The cause of abdominal pain may also be located outside the abdominal region, e.g. in respiratory infections. Of non-surgical diseases, urgent treatment is indicated in severe bacterial infections (pyelonephritis, meningitis).

It is crucial to improve the diagnosis of acute appendicitis in order to prevent unnecessary surgery. In spite of technologic advances, the diagnosis of appendicitis is still based mainly on the patient's history and the physical examination. Approach to a child with a suspected appendicitis in Lebanon has rarely been studied. This study was designed to analyze the approach to children presenting to the emergency room with an acute abdominal pain and evaluate the outcomes depending on the clinical and Para clinical findings and try to elaborate a clinical scoring system for early diagnosis of appendicitis. Methods: This was a retrospective study of all children admitted to Notre Dame des Secours hospital for suspected appendicitis from January 1998 to August 2008. As there was no established protocol, para clinical exams were performed according to the judgment of the involved physicians. The results of the physical exams, blood tests, abdominal radiology, ultrasonography, CT scans and pathology were registered. Results: Among 190 children presenting for abdominal pam, 88 had appendicitis. The peak incidence of children with appendicitis was between 10 and 16 years old. The mean scoring in Samuel (mean 10.36), Alvarado (mean= 8.43) and Eskelinen (mean= 17.63) was the highest in patients with appendicitis. 66.7% of appendectomies were performed by general surgeons. Negative appendectomy rate was 5.37%. Patients treated by specialist pediatric surgeons tended to be younger and were more likely to have perforated appendicitis. Conclusion: Using clinical scoring systems amplify the exactness of the diagnosis of appendicitis. This study can be considered as a pilot study and we suggest conducting it at a national level with the aim of organizing the surgical care of children in Lebanon.
