Book excerpt: Excerpt from The Retrospect of Medicine, Vol. 16: Being a Half-Yearly Journal, Containing a Retrospective View of Every Discovery and Practical Improvement in the Medical Science; July December, 1847 It is of great consequence to be able to diagnose the two forms of fever from each other as soon as the patients are examined. An experienced eye can almost always form a correct opinion: he will recognize in the flushed countenance, strong bounding pulse, and white swollen tongue, the simple continued fever; whilst the de pressed appearances of the patient, the quick small pulse, the tongue nearly clean, slightly covered with a light yellow hue in centre, and the great prostration of strength, are sure indications that the patient is seized with typhus. Book excerpt: Fall from Grace is a candid, personal history of an academic physician and biotechnology executive that reflects on medicine as it was in the mid-twentieth century and chronicles the changes in society and medicine during the second half of that century. The book investigates the social revolution of those times; the scientific and technological advances that occurred; the influence of the computer and the digital revolution; the entry of corporate management into health care; and the effects of the profit motive on the care of patients. All of these have had enormous influence on the role of the physician in health care. The inadequacies, over the years, of the fee-for-service system and the consequent governmental involvement in reimbursement systems are discussed and compared with other health care payment systems around the world. The net effect of these various forces has been to benefit patients through greatly improved technology yet has caused medicine to evolve from an art form focused on personal care to a more technical exercise largely controlled by fiscal considerations. These changes also refashioned the role of the physician from healer and counselor into manager of an impersonal health care team. The book provides a view of the current state of medicine, patients, and physicians and a perspective on the future.

Book The Retrospect of Medicine Vol 28

Download or read book The Retrospect of Medicine Vol 28 written by W. Braithwaite and published by Forgotten Books. This book was released on 2017-12-18 with total page 526 pages. Available in PDF, EPUB and Kindle. Book excerpt: Excerpt from The Retrospect of Medicine, Vol. 28: Being a Half-Yearly Journal, Containing a Retrospective View of Every Discovery and Practical Improvement in the Medical Sciences; July-December, 1853 The specific gravity of a cubic inch of air is '185, that of water is 1000, and that of blood 1050. Forty cubic inches of air weigh about 12980 grains; the same amount of blood weighs about grains. Thus, the latter fluid is somewhat 865 times heavier than air; or, as '185 is to 1050. The amount of air, as it passes into the lungs, is op posed by a column of blood coming from the heart, in the proportion of one inspiration to four pulsations of the right ventricle. Each inspira tion carries into the pulmonary cells 40 cubic inches of air, and the average number of inspirations in an adult is 20 per minute. Each ventricular contraction throws two ounces of venous blood into the pull monary cells; and, as there are four contractions to one inspiration, about eight ounces of blood is therefore dispersed through the pulmonary capillaries, from minute to minute. Available in PDF, EPUB and Kindle. Book excerpt: Excerpt from The Retrospect of Medicine, Vol. 46: Being a Half-Yearly Journal, Containing a Retrospective View of Every Discovery and Practical Improvement in the Medical Sciences; July-December 1862 In bloodletting you are wielding a dangerous, but in some cases a necessary weapon. When it should be employed and when not, is one of the most important questions in clinical medicine. We must judge of the necessity for this treatment by the balance between the heart and the arteries. If the apex of the heart strikes strong, while the pulse at the wrist is defective, act free] y and confidently. If, on the contrary, the ventricles are weak while the pulse is full, large, and rap ping, be cautious what you do, and if you draw blood at all, let it be by cupping the chest. Remember that the pathology of the disease is a temporary death of a portion of the lungs, and that the action of bloodletting is purely mechanical Position is of great importance. When the elasticity of the vessels is lowered by disease, the blood gravitates towards the lowest part of the viscus. Let the patient, therefore, lie on the side Opposite to that affected, that thus gravitation may tend to withdraw the congestion from the weaker point. If both lungs are affected posteriorly, the patient may lie on the face for a time. Never give purgatives in pneumonia. A patient with constipated bowels will do better than one with the Opposite state. (dr. T. K. Chambers, p.
