Book excerpt: The prison population in England and Wales has been increasing since the 1990s and by November 2005 it reached a record level of 77,800, resulting in increased levels of overcrowding and stretched resources. Following on from a NAO report (HC 458, session 2005-06 (ISBN 0102935696) published in October 2005, the Committee's report examines how the Home Office, the Prison Service and the National Offender Management Service (which has responsibility for managing and accommodating prisoners) are dealing with the challenges involved in accommodating this record number of prisoners, the construction and use of temporary accommodation and the impact on the delivery of education and other training for prisoners. The Committee makes a number of conclusions and recommendations including in relation to: the deportation of foreign nationals, the use of alternatives to remand such as electronic tagging, contingency planning to ensure greater flexibility in accommodation plans including pilot testing new accommodation to identify possible problems early on, the application of best practice in anti-suicide monitoring measures, and the impact of moving prisoners around the prison estate on their training needs.

Book excerpt: The National Offender Management Service directly manages 117 public prisons, manages the contracts of 14 private prisons, and is responsible for a prisoner population of around 86,000. It commissions and funds services from 35 probation trusts, which oversee approximately 165,000 offenders serving community sentences. For 2012-13, the Agency's budget is £3,401 million. The Agency achieved its savings targets of £230 million in 2011-12 and maintained its overall performance, despite an increase in the prison population. However, the Agency's savings targets of £246 million in 2012-13, £262 million in 2013-14 and £145 million in 2014-15 are challenging. The Agency believes it has scope to make the prison estate more efficient by closing older, more expensive prisons and investing in new ones. These plans, however, assume the prison population will stay at its current level. Furthermore, the Agency has not yet secured the up-front funding for the voluntary redundancies needed to bring down prison staffing costs. Unless overcrowding is addressed and staff continue to carry out offender management work it is increasingly likely that rehabilitation work needed to reduce the risk of prisoners reoffending will not be provided. The Agency has not done enough to address the risks to safety, decency and standards in prisons and in community services arising from staffing cuts implemented to meet financial targets. The Agency plans to increase the role of private firms and the third sector in probation but the probation trusts don't appear to have the infrastructure and skills they need to commission probation services from these providers effectively

Book excerpt: The National Offender Management Service (the Agency) is responsible for the prison system in England and Wales which holds around 84,000 prisoners. The prison estate consists of some 130 prisons of varying layout, geographical location, age and state of repair. The main factor behind the Agency's estate strategy, of closing small costly prisons and building new accommodation that is cheaper to run, is the need to make recurring savings. Under the strategy, the Agency had by the end of 2013, closed 13 prisons and built two new prisons. The estate strategy's objectives are to reduce resource costs; create durable, good standard accommodation and provide an estate that better meets offenders' needs, allowing more of them to work and be kept closer to their homes. Against these objectives, the Agency has built new, good quality, accommodation to time and within budget; is on track to achieve cost reductions of £70 million a year; and is starting to match better the geographical spread of prisons to the prison population. Key factors behind this good performance are that senior staff in the Agency have experience and knowledge and have remained in post throughout this period of change. However, the performance of the two new prisons, HMP Oakwood and HMP Thameside, has been poor and disappointing since they opened. They do not give sufficient priority to meeting offenders' rehabilitation needs, nor do they provide enough quality purposeful activity for prisoners. The Agency has also closed some prisons that were performing well.

Book excerpt: The Criminal Justice Act 2003 introduced a new style of community sentence known as a community order. For offences committee after 1 April 2005, magistrates and judges have been able to tailor communty sentences to the severity of the offence and, at the same time address offending behaviour. The most common order contains a single requirement obliging the offender to complete specified number of unpaid work hours. The National Probation Service supervises all offenders subject to such orders, plus those released from prison on licence or given other sentences to be served in the community. This report examines how well they have been implemented and whether they are meeting sentencing objectives. It is felt the National Probation Service could improve efficiency by increasing the consistency with which community orders are implemented within local Probation Areas. Better data on capacity, costs and the number of orders completed as sentenced would help the Service demonstrate value for money. The report makes several recommendations to this end.

However there are risks and treatments can go wrong. A report by the Chief Medical Officer in 2000, ('An organisation with a memory', ISBN 0113224419) estimated that one in ten patients admitted to hospital were unintentionally harmed and that a blame culture and lack of a national system for sharing experience were key barriers to reducing the number of patient safety incidents. In Government's response included plans, timetables and targets to promote patient safety and the establishment of the National Patient Safety Agency. This report finds that insufficient progress has been made. In particular there is a question mark over the National Patient Safety Agency because of cost over-runs and delays in its National Reporting and Learning System and the limited feedback it has so far provided to trusts.

Book excerpt: Following on from a NAO report (HCP 798, session 2005-06, ISBN 0102936978) published in February 2006, the Committee's report concludes that the handling of cases in magistrates' courts has in recent years become complex and protracted to the extent that it no longer amounts to summary justice. 55 per cent of the £173 million cost of delay in the magistrates' courts is attributable to the defence, but the police and the Crown Prosecution Service (CPS) account for another 14 per cent (£24 million) each. The CPS needs to review its organisational structure, revise its system for preparing for magistrates' court cases by adopting current best practice, and address the cultural resistance within the organisation to more modern working practices.

Book excerpt: Strokes are one of the top three causes of death in England and a leading cause of adult disability. There are 110,000 strokes each year in England, with a quarter occurring to people under 65 years. Some 300,000 people in England are living with moderate to severe disabilities as a result of a stroke. As the NAO report on this subject pointed out (HCP 452, session 05/06 NAO ISBN 010293570X), it costs the economy in total about £7 billion a year, with the direct cost to the NHS about £2.8 billion. This Committee of Public Accounts report takes evidence from the Department of Health and sets out a number of recommendations. The cost of stroke, in both economic and human terms, could be reduced by re-organizing existing services more effectively. Brain scans of many stroke patients are being delayed, everyone who suffers a stroke should be scanned as soon as possible after arrival in hospital, and should not wait more than 24 hours. Stroke patients should spend longer in hospital on a stroke unit, this could reduce the number of deaths. There needs to be an increase in the number of consultants who have training in dealing with strokes, as well as therapists and other specialist staff with expertise in stroke care across the primary and secondary healthcare sectors. The Department of Health should improve provision of information to stroke survivors and carers, so they are made more aware of the support services available. The Department should run an awareness campaign to improve public knowledge about strokes
