminished appetites of individuals under stress. Many less controllable and unpredictable factors, such as individual preferences and climate, come into play to reduce appetite. In fact, soldiers usually consume about half of the calories needed, leaving them in a state called "negative energy balance." The consequences of being in negative energy balance while under these circumstances range from weight loss to fatigue to mental impairments. An individual's physiological and nutritional status can markedly affect one's ability to maximize performance during missions and may compromise effectiveness. With the number of these missions increasing, the optimization of rations has become a high priority.

Likewise, the assembly declared the year 2015 as the “International Year of Soils” to promote awareness of the role of “healthy soils for a healthy life” and the International Union of Soil Science (IUSS) has declared 2015-2024 as the International Decade of Soils. Including legumes in cropping systems is an important toward advancing soil sustainability, food and nutritional security without compromising soil quality or its production potential. Several textbooks and edited volumes are currently available on general soil fertility or on legumes but‚ to date‚ none have been dedicated to the study of “Legumes for Soil Health and Sustainable Management”. This is important aspect, as the soil, the epidermis of the Earth (geoderma)‚ is the major component of the terrestrial biosphere. This book explores the impacts of legumes on soil health and sustainability, structure and functioning of agro-ecosystems, agronomic productivity and food security, BNF, microbial transformation of soil N and P, plant-growth-promoting rhizobacteria, biofertilizers, etc. With the advent of fertilizers, legumes have been sidelined since World War II, which has produced serious consequences for soils and the environment alike. Therefore, legume-based rational cropping/soil management practices must support environmentally and economically sustainable agroecosystems based on (sequential) rotation and intercropping considerations to restore soil health and sustainability. The gradual evolution over the last 20 years has been towards a more precise definition of the limits for occupational exposure. These have been created not only in terms of short-term effects but also more importantly in terms of long-term risks involving such problems as the potential for carcinogenesis and genetic change. In the United States the National Committee for Radiation Protection has recom mended that 5 rems (50 mSv) should remain as the maximum permissible dose equiva lent for total body exposure. This would represent the sum of internal and external ex posure and should be regarded as the upper limit allowed. The community of radiation users is required to conduct its operations in such a man ner that the absolute value of the individual's dose equivalent in rems does not exceed his age in years. There should be additional limits for tissues and organs based on short term effects. Therefore, individual organs are limited to dose equivalents low enough to ensure that the dose threshold values are not exceeded.
