Severe AKI substantially prolongs the intensive care unit (ICU) length of stay, with increased risk of death. AKI is also a predisposing factor for multiple organ dysfunction syndrome and is usually a part of it. For these and other reasons, critical care nephrology plays an irreplaceable role in treating critically ill patients in ICUs. The current trend in medicine generally not only in critical care nephrology is to gather as much scientific knowledge as possible and in medicine, everything is interrelated. Further, knowledge of pathophysiology and understanding the causes of illness come in an era of increasing polypharmacy and the escalating side effects of drugs. The latter can cause conditions with the same clinical picture as immune system disorders, viruses and bacteria. Another issue is poisonings and drug intoxications, which are a daily issue in ICU settings. In short, the aetiological spectrum of disease has expanded. This book presents the basic principles in critical care nephrology and covers the epidemiology, pathophysiology, the most frequent syndromes, poisonings and conservative or invasive treatment options. With development of surgical and endovascular techniques, treatment outcomes have been improved gradually. However, a lot of problems still require to be resolved. Aortic diseases in the region of visceral arteries are difficult to treat, especially for aortic dissection, which is usually associated with the narrow true lumen. Conventional open repair is associated with high trauma and high morbidity and mortality. Endovascular intervention has been applied in the last decade. Fenestration technique, chimney technique, and branched stent-grafts have been used for aortic aneurysms. For aortic dissection, management in the region of the visceral arteries is more complicated, and more techniques have been described. Besides these three methods, exclusion of the re-entry by vascular occluder, road block strategy, and candy-plug technique are also reported. However, these endovascular techniques vary in characteristics, procedures, and outcomes. There is no consensus on the optimal treatment for these complicated entities.

This means that deepening our understanding of the pathobiology of oxygen metabolism in such diseases could be a fruitful path towards tangible clinical benefits. Studies in Renal Disorder collects reviews from leading researchers and clinical scientists working in exactly this field, providing an overview of the latest advances. The causal role of impaired oxygen metabolism in kidney disease has numerous clinical implications. It affects our understanding of the therapeutic benefits accruing from anti-hypertensive agents; the way we control hyperglycemia/hyperinsulinemia and hyperlipidemia; and our use of dietary approaches to the correction of obesity. The defensive mechanisms against oxidative stress, such as the Nrf2-Keap1 system, and hypoxia, such as the PHD-HIF system, have recently been explored in various cells, including kidney cells. These mechanisms include intracellular sensors for oxidative stress and hypoxia. This means that novel approaches targeting these sensors may offer clinical benefits in kidney disease in which oxidative stress and/or hypoxia is a final, common pathway.
