The quality of care a patient receives is based on the soundness of judgment exercised by the health care team. Underlying sound judg ment is up-to-date information. What is required is not necessarily more rapid and efficient informa tion services. Modern technology can process immense amounts of data in the blink of an eye. What we in the health professions need are information systems that are more intelligent, systems that can inte grate information from many sources, systems that analyze and syn thesize information and display it so that it may be applied directly in patient care-in other words, information that answers a question or even gives practical advice. In order to accomplish such objectives, work is needed to establish the scientific and theoretical basis for the use of computing and infor mation systems by health professionals. Nurses therefore have valuable insights and unique abilities to contribute as partners with other health care professionals in improving the quality and safety of care as envisioned in the Affordable Care Act (ACA) enacted this year. Nurses should be fully engaged with other health professionals and assume leadership roles in redesigning care in the United States. To ensure its members are well-prepared, the profession should institute residency training for nurses, increase the percentage of nurses who attain a bachelor's degree to 80 percent by 2020, and double the number who pursue doctorates. Furthermore, regulatory and institutional obstacles-including limits on nurses' scope of practice-should be removed so that the health system can reap the full benefit of nurses' training, skills, and knowledge in patient care. The relationship between the caring aspects of care delivery and the use of technology can be paradoxical. Technology acceptance in nursing varies significantly depending on many factors that may influence technology adoption for nurses. Technology acceptance has historically been centered around assessing people, process, and technology factors more generally, and has not included measures specific to caring in nursing. As both an art and a science, caring is at the center of nursing theory and nursing practice, it is inherent in what nurses do with and without technology. Two studies comprise this dissertation. The first study is a concept analysis that aimed to clarify the meaning of the use of technology in nursing and explore the concept of technology acceptance in nursing. The second study is an online survey of registered nurses, which includes four aims: 1) to describe nurses' perceptions of caring and the influence of technology on care, 2) to evaluate the association of nurses' perceptions of caring with the perceptions of the influence of technology on nursing care, 3) describe the relationship between perceptions of caring and perceptions of the influence of technology based on the demographic variables of a nurses' age, educational background and years of experience, and 4) to explore the perspective and future vision for technology use in care from the lens of registered nurses actively engaged in direct patient care. Findings from this dissertation can provide additional knowledge to guide and inform technology development, nursing education and practice related to the use of technology in care, and future research for new models to measure technology acceptance in nursing that incorporate caring attributes.
