Book excerpt: Excerpt from X-Ray Observations for Foreign Bodies and Their Localisation This small contribtttion to War Radiology was written by inviiation in June, 1917. It ivas to have been a chapter on the localisation of foreign bodies in a ivork that was being compiled for the American Council of National Defence but owing to the death of the Editor the publication was abandoned. The matter remains unchanged with the exception of some stnall additions and appendices. The Hospital at Ris Orangis has, during 1917 and 1918 been largely used as a training centre, and it is in response to the requests of many visitors to the clinic that the book is now published. It formed the basis of the lectures given on the subject. These observations are based on the personal experience of the Author with the methods referred to in this book, constituting over 4 years application to the problems of war radiology (the first seven months were passed in an advanced ambidance within three miles of the line, the remainder has been spent at the base, apart from visits of detached duty), but for the majority of them, no originality is claimed; and it is impossible to give personal acknowledgment to all who have generously helped me, or to the originator and every method mentioned. I should like, however, to acknowledge the great kindness and courtesy shown to me, and the information and experience so freely placed at my disposal, by my French colleagues. It is largely owing to their keen appreciation of the questions involved, and their unwearied application to the solution of the problems presented, that much progress has been made. To Dr. Belot and Dr. Fraudet I am indebted for the privilege of inserting their method of localisation of foreign bodies in the eye, from their original work in the "Journal de Radiologic et d' Electrotherapies." No attempt has been made in these pages to cover the vast ground of X-ray work in general; the aim has been merely to bring together a number of practical points connected with the author's personal work. The system he advocates has stood the test of time, but it has also Shown itself adaptable to the more rapid procedure which is demanded, and the simplifications which have been introduced into the original method will be found fully described in the body of this book. The author would direct special attention to the final chapter, which deals with the precise localization Of foreign bodies in eyeball and orbit, for he trusts that this application of X rays may be extended. As every ophthalmic surgeon knows, foreign bodies in the eye ball and orbit are ever with us. At present, owing to hand-grenade warfare, such cases are occurring in very large numbers, but in civil life, especially in engineering workshops and various industrial concerns, these cases are constantly met with; and very often it happens that the eye can be saved if - but only if - the exact position of the foreign body is known, so that it may be extracted without difficulty. The x-rays are broken down into 3 categories.Category 1: Ingested or Inhaled The mouth innocuously enough accepts foreign bodies. Unfortunately, there are tight areas within the gastrointestinal tract where these items can get stuck. Furthermore, some objects (such as sharp items or certain types of batteries) can be dangerous or toxic if not retrieved. Category 2: Introduced Welcome to the gun and knife show. Whether inadvertently caused by the patient or inflicted by someone else intending harm to the patient, x-rays play an important role in characterizing and localizing the weapons used. In addition to their medical relevance, these x-rays often end up in forensics, delineating the type and trajectory of the weapon involved. Category 3: Inserted Last but not least...the XXX-rays. Technically, these foreign bodies can be classified as introduced. However, given the "unique" circumstances under which many of these cases present, they deserve a category of their own.
