Book excerpt: This book is about the substitution treatment of a missing 1st molar with protraction (forward movement) of the 2nd and 3rd molars by orthodontic space closure. It is one of the most recent treatment methods using miniscrews (TADs: Temporary Anchorage Devices). In cases of missing teeth--not only the 1st molar but also other teeth--an implant or prosthetic treatment is usually necessary. However, what if the patient considers receiving orthodontic treatment after an implant or prosthetics? Orthodontic treatment would become very difficult because an implant or prosthetics would prohibit orthodontic teeth movement. Figures on the front cover of this book show that the 2nd and 3rd molars have mesially drifted to the missing 1st molar space. The formerly impacted (unable to come in) 3rd molar erupted (came in) properly. Therefore, the 2nd and 3rd molars can function as the 1st and 2nd molars, respectively. In this case, troublesome extraction of the impacted 3rd molar and an implant or prosthetic treatment was unnecessary. Is this possible in all cases? If yes, then to what extent? Would there be any other problems in the periodontal and root structure? The substitution treatment of a missing 1st molar may also have its own risks, but if successful, it can largely benefit patients. This book is an organization of the 110 cases that I have treated, discussing both the positive and negative aspects of the concerned treatment. In addition, this book highlights missing space closure of the 1st molar because it is the most difficult type of orthodontic space closure. The missing space of other teeth is generally easier to close. Prosthodontic, endondontic, or periodontic doctors may have more chances to meet patients who have missing teeth than orthodontic specialists. I'd like to suggest that these doctors take this orthodontic treatment into consideration. Patients, as well as dentists, must be aware of various treatment options, including orthodontic treatment methods. I hope it can be a good option for missing teeth.

Book The Biology of Tooth Movement

Download or read book The Biology of Tooth Movement written by Louis A. Norton and published by CRC Press. This book was released on 1988-11-30 with total page 434 pages. Available in PDF, EPUB and Kindle. Book excerpt: :Written by a broad spectrum of dental, medical and basic science researchers from around the world, this book presents state-of-the-art knowledge concerning the biology of connective tissues and their response to exogenous mechanical stimulation at the cell biology level. The text goes well beyond the traditional morphologic descriptions of tooth movement, covering the cell biology of the connective tissues involved, the various in vitro and in vivo research models, possible pharmacological means of influencing tissue responses, and biophysical considerations. Many cellular events that occur during tooth movement are discussed, as well as the exciting challenges, unanswered questions and possibilities in the future. This publication is extremely relevant to the work of dental specialists in orthodontics, pediatric dentistry, and periodontics plus orthopeadists and basic scientists working in connective tissue research.

Book Evaluation of Transverse Bodily Tooth Movement and Its Effects on the Surrounding Hard Tissue

Download or read book Evaluation of Transverse Bodily Tooth Movement and Its Effects on the Surrounding Hard Tissue written by Chad Jason Capps and published by . This book was released on 2015 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: Orthodontic expansion has been associated with uncontrolled tipping and alveolar bone loss. Recent research evaluating orthodontic expansion has shown osteoblastic activity on the buccal cortical bone apical to the dehiscence. We hypothesize that the negative effects seen during orthodontic expansion is a result of tipping rather than expansion. The aim of the present study was to produce buccal translation, with little or no coronal tipping, and evaluate hard tissue changes of the dental-alveolar complex. A prospective, randomized, split-mouth study was conducted with 11 patients (average age 14.1 years, range 12.5-16.9 years) requiring maxillary first premolar extractions for comprehensive orthodontic treatment. Pre and post-treatment records included models, photographs, and small field of view cone beam computed tomographic images (FOV CBCT) of the right and left maxillae. One maxillary first premolar was randomly chosen and moved to the buccal with 50 grams of force applied approximately at the tooth's center of resistance. The other premolar served as the control tooth. Forces were re-activated every 3 weeks for approximately 9 weeks of active movement, after which the tooth was held in place for 3 weeks to allow for maturation of the surrounding tissue. Pre and post treatment records were analyzed and superimposed to evaluate changes in the dental-alveolar complex. The results showed significant movement (0.96 mm, p=0.008) of the experimental premolar occurred 3 mm apical to the CEJ. There was minimal buccal tipping (2.2°, p=0.003). Maximum and minimum buccal bone heights decreased 0.60 mm (p=0.003) and 0.25 mm (p=0.262) respectively. The distribution of the maximum bone height measurement was bimodal, with 6 patients showing 0.42 mm (IQR -0.25 mm to 0.52mm) and 5 patients showing 8.3 mm (IQR 7.15 to 10.05) of vertical bone loss. Buccal bone thickness at the midline 3 mm apical to the CEJ decreased 0.63 mm (p=0.016). Based on direct measurements and CBCT superimpositions, buccal bone grew 0.46 mm (p=0.005) and 0.51 mm (p=0.036), respectively. Using light continuous forces, it is possible to produce buccal tooth movement with only limited amounts of tipping. With such movements buccal bone growth occurs, but there are potential limitations. The electronic version of this dissertation is accessible from http://hdl.handle.net/1969.1/152699
