Book excerpt: Strategic aeromedical evacuation (AE) of casualties from the theater of operations to the CONUS during wartime is a complex operation that involves the integration of medical personnel and policies with airlift concepts and capabilities. Military analysts within the Air Mobility Command Analysis Group (AMC/XPY) have traditionally used deterministic linear programming techniques to estimate the number of aircraft the United States Air Force (USAF) requires for given contingency scenarios. However, this group has yet to develop a stochastic approach to validate their resource recommendations, and more importantly, to study the interrelationships between key factors comprising strategic aeromedical evacuation. As the possibility for many smaller campaigns around the world increases, USAF medical planners require a flexible, analytical tool which captures the major elements of this important mission in order to quickly evaluate differing medical airlift plans and policies. This thesis develops, documents, and demonstrates the use of a computer simulation model for strategic AE operations that is modular in nature, completely data driven, and quickly adaptable to scenario changes, as a policy/planning aid for the AMC Surgeon and his staff. In addition, this thesis investigates the use of two statistical techniques, principal component analysis and factor analysis, for interpretation of the simulation output.... Air mobility is a network of systems that combines airlift, airdrop, aeromedical evacuation (AE), air refueling (AR), and air mobility support assets, processes, and procedures into an integrated whole to support the transport of personnel and materiel. The Secretary of Defense (SecDef) directs the assignment of air mobility forces to the Commander, United States Transportation Command (CDRUSTRANSCOM) and other combatant commanders (CCDRs). To deter threats against, or to assist in the defense or pursuit of United States (US) national interests, the Department of Defense maintains forces, organizations, and processes necessary to conduct air mobility operations. The National Air Mobility System (NAMS) is a broad and comprehensive system that provides the President, SecDef, and CCDRs with rapid global mobility. The mobility air forces (MAF) are those forces assigned to combatant commands that provide rapid global mobility and conduct air mobility operations. The MAF's four core functions are airlift, AR, air mobility support, and AE. NAMS consists of forces that perform intertheater, intratheater, and organic mobility operations. US Transportation Command (USTRANSCOM) and the geographic combatant commands (GCCs) possess air mobility assets that are capable of performing both intertheater and intratheater operations. A unique aspect of these operations is their reliance on Global Air Mobility Support System (GAMSS) and the worldwide command and control (C2) capabilities of the Air Mobility Command 618th Tanker Airlift Control Center (618th TACC). The GAMSS is comprised of a limited number of permanent en route support locations plus mobile forces that deploy under the global reach laydown strategy. This publication has been prepared under the direction of the Chairman of the Joint Chiefs of Staff. It sets forth joint doctrine to govern the activities and performance of the Armed Forces of the United States in joint operations and provides the doctrinal basis for interagency coordination and for US military involvement in multinational operations. It provides military guidance for the exercise of authority by combatant commanders and other joint force commanders (JFCs) and prescribes joint doctrine for operations, education, and training. It provides military guidance for use by the Armed Forces in preparing their appropriate plans. It is not the intent of this publication to restrict the authority of the JFC from organizing the force and executing the mission in a manner the JFC deems most appropriate to ensure unity of effort in the accomplishment of the overall objective. Joint doctrine established in this publication applies to the joint staff, commanders of combatant commands, subunified commands, joint task forces, subordinate components of these commands, and the Services. The guidance in this publication is authoritative; as such, this doctrine will be followed except when, in the judgment of the commander, exceptional circumstances dictate otherwise.

Book excerpt: "Hurricane Katrina and other recent natural disasters have highlighted the need for AE military assistance to state and national disaster response operations within CONUS. The purpose of this study is to evaluate the best method to employ an integrated AE patient movement and tracking system across all military, state and federal agencies for disaster response. The study answers this question: how can AE patient movement and tracking be improved between military and civil authorities in the event of a federal disaster? This study uses the evaluation method to consider what steps should be taken to adopt a uniform aeromedical evacuation (AE) patient movement and tracking system for CONUS disaster response. The research provides a brief history of military AE, a case study of AE in Operation Iraqi Freedom and a case study of AE response in Hurricane Katrina. The research then examines the current state of AE in federal disaster response and evaluates concepts and systems that could be adopted for improved AE integration across multiple levels of state and federal government. Inputs from subject matter experts in patient movement and tracking are used in this evaluation process. Finally, the research concludes by assessing the need for improved patient movement and tracking systems. The recommendations from this study include patient movement and patient tracking improvements. The primary recommendation from the study is to adopt JPATS as the universal patient tracking tool for all state and federal agencies in AE disaster response. The secondary recommendation is to adopt the Disaster Aeromedical Staging Facility (DASF) with Mobile Acute Care Strike Team (MAC-ST) concept as the AE staging standard for civil/military patient movement in federal disaster response operations."--Abstract.
