Book excerpt: The Human Papillomavirus (HPV) is an extremely prevalent and sexually transmitted infection that is a known cause of morbidities such as genital warts and cancers of the cervix, anus, and oropharynx. Non-cervical HPV-related cancers have been a developing problem in North America, increasing in incidence by up to 225% in some instances over a span of two decades. This study investigated levels of awareness and knowledge of HPV, Oropharyngeal Cancer (OPC), and the HPV vaccine using a self-administered web-based survey designed specifically for this research. University students (n=1,005) aged 18-30 completed a 42-item questionnaire that included demographic information, awareness questions, and a series of true/false/I don't know knowledge questions. Results revealed that participants had relatively high levels of awareness. However, many respondents had significant gaps in their knowledge of HPV, OPC, and the HPV vaccine. These data suggest that further efforts to educate young adults on these topics are warranted.

Book excerpt: Background: Human papillomavirus (HPV) is the most common sexually transmitted infection in the United States, with nearly 80% of the population infected at some point in their lifetime. HPV is the cause of approximately 70% of oropharyngeal cancers (OPC). Methods: Chapter 1 utilized data from the Health Information National Trends Survey 5, cycles 1-3; logistic regression analyses examined differences in HPV-related oral cancer knowledge between sexual minorities and their heterosexual counterparts. Chapter 2 utilized electronic health record data of patients without prior history of HPV vaccination. Logistic regression modeling, guided by the Andersen Behavioral Model, examined associations between predisposing, enabling, and need factors with HPV vaccine initiation. Chapter 3 utilized survey data from medical and dental providers that assessed attitudes regarding HPV-related OPC patient engagement. Guided by the Theory of Planned Behavior, HPV-related OPC responses were compared between medical and dental providers using chi-square tests and linear regression analyses. Results: In Chapter 1, HPV-related oral cancer knowledge overall was low and there were no significant differences in knowledge for sexual minority men or women compared to heterosexual counterparts. In Chapter 2, HPV vaccine initiation rates overall, were relatively low (20%). For males, minority sexual orientation (OR:1.75; 95% CI:1.20-2.55) and HIV+ status (OR:2.63; 95% CI:1.16-5.97) were associated with greater odds of HPV vaccine initiation; as age increased, odds of HPV vaccine initiation decreased (OR:0.74; 95% CI:0.66-0.84). For females, non-White race (OR:1.26; 95% CI:1.04-1.53), having a nurse practitioner/physician assistant as a provider (OR:1.33; 95% CI:1.08-1.65), and more frequent utilization (OR:1.54; 95% CI:1.31-1.80) were associated with greater odds of HPV vaccine initiation. Because men are behind in this vaccination campaign, this study compared knowledge and attitudes regarding HPV and the vaccine among college-aged men and women in order to improve vaccination strategies among this at-risk population. Methods: A convenience sample of 95 university students, 40 men and 55 women, ages 18 to 26 (mean age of 20.2) completed self-administered pencil-and-paper questionnaires with approximately 40 questions that assessed the desired variables. Knowledge was measured through multiple choice and true/false questions. Attitudes were measured by Likert scale responses following theoretical framework. Data were analyzed and summarized through descriptive statistics using SPSS software. Results: Awareness of the HPV vaccine's availability for men was significantly low for both genders, particularly for men. Health practitioners' and parents' approval both play a pivotal role in considering the vaccine. For men, peers' approval was also important in this decision-making process. Overall, females showed higher rates of awareness in HPV and the vaccine and indicated more positive social norms associated with obtaining the vaccine. Conclusions: According to the results of this study, the HPV vaccine is still prevalently associated with female health and therefore detracting from the male consumer. In order to increase uptake of the HPV vaccine in both genders, a gender neutral vaccine campaign that does not focus on one specific health issue is suggested.--P. vii.

Book excerpt: Human papilloma virus (HPV) is the most common sexually transmitted infection (STI) in the U.S. HPV is known to cause many types of cancers such as cervical, vulvar, vaginal, penile, anal, and oropharyngeal cancers. A preventative measure, the HPV vaccine, is recommended by the Centers for Disease Control for both boys and girls. Since past prevention efforts primarily focused on female vaccination it is uncertain if males are aware of HPV, its effects and preventative measures. The purpose of this study was to determine the knowledge level of HPV, perceived risk of HPV and intent to obtain HPV vaccination among male university students. Participants (n=361) completed a 31-item electronic survey administered by email. Responses to knowledge-based questions indicted relatively low levels of knowledge about HPV and HPV vaccinations. Fifty percent knew that HPV was the most common STI, over half (52%) knew of the HPV association with multiple cancers, only 39% could correctly identify the virus and two-thirds of participants were unable to identify the virus strains controlled by vaccination. A significant portion of participants either agreed (54%) or strongly agreed (19%) that they were at a high risk for HPV. Most participants either agreed (49%) or strongly agreed (39%) that unprotected sexual activity put them at risk but nearly half (46%) were unaware that HPV vaccinations were available for males. Despite risk acknowledgment and over 50% vaccine awareness, 86% were not vaccinated and 65% of these participants did not plan to become vaccinated. In addition, of the 86% who were not vaccinated, 45% did not perceive themselves at risk and 47% were not aware that a vaccination existed. Only 9% were fully vaccinated against HPV with 6% reporting they had received partial vaccination. Results indicate, though male students understand the scope of HPV infection and many perceive themselves to be in a high risk group, few of them are, or intend to be, vaccinated. This is a significant issue for public health promotion. Prevention efforts should focus on promoting HPV vaccinations and increasing awareness of the long term health effects associated with HPV to the male population and parents of underage males.

A safe and efficient vaccine has been approved for both boys and girls. The Center for Disease Control and Prevention announced that teens and young adult under age 26 are at a particular risk for HPV. It is essential to start the vaccination series between 9 and 17 years of age. Parental and caregivers acceptance of HPV vaccine for their children has been a problem. Despite, the HPV vaccine promotions and availability, the vaccination rates of HPV continue to low. The focus should be on parents and caregivers because parental attitudes, beliefs, knowledge, intention to vaccinate and behaviors appear to contribute to low parental acceptance of the vaccine (Gamble et al., 2010). Parental and caregiver education improved parental knowledge, intention to vaccinate and attitude towards HPV vaccination. The purpose of this project was to develop an evidence-based parental and caregiver educational intervention to improve knowledge, intention to vaccinate and attitude towards HPV vaccine, hoping to influence parents and caregivers of children ages 9 to 17 to consent to the HPV vaccine. Parental education as an intervention, focusing on HPV and HPV vaccine was found to have positive impact on parental knowledge, intention to vaccinate and parental attitude toward HPV vaccine. There was a 40%, 73%, and 33% improvement respectively on parental knowledge, attitude, and intention to vaccinate which was higher than the benchmark. Book excerpt: Human papillomavirus (HPV) infection results in serious health issues including cervical, anal, vulvar, penile and oropharyngeal cancers. There are three vaccines against HPV but vaccination rates in the United States remain low. One barrier to uptake is a concern that individuals who are vaccinated may increase their risky sexual behaviors or decrease their use of cervical cancer screenings, an adjustment in perceived level of risk for HPV that can be studied using risk compensation theory. Methods Three distinct studies examined risk compensation after HPV vaccination. A systematic review examined literature from January 1, 2008-June 30, 2015, using three databases. A qualitative study using semi-structured interviews of 22 healthcare providers that assessed their beliefs regarding sexual disinhibition and cervical cancer screening following vaccination. A cross-sectional survey that assessed cervical cancer screening practices, awareness and comfort with recommendations, and knowledge regarding the purpose of a Papanicolaou (Pap) test. Results Twenty articles were included in the systematic review. None of the studies of sexual behaviors and/or biological outcomes found evidence of riskier behaviors after HPV vaccination. Instead, the studies found vaccinated individuals were less likely to report risky sexual behaviors, sexually transmitted infections (STIs), and pregnancy. Qualitative interviews found no healthcare providers believed the HPV vaccine would result in increased risky sexual behavior or decreased cervical cancer screening, and these concerns would not influence their vaccination recommendations. The survey included 291 women 21-35 years old; 62% were non-Hispanic black, 84% had a Pap test in the last three years, and 33% had at least one HPV vaccine. Logistic regression showed that vaccinated women did not have greater odds of having a Pap test in the past three years (OR=1.32; 95% CI=0.66-2.65; p=0.427). However, this odds ratio was significant when controlling for age and race (AOR=3.06; 95% CI=1.37-6.83; p=0.006). Conclusion These studies found no evidence of increased risky sexual behaviors or decreased cervical cancer screening rates after HPV vaccination. Furthermore, vaccinated women showed less evidence of risk compensation. These results should alleviate concerns about administering the HPV vaccination among parents and providers.

Nelly Oudshoorn explains why it is that, although the technical feasibility of male contraceptives was demonstrated as early as the 1970s, there is, to date, no male pill. Ever since the idea of hormonal contraceptives for men was introduced, scientists, feminists, journalists, and pharmaceutical entrepreneurs have questioned whether men and women would accept a new male contraceptive if one were available. Providing a richly detailed examination of the cultural, scientific, and policy work around the male pill from the 1960s through the 1990s, Oudshoorn advances work at the intersection of gender studies and the sociology of technology. Oudshoorn emphasizes that the introduction of contraceptives for men depends to a great extent on changing ideas about reproductive responsibility. Initial interest in the male pill, she shows, came from outside the scientific community: from the governments of China and India, which were interested in population control, and from Western feminists, who wanted the responsibilities and health risks associated with contraception shared more equally between the sexes. She documents how in the 1970s, the World Health Organization took the lead in investigating male contraceptives by coordinating an unprecedented, worldwide research network. She chronicles how the search for a male pill required significant reorganization of drug-testing standards and protocols and of the family-planning infrastructure—including founding special clinics for men, creating separate spaces for men within existing clinics, enrolling new professionals, and defining new categories of patients.
