Pickering served as Chairman, with Admiral Michael Mullen as Vice Chairman. Additional members were Catherine Bertini, Richard Shinnick, and Hugh Turner, who represented the IC. The criminal investigation of the September 11-12, 2012, Benghazi attacks, for which the statutory responsibility rests with the Federal Bureau of Investigation (FBI), was still underway at the time of this report. The Board enjoyed excellent cooperation with the Department of Justice and FBI throughout preparation of this report. The key questions surrounding the identity, actions and motivations of the perpetrators remain to be determined by the ongoing criminal investigation. As called for by the Act, this report examines: whether the attacks were security related; whether security systems and procedures were adequate and implemented properly; the impact of intelligence and information availability; whether any other facts or circumstances in these cases may be relevant to appropriate security management of U.S. missions worldwide; and, finally, whether any U.S. government employee or contractor, as defined by the Act, breached her or his duty."--Introduction.

Book excerpt: When Secretary Clinton transmitted the report of the ARB, the Accountability Review Board, to our committee, she noted in her accompanying letter that all of us have a responsibility to provide the men and women who serve this country with the best possible security and support. Most of all she says, ''It is my responsibility as Secretary of State.'' Tragically the Department did not meet its responsibility to our personnel in Libya. The lethal attack on our diplomatic mission in Benghazi was not the result of a protest against an obscure video as was initially claimed. Instead, and as the evidence makes clear, the attack was coordinated and carried out by terrorists targeting U.S. personnel. After the attack that killed Ambassador Stevens, Sean Smith, Tyrone Woods and Glen Doherty, the Secretary of State convened an ARB, which is required by statute due to the fatalities at the post. The ARB states that the attacks on our U.S. Nation in Benghazi on September 11th of this year were, and I quote, ''terrorist attacks.'' Contrary to initial assertions by the Obama administration, the ARB states that the attacks were security related and did not involve a protest prior to the attacks, which were unanticipated in their scope and intensity. Dispatches from the command center of the State Department's Bureau of Diplomatic Security on the day of the attack clearly reported it as a terrorist event, yet officials in Washington refused to recognize and label the attack both during and after September 11th for what it was.

Egypt remains an authoritarian state, Syria and Yemen are in the midst of devastating civil wars, Libya has descended into anarchy, and the self-declared Islamic State rules a large swath of territory. Even Turkey, which also experienced large-scale protests, has abandoned its earlier shift toward openness and democracy and now more closely resembles an autocracy. How did things go so wrong so quickly across a wide range of regimes? In False Dawn, noted Middle East regional expert Steven A. Cook looks at the trajectory of events across the region from the initial uprising in Tunisia to the failed coup in Turkey to explain why the Middle Eastern uprisings did not succeed. Despite appearances, there were no true revolutions in the Middle East five years ago: none of the affected societies underwent social revolutions, and the old structures of power were never eliminated. Even supposed successes like Tunisia still face significant barriers to democracy because of the continued strength of old regime players. Libya, the state that came closest to revolution, has fragmented into chaos, and Turkey's president, Recep Tayyip Erdogan, has used the recent coup against him as grounds for a widespread crackdown on his opponents, reinforcing the Turkish leader's personal power.
