Book excerpt: La mise en oeuvre d'un soutien professionnel est une facette importante du soin pour les patients soufrant d'une maladie d'Alzheimer ou d'un syndrome apparenté car, dans la majorité des cas, la perte d'identité et la modification des liens familiaux engendrées par la maladie, provoquent un stress traumatique pour l'aidant principal avec un épuisement (selon l'étude Pixel, 47% des aidant développent un sentiment dépressif réactionnel). Mettre en place une aide professionnelle efficace susceptible de réduire la charge psychologique s'inscrit dans l'objectif de préserver la qualité de vie de l'aidant au fur et à mesure de la progression de la maladie, mais cette aide doit nécessairement être acceptée et faire l'objet d'une négociation pour organiser la venue de tiers à domicile. Nous avons voulu connaître le type d'aides proposées à domicile en fonction du profil du patient, ainsi que leur acceptation en fonction du type d'hospitalisation et l'impact de cette acceptation sur le devenir du patient. 100 patients avec troubles cognitifs hospitalisés soit en hospitalisation de jour (groupe G1 : 50 patients), soit en hospitalisation complète (groupe G2 : 50 patients), ont fait l'objet d'un bilan répété du plan d'aide au temps T0, temps du diagnostic, au temps T1, c'est-à-dire lors de l'entrée en hospitalisation, au temps T2 au moment de la sortie, et au temps T3, soit 6 mois après la sortie par enquête téléphonique ainsi que d'une étude du fardeau de l'aidant et du devenir des patients. L'âge moyen des malades était de 80,3 ± 5,3 ans avec 87% de femmes. 85% d'entre eux souffraient d'une maladie d'Alzheimer. Le MMS moyen était à 17,4 ± 6,4, le GIR moyen à 3,8 ± 1,3. 81% avaient une pathologie cardiovasculaire associée et 37% un handicap sensoriel. L'aidant principal était un enfant dans 58% des cas, un conjoint pour 27%, un autre aidant pour 11%; enfin, 4% étaient isolés. Au temps T1: 81% des patients avaient eu des propositions d'aides professionnelles; dans 33% des situations, les aides n'avaient pas été mises en place. Au temps T2 : les aides ont été systématiquement proposées (infirmière : 97%, aide ménagère : 88%, portage des repas : 33%). Au temps T3 : les aides n'étaient pas mises en place dans 18% des cas ou partiellement dans 54% des cas. L'observance était plus élevée après une hospitalisation complète (G1 : 33% versus G2 : 82,5%). Les causes de non mise en place ou de mise en place partielle sont multiples : échec de mise en place : 12%, indisponibilité des aides : 15%, refus soit du patient, soit de l'aidant : 73%. Au terme de ce suivi de 6 mois, 43% ont été ponctuellement réhospitalisés dont 70% n'avaient accepté qu'une aide partielle, 26% sont entrés en EHPAD dont 62% avaient eu une proposition à T2 d'entrée en EHPAD, 15% sont décédés et 47% ont pu rester à domicile. Les survenues d'évènements intercurrents étaient liés à l'acceptation des aides ainsi que la fardeau de l'aidant. Une observance partielle du plan d'aide a semblé plus délétère qu'un refus total. L'aide familiale est influencée par les relations de dépendance et de protection existant dans la dynamique familiale. En acceptant les services de soutien, les aidants craignent de perdre leur liberté, le droit à choisir son mode de vie; c'est l 'un des paradoxes de l'aide proposée et refusée car « c'est sortir d'une dépendance par le recours obligée à une autre; la recherche de l'aide est toujours une cation risquée » (G. Coudin). Il paraît fondamental de prendre le temps de modifier la représentation sociale et familiale de l'aide formelle.
