Available in PDF, EPUB and Kindle. Book excerpt: Prevention of ventilator associated pneumonia (VAP) includes routine oral care of the mechanically ventilated patient. Oral care is recognized as an essential component of patient care and nursing documentation provides evidence of this process. This study examined the frequency of oral care in ventilated and non ventilated patients. A retrospective chart review of patients admitted to an intensive care unit (ICU) between July 1, 2007 and December 31st 2007 was conducted. Data were analyzed using logistic regression to determine which variables if any were related to the likelihood of the patient receiving oral care. Frequency of oral care documentation was found to be done at an average of every 2.91 to 3.44 hours with a range of 1-8 hours suggesting inconsistencies in practice. This study also found that the nursing critical care flow sheet did not provide an opportunity for detailed documentation of oral care.

Book excerpt: Based on documented studies, ventilator-associated pneumonia (VAP) is a leading nosocomial infection in critical care settings. In a critically ill patient, the development of VAP significantly increases their risk of mortality, length of hospital stay, and decelerates the speed of recovery. Research has shown a substantial correlation between poor oral hygiene in mechanically ventilated (MV) patients and the development of VAP. The implementation of an oral hygiene protocol and/or order-set increases nurse adherence to performing these interventions, improves quality of oral care practices, and ultimately reduces the incidence of VAP. The successful implementation of an oral care protocol requires thorough preparation. Components of implantation will include gathering of evidence, literature review, stakeholder presentation, involvement and education of intended staff, actual implementation, evaluation plan, and dissemination of evidence. The effective implementation of an oral care protocol in intensive care units for MV patients will decrease the incidence of VAP, reduce hospital stays, and decrease costs.

Oral care instructions and follow-up actions have been proved its effectiveness and efficiency. Meanwhile, high oral care compliance rate is associated with better patients' outcomes, shorter intensive care unit stay and hospitalization, lower mortality rate, and financial benefit for a hospital. Implementing specialized education for critical care nurses with systematic follow-up actions can increase the compliance rate with all mentioned above positive outcomes. This project is focused on comprehensive literature review, clinical observation of current practices and policies, and proposal of improvement actions.

Book excerpt: This study is a comprehensive literature review and clinical observation of current practices and policies and the proposed method of different oral hygiene in reducing the incidence of ventilation- associated Pneumonia. This condition (VAP) is a huge concern in the intensive care units as a result of infection which occur few days after intubation with endotracheal tube. It can be very challenging taking care of these type of patients due to the multifaceted health issues, particularly if their respiratory system is already compromised. Oral hygiene is considered to be an important intervention, in combination with other strategies, for the prevention of ventilator-associated pneumonia. Data suggest that the simple reduction of risk through better oral hygiene can lead to fewer incidence of Ventilator-Association Pneumonia. In addition to brushing, providing oral moisturizer to oral mucosa and lips every 2 to 4 hours is often a component of the oral care protocol. Implementation of an oral care protocol, ongoing nurse education, and evaluation were important in reducing the incidence of VAP. The use of teamwork tools and strategies to ensure this consistency can have a tremendous impact on closing the gap, improving patient outcomes, shorter length of stay in the intensive Care Unit and reduction of health care cost.

Book excerpt: Non-ventilator associated hospital acquired pneumonia (NV-HAP) is a leading cause of nosocomial infections in the non-intensive care acute care setting. The purpose of this performance improvement project was to implement enhanced oral care standards, verified through ongoing documentation and point-of-care surveillance, to promote a nursing practice change at the bedside and improve quality outcomes for the patient. The Plan-Do-Check-Act (PDCA) model provided the framework for this planned intervention. Using a multidisciplinary approach, a detailed curriculum and educational delivery plan were developed to prepare staff on the pilot unit for implementation of new oral care standards. Documentation of oral care delivery improved throughout the course of the pilot project. Direct observation monitoring revealed positive results in patient's mouth and oral mucosa moistness, cleanliness, and plaque-free state. Oral care is a fundamental, basic nursing intervention. This nurse-led practice change empowered staff to help reduce the risk of patient acquiring NV-HAP during their hospital stay. By incorporating enhanced oral care into daily practice, nursing staff are in a position to impact clinical outcomes and the health status of patients under their direct care.
