Motivational approaches are based on the principles of person-centered counseling. Counselors' use of empathy, not authority and power, is key to enhancing clients' motivation to change. Clients are experts in their own recovery from SUDs. Counselors should engage them in collaborative partnerships. Ambivalence about change is normal. Resistance to change is an expression of ambivalence about change, not a client trait or characteristic. Confrontational approaches increase client resistance and discord in the counseling relationship. Motivational approaches explore ambivalence in a nonjudgmental and compassionate way.

Book excerpt: In spite of a massive national investment in alcohol education, the evidence clearly demonstrates that our current efforts are overwhelmingly ineffective and are often counterproductive. The assessment of the effectiveness of alcohol programs over the past 15 years reveals that those based on the responsible-use approach tend to be superior to those based on the abstinence approach. Hanson puts the historical relationship between Americans and alcohol into perspective, discusses federal policies on alcohol that are highly ideological and biased, and advocates the development of responsible-use curricula that will prepare students to be, as adults, responsible consumers of alcohol. While alcohol has been praised as a sign of refinement, a source of relaxation, and a complement to good times, it has also been cursed as the cause of disease and death. The temperance movement, which taught that alcohol was poison and that moderation in its use was no more possible than moderation in murder, has greatly influenced American thought and is reflected in our abstinence-oriented approach to alcohol education. Current programs reflect not only these abstinence origins but the inclusion of drug education several decades ago. Hanson finds that, despite good intentions, evidence indicates that this approach has proved ineffective. He considers the strong and often mixed feelings Americans have toward alcohol that are reflected in ever-changing public policy and concludes that the development and evaluation of alcohol education programs should be based on the best scientific evidence available rather than on political ideology. Recommendations for change in alcohol education are presented, with the most important being that we must promote the development and evaluation of diverse responsible-use curricula with the goal of reducing alcohol abuse.

Moreover, a pattern of heavy drinking established in adolescence and young adulthood may continue into an adult pattern of alcohol abuse. Concerned communities and institutions across the nation are tackling the problem of alcohol use and abuse by young people. Research-based knowledge is urgently needed to inform these efforts and to ensure that limited prevention resources are used as effectively as possible. The origins of youthful alcohol use and abuse are found within the complex interplay of individual characteristics, family and peer influences, the larger societal context for alcohol use, environmental conditions, and maturational processes that accompany adolescence. This volume, which began as a special issue of the Journal of Research on Adolescence, contains all of the material from the journal issue plus additional chapters. It helps researchers to meet the tremendous challenge of disentangling the key determinants of risk, and developing effective interventions. Primary sources of influence on youthful alcohol use are described, ranging from individual expectancies about alcohol effects and cognitive decision processes to parenting practices, peer influences, social environments, and economic factors; and a corresponding range of prevention interventions is discussed. This book will serve as a primer to those with an interest in developing and improving effective programs and activities to reduce alcohol-related problems among young people. For those engaged in prevention research, the text will provide useful reviews and current findings that should aid in directing future research activities.

The goal of REACH is to prevent drinking and driving. The authors conducted a randomized clinical trial evaluating how helpful REACH was compared with existing DUI groups in program for individuals with a first-time DUI offense, and they found that clients who received REACH reported lower odds of impaired driving upon program completion than did clients in existing groups. This manual presents the 12-session REACH protocol used in the research study. Each session is interactive and encourages clients to share ideas and information; examine the thoughts, feelings, and behaviors that put them at risk for future drinking and driving; learn or enhance existing coping skills; and provide fellow group members with peer support.

In 1999, the Australian Institute of Criminology hosted a Roundtable on the theme of Alcohol, Young Persons and Violence. Its purpose was to highlight new empirical data, to explore innovative interventions and to inform the development of a National Alcohol Strategy. This publication gathers together nine papers from that Roundtable as well as others related to the subject of alcohol, young persons and violence. These papers do not attempt to provide a comprehensive description of the extent and nature of the 'problem', nor do they offer a prescriptive policy solution. They do, however, provide examples of recent research and policy opportunities in the area and offer evidence which will contribute to the development of the National Alcohol Strategy.

The specific areas examined included alcohol, speed, seat belts and signalised intersections. The review documents the types of traffic enforcement methods and the range of options available to policing authorities to increase the overall efficiency (in terms of cost and human resources) and effectiveness of enforcement operations. The review examines many of the issues related to traffic law enforcement including the deterrence mechanism, the effectiveness of legislation and the type of legal sanctions administered to traffic offenders. The need to use enforcement in conjunction with educational and environmental/engineering strategies is also stressed. The need for publicity to support enforcement operations, police training and education programs is also documented. The review concludes with a series of recommendations regarding the most promising options available to authorities to improve the effectiveness of traffic law enforcement operations.
