Book excerpt: During the 7th International Bile Acid meeting held in Cortina d'Ampezzo. , Italy, during March 1982, there were two important Workshops dealing with recent advances in two areas of bile acid metabolism - Serum Bile Acids i,n Health and Disease and The Pathophysiology of the Enterohepatic Circulation. These Workshops complemented the broad range of topics in bile acid metabolism which were presented formally as part of a Postgraduate Course. The proceedings of these formal presentations have already been pub lished by MTP Press under the title Bile Acids in Gastroenterology but in view of the exciting, important, new information which emerged during both Workshops, the organisers of the meeting felt that it would be valuable to record and edit the proceedings of the Work shops, which form the basis of this volume. The advent of new techniques for measurement of total and indi vidual bile acids in serum now means that accurate and reliable 7 PREFACE measurements of bile acids in peripheral circulation are available to all. This has resulted in a veritable explosion of information in the serum bile acid field and, therefore, the appearance of a critical evalu ation of the technical and basic scientific and clinical aspects of measuring serum bile acids is indeed timely.

Those colleagues from time to time at International Meetings which are often held in attractive parts of the world. The 7th International Symposium on bile acids 'Bile Acids in Gastroenterology' was no exception. It took place in Cortina d'Ampezzo in the heart of the Italian Dolomites, from 17th-20th March 1982. This meeting was organised by a Scientific Committee, with representatives from Italy, the United States and Great Britain, in collaboration with the Italian Society of Gastroenterology. The format of the meeting was somewhat different from that of previous years. In addition to the free communications (verbal and poster presentations) which characte rise many scientific meetings, there was also an Advanced Postgraduate Course on bile acids given by a distinguished international panel of experts. Their contributions form the basis for this timely volume which should be of interest both to basic scientists and to clinical investigators alike. The editors are indebted to Dr Gian Germano Giuliani, Gipharmex SpA, Milano, whose generous support made the meeting possible. They also thank Mr P. M. Therefore, a bidirectional interaction between the maternal and the fetal immune systems must exist, which could be disturbed by preexisting autoimmune liver diseases. Since nowadays autoimmune liver diseases are increasingly detected as early stage diseases due to modern diagnostic procedures, e.g. in women of childbearing age, and since the number of posttransplant pregnancies has become an issue due to improved survival, questions concerning regnancy, autoimmune liver disease and treatment options with bile acids and/or immunosuppressants are of an ever-increasing interest. This book, the proceedings of a Falk Workshop held in Freiburg, Germany, on June 2, 2002, is certainly unable to give any definite answers to any of the hundreds of still remaining questions in this fascinating field, but hopefully it will help to stimulate and initiate cooperation among immunologists, bile acid researchers, gynaecologists and internists.

Although originally mainly physiological data on bile acid synthesis and transport were emphasized, and later on also the therapeutic benefit of bile acids in gallstone disease and cholestasis was discovered, we have come now to the molecular biology and genetic era with major discoveries in transport defects and related diseases. This book is the proceedings of Falk Symposium No. 120, held in The Hague, The Netherlands, on October 12-13, 2000 - the 16th International Bile Acid Meeting. One of the main discoveries recently has been the identification of nuclear receptors for bile acids, which gives them a much broader perspective than previously anticipated. It even suggests that bile acids can regulate their biosynthesis and enterohepatic circulation transcriptionally. It will therefore not be surprising that this topic, together with the molecular regulation of cholesterol 7alpha-hydroxylase and cholesterol homeostasis, has a dominant place in these proceedings. Another important topic is the progress in our molecular understanding of hepatic (both at the basolateral and canalicular sites), cholangiocytic and intestinal bile acid transport processes. Further insights into genetic defects causing cholestasis or intestinal malabsorption in animal models and in human diseases are also discussed by a number of well-known authors. Finally the last section deals with new findings on the role of bile acid therapy in cholestatic syndromes or chemoprevention and with the potential benefit of bile acid inhibitors. All contributors provide an update on the most recent developments in their field.

Book excerpt: This book is the proceedings of the Falk Symposium No. 127 on `Autoimmune Diseases in Paediatric Gastroenterology' (IV International Falk Symposium on Paediatric Gastroenterology), held in Basel, Switzerland, on November 8-9, 2001. The symposium focused on the role of the immune system, both the acquired and the innate systems, in inflammatory bowel disease (IBD) in children and adolescents. The innate system has an important fundamental role in host defence by initiating immune responses against potentially deleterious matter. However, a mutation within the innate system may elicit an immune response against the host: hence, an autoimmune response. Chronic autoimmune hepatitis occurs predominantly in young people, and especially in women. Immunological changes are conspicuous. Tissue antibodies are found in a large number of patients. This is a disease of disordered immunoregulation marked by a deficit in suppressor T cells causing the production of autoantibodies against specific hepatocyte surface antigen. Liver membrane protein is found in the sera of patients with autoimmune chronic acute hepatitis and with primary biliary cirrhosis (PBC). The latter condition of progressive granulomatous destruction of intrahepatic bile ducts is, in many respects, analogous to the graft-versus-host syndrome where the immune system has become sensitized to foreign HLA-molecules. Primary sclerosing cholangitis (PSC) is another condition of unknown origin. All parts of the biliary tree can be involved in a chronic, fibrosing, inflammatory process that results in obliteration of the biliary tree and ultimately in biliary cirrhosis. About half of the patients also suffer from ulcerative colitis and rarely from Crohn's disease. Circulating antibodies to some antigens are found in obstructed portal tracts, as well as increased concentrations of biliary immune complexes in patients with PBC. In all three previous Falk symposia on paediatric gastroenterology, attention was focused on the role of the innate immune system in the aetiology of IBD. It has become increasingly clear in recent years that the innate system has a much more important and fundamental role in host defence. The decision to initiate an immune response is one of the major roles of the innate system. Mutations within this system could transform it into becoming constitutively active, resulting in an inflammatory reaction and thus eliciting an autoimmune response. Following an introduction to the basic phenomena of autoimmunity, the proceedings discuss clinical aspects of autoimmune diseases. In particular, current knowledge and the state of the art about the diagnosis and treatment of the autoimmune diseases of the gastrointestinal tract are described by world-renowned experts. The book also contains the short presentations on selected topics, as well as abstracts of the mini-posters read by title, which were included in the symposium.

Since a significant proportion of patients is steroid-dependent or refractory and because of the problematic side-effects of long-term systemic steroids, active immunosuppression has gained acceptance in the field. The classical immunosuppressants azathioprine and 6-mercaptopurine have long been evidence-based in IBD, but underused. Recently, methotrexate and cyclosporine have also been proven to be effective in certain situations. Newer drugs like tacrolimus, mycophenolate and others may be similarly useful but their potential is still unclear. Finally, the immune modifiers including IL-10 and TNF-antibodies have been successfully subjected to controlled trials. Other experimental drugs discussed in this book are on the horizon. This volume is the Proceedings of Falk Symposium 119 held in Freiburg-im-Breisgau, Germany, October 3-4, 2000, and covers systematically the field of immunosuppression in inflammatory bowel diseases. It is hoped that it will further the prudent use of these drugs in the proper clinical situations by increasing our understanding of both the mechanisms of action and the clinical benefit to the patient.

Book excerpt: This book is the proceedings of the Falk Symposium No. 121 on 'Steatohepatitis (NASH and ASH)', held in Den Haag, The Netherlands, on October 14-15, 2000. The histological features of what we now call non-alcoholic steatohepatitis were described as early as 1962 by the Honorary President of the Symposium, Professor Herbert Thaler, from Vienna. Others followed, and in 1980 Jürgen Ludwig, one of the speakers of this symposium, introduced the name `non-alcoholic steatohepatitis' or NASH. In a Consensus Symposium organized by the National Institute of Health (NIH) in Washington, USA, in December 1998, NASH was recognized as one of the most common liver diseases in Western countries when viral hepatitis and heavy alcohol consumption were excluded. ASH, or alcoholic steatohepatitis, is more common than NASH, since alcohol is omnipresent in Western as well as Eastern cultures. Histologically NASH and ASH are similar or even identical. Morphological findings range from fatty degeneration to inflammation and fibrosis, and may end up in liver cirrhosis. In spite of the well-defined morphological features, our knowledge of epidemiology, aetiology, and pathogenesis is full of gaps, especially for NASH. Therefore, it is the purpose of this book to show the state of the art, to discuss recent scientific data, and to suggest possible treatment strategies, hoping to stimulate clinicians as well as scientists.
