Book excerpt: Excerpt from The Acute Abdomen The first edition of Lectures on "The Acute Abdomen" having become exhausted some months ago, I was requested by the publishers to write a second edition, with such alterations and additions as might seem advisable. The continued advance of our knowledge of the acute conditions which may arise in the abdomen has necessitated an increase in size of the volume, whilst it has appeared desirable to add the accounts of many cases and illustrations with the view of more thoroughly elucidating the subject. So many acute abdominal diseases have proved curable by operation, when it is performed early, that I hope my readers will not consider the subject unduly magnified considering its importance in modern practice. The treatment of the acute abdomen (including that of lesions the result of traumatism) demands an acquaintance with the surgical part of the subject which cannot always be obtained in text-books on surgery. In addition, it may be added that in order to be of general application the methods described should be as simple as possible, so that no practitioner need fear to make an attempt to save a threatened life because he has not some particular clamp, bobbin or brand of catgut in his possession. Silk is still recommended, because it is strong and can be trusted. Moreover, it is easily sterilised by boiling, even in a cottage. The principal additions to the present volume, which is divided into sections, are those on the planning of abdominal incisions and injuries to the abdomen, external and internal, including most of the subject-matter of the Oration given before the Medical Society of London in 1910. It is simple unadulterated nonsense to tell me that I could have done with safety, through the thickness of the tissues of the abdominal wall and vagina, that which I was barely able to do with my fingers directly on the disease, and the patient fully anaesthetized and massage, if carried to the extent'advocated by some of its votaries, loses even the merit of being harmless, but becomes extremely dangerous. If the patients of the gentlemen who use this remedy (or say they do) will submit to the pain necessarily incident to this treatment, they must be dealing with a much less serious type of cases than it has been my fortune to handle. The book is offered as a contribution towards the elucidation of the disorders and diseases of the stomach and other abdominal viscera as these have to be dealt with by the family practitioner, and by the hospital and consulting physician. The writer was interested in these matters before the advent of abdominal surgery, but the opening of that epoch gave a motive and an impetus to abdominal diagnosis which had previously been lacking. That diagnosis could be tested on the operating table, instead of the post-mortem-room table only, gave a significance to abdominal diagnosis which had been largely wanting. As a teacher in a great medical school, and as a physician to a great general hospital, where every type of disorder and disease was admitted into the wards, it was a duty and a privilege, not only to treat sick people, but to expound to students the methods and the steps by which diagnosis was reached, and on which treatment was based. The advent of abdominal surgery opened up a new field for investigation, and for teaching internal diagnosis, of which the writer took as full advantage as circumstances allowed. As time passed, and experience was tested and re-tested, the problems became simpler and the points essential to correct diagnosis clearer. This led to simplification in teaching, and this book represents more or less fully the position reached. There is little in it which has not been taught and re-taught by the writer as abdominal cases were admitted to his wards. Book excerpt: Excerpt from Ulcer of the Stomach The ultimate object of our investigations into the causes of, and abnormal processes concerned in, a disease is the scientific treatment of our patients, the aim of which is the restoration to the normal of the morbid processes constituting the malady. In order to do this, it is essential to know what these processes are, and the relative part that each individually plays in the production of the clinical phenomena of the disease. The physician is unable effectively to study these processes of disease in man, because, although our methods of clinical research have vastly improved during the past few years, they are yet very limited, and particularly those dependent upon experiment and not upon mere observation. Again, when the case comes under observation, so many processes are usually disordered together that it is impossible to say which are of primary and which of secondary importance. It is, therefore, essential in order to study the causes of, effects of, and phenomena exhibited by isolated morbid processes at various stages in their course, to resort to experiments upon animals; but unless the anatomical basis of a disordered process in the human being has been previously ascertained, one has no idea how to reproduce the condition in an animal. The study of morbid anatomy and clinical medicine is thus inextricably blended with that of experimental pathology. It is true that certain pathological problems can be studied exclusively in the laboratory, but these are merely parts of the whole, and eventually must be correlated with the facts obtained through a study of the patient. The pathological and clinical aspects of a disease must, therefore, be studied side by side, otherwise our knowledge of it can never be precise and complete. The manifestations of a lesion of the nervous system can be observed with accuracy, and its site determined largely from a knowledge of anatomy and physiology. In the case, too, of disease affecting the heart or lungs the methods of physical examination are usually adequate to enable us to determine the seat, and to some extent the nature, of the lesion. But in the investigation of abdominal diseases the means at our disposal are more limited in their application; and in making a diagnosis we have to fall back - to a greater extent than in the case of disease else where - ou our knowledge of pathology, and on our former experiences of cases presenting a clinical history similar to that of the case under considera tion. Hence the necessity for giving careful attention to every abnormal phenomenon which is essential in the investigation of disease in any part of the body, is especially called for in the case of disease situated in the cavity of the abdomen. The object of the present article is to take a brief survey of the main points to be attended to in the investigation of an abdominal case, and the subject will be considered under the following headings -(a) The symptoms from which the patient suffers, together with facts relating to his previous history; (b) The physical examination of the patient; (c) The examination of the excreta, of any vomited material, and of fluids obtained by exploratory puncture.
