Book excerpt: Although the anticholinesterase (anti-ChE) agents have only limited applica tions in therapy, and from the viewpoint of practical significance they are more appropriately classified as toxic compounds or insecticides than as drugs, in their capacity of pharmacological tools they have few equals. The concept of neuro humoral transmission was originally established largely from experiments in which physostigmine, or eserine, was employed to protect acetylcholine (ACh), the trans mitter of the cholinergic nerves, from rapid hydrolytic destruction by acetyl cholinesterase (AChE) and other cholinesterases (ChE's). Since then, a great num ber of additional reversible and irreversible anti-ChE agents also have been indis pensable in studies of synaptic and neuroeffector transmission, and of other physiological processes. At the same time, there is practically no other class of compounds for which a mechanism of pharmacological action can be described in such concrete biochemical and physiological terms. Consequently, it is not sur prising that a huge literature has developed on these several closely interdependent topics. The assembling anrl proper correlation of this material for the present volume has taken the collaborative efforts of over two dozen . investigators. It iR believed that their contributions to this end will prove invaluable to future in vestigators in providing a ready, inclusive source of established information, in defining areas where further studies are indicated, and in preventing unnecessary duplication of past work. How well these aims have been accomplished will be for time and the reader to judge.

Research in this area has involved the renewed investigation of the cholinomimetics physostigmine and arecoline, and has been facilitated by the development of new potential cholinomimetics such as dimethylaminoethanol (Deanol) and choline chloride. Current investigators also have taken advantage of new approaches to neurochemical studies. Increasingly sensitive assay techniques such as gas chromatography/mass spectrometry have allowed investigators to measure low concentra tions of choline and acetylcholine in brain tissues. Improved neuroanatomical procedures such as immunohistochemical staining and the use of microelectrodes and micropipets have enabled skilled investigators to begin to map central acetylcholine pathways and to dissect the component parts of the complex interactions between brain acetylcholine and other neurotransmitters. The convergence of data from both clinical and basic studies now suggest that brain acetylcholine may be involved in affective illness, several movement disorders, and some cognitive disturbances. The purpose of this book is to summarize recent clinical and pre-clinical investigations on the possible role of brain acetylcholine in several neurological and psychiatric disorders. High acetylcholine concentrations have been encountered in growing parts of plants, in the royal jelly of bees, and in the human placenta. In vertebrates, many non-neuronal cells, such as epithelial cells, endothelial cells, immune cells or spermatozoids, secrete acetylcholine in response to specific signals. Acetylcholine is better known as a neurotransmitter substance, supporting synaptic transmission in the central and autonomic nervous systems. Transmission in these places is a point-to-point process which is relatively rapid and shows many anatomical and physiological variations, according to synapses and animal species. Much more rapid is transmission in giant and specialised synapses, such as the neuromuscular junctions and their homologues, in the electric organs of certain fish. The mechanisms of acetylcholine release in these ultra-rapid junctions have been revisited in recent decades and the picture has completely changed. As the book shows, acetylcholine signalling (also called cholinergic signalling) is of particular physiological and toxicological importance, since it can be perturbed by many natural or synthetic substances.

It is a key molecule in the chemical communication between nerve cells and between nerve cell and muscle. The structure and function of muscular nicotinic receptors have been unraveled in recent years and its beauty and mysteries were reviewed in the Santorini NATO ARW organized by Dr. Maelicke in 1986. The neat, linear structure of this molecule and its conservation throughout evolution, from bacteria to humans, have led to the suggestion that it has reached the optimal structure for performing its function. But when scientists began to look at the nicotinic receptor in the nervous system, they found several surprises. From the beginning, pharmacological and physiological experiments, have made it clear that the functional characteristics of neuronal nicotinic receptors are substantially different from those of muscle receptors. Furthermore, recent sophisticated techniques such as patch clamp and gene cloning have revealed that the nicotinic receptor in the nervous system is not a single molecule but (although there are some important exceptions) a large family of similar molecules, which have in common the property of binding nicotinic agonists or antagonists. Over the last few years, the physiological implications of nicotinic receptors have been reevaluated on the basis of more precise behavioural and pharmacological techniques. Moreover, the involvement of nicotinic receptors in degenerative pathologies of the eNS, such as Parkinson's and Alzheimer's disease, has also been discovered very recently.
