Book excerpt: Using data from the 2015 Current Population Survey -- Annual Social and Economic Supplement, this analysis presents estimates of the characteristics of those remaining uninsured after implementation of the Affordable Care Act's coverage provisions. The researchers also analyze the characteristics of two subpopulations that they posit are those for whom additional outreach and enrollment efforts are likely to be most successful: those eligible for Medicaid/CHIP but not enrolled and those who are both eligible for Marketplace tax credits and have incomes below 200 percent of the federal poverty level. They present potential targeted outreach strategies based on the characteristics of these groups.

The model enables researchers to estimate anticipated enrollment in QHPs in every state when the law is fully implemented. Because new health coverage programs generally take several years to reach their full enrollment levels, in its enrollment projections Urban assumes the following "ramp-up" in enrollment from 2014 to 2016: one-third of full enrollment in 2014, two-thirds of full enrollment in 2015, and full enrollment in 2016.

Book excerpt: This paper explores several strategies states could implement beyond federal requirements, using policy decisions in 11 states--Alabama, Colorado, Illinois, Maryland, Michigan, Minnesota, New Mexico, New York, Oregon, Rhode Island, and Virginia--to illustrate the array of choices being made. While rate shock and adverse selection are potential concerns in both the small group and individual insurance markets, the authors focus exclusively on strategies in the individual market, the market most susceptible to adverse selection. Explores mechanisms intended to reduce adverse selection against the individual market in the early transition years of the reforms--those intended to address the rate shock concerns, as well as those designed to ensure stability in the individual market and the individual exchanges in the long-term. The findings indicate that study states had mixed approaches to mitigating rate shock and adverse selection, with some taking steps beyond the required federal measures but with other policy options left unexplored. Minimizing the impact of adverse selection--both against the overall insurance market and the exchanges--will require strong monitoring and oversight.

Book excerpt: This paper provides detailed data on marketplace premiums and enrollment by insurer (and by plan when available) and plan tier (bronze, silver, gold, and platinum) in California, Rhode Island, New York, Maryland, and Connecticut; sufficient data are not yet available for other states. It also finds that individuals' choices, both in choosing silver or bronze plans and in selecting the lowest-cost plans within those tiers, are heavily determined by price. But considerable evidence suggests that significant numbers of consumers choose plans based on other factors, such as previous coverage with insurers, name recognition, perceptions of customer service, and perceived breadth of provider networks.

Book excerpt: The federally facilitated health insurance marketplace (FFM) is attempting to improve consumers' ability to make plan-to-plan comparisons during the 2017 open enrollment season by encouraging insurers to offer standardized benefit designs. In doing so, the FFM is following the path of several state-based marketplaces (SBMs) that require insurers to offer standardized health plans, although the FFM and most SBMs also allow insurers to offer nonstandardized options. Through an analysis of policy guidance, consumer-facing marketplace websites, and interviews with state officials and key stakeholders, this paper explores the experiences of SBMs in Connecticut, Massachusetts, New York, and Oregon that have required participating insurers to offer standardized plans. The authors find that although broad consensus exists among state officials and stakeholders that the primary goal of health plan standardization is to facilitate "apples-to-apples" plan comparisons, these states' policy choices and website interfaces have curtailed their ability to achieve these stated goals. In particular, by allowing insurers to offer nonstandardized options in addition to standardized options and failing to use web-based decision support tools to differentiate between plan options, consumers in these SBMs have limited ability to conduct the plan-to-plan comparisons as originally envisioned by policymakers.

Book excerpt: Studies before the ACA's implementation in 2014 found that veterans were less likely than the general population to be uninsured: 1 in 10 nonelderly veterans neither had comprehensive health insurance coverage nor used health care available through the Department of Veterans Affairs (VA) (Chokshi and Sommers, 2014; Haley and Kenney, 2013, 2012). Some uninsured veterans may qualify for VA care, but not all take up the available coverage or meet the eligibility requirements, which are based on service-connected disability status, veteran discharge status, income, and other factors (Panangala, 2015). The ACA's new options offered veterans the potential to gain coverage through increased Medicaid enrollment, enrollment in VA care, or participation in the new marketplaces. Before 2014, an estimated 4 in 10 uninsured veterans had incomes below 138 percent of Federal Poverty Level. Uninsured veterans in that income group living in states that expanded Medicaid would qualify for Medicaid in 2014 (Haley and Kenney, 2013).

Book excerpt: This report examines how plan selection rates vary across states and income groups with information available from the 37 states that used HealthCare.gov. It find that across all states using HealthCare.gov, plan selection rates among the tax credit eligible population declined with higher incomes, ranging from a median rate of 62 percent for incomes below 200 percent of the federal poverty line (FPL), 29 percent for those with incomes between 200 and 300 percent FPL, to only 13 percent for those with incomes between 300 and 400 percent FPL. The consistency across states in the estimated plan selection rates within these income groups indicates much lower willingness of eligible people to pay for health coverage with the ACA's tax credits among those with incomes above 200 percent and particularly above 300 percent of the FPL. The limits of this willingness to pay could make increases in future enrollment difficult to achieve without policy changes to improve affordability.
