Book excerpt: "The Veterans Health Administration (VA) is the United States' largest unified healthcare provider, but little is known about the experiences of the professionals who work there. In all healthcare settings, and in the helping professions in particular, women are at an increased risk of victimization. Therefore, the current study sought to better understand the workplace climate of VA settings for women therapists, including experiences of being victimized by their clients. Specifically, using grounded theory, this study examined three questions: 1) In what ways does military culture make its ways into VA settings? 2) what is unique about VA culture that presents women therapists with challenges when working with clients? 3) what are the experiences of being sexually harassed, assaulted, or stalked (by clients or in general?) in this sample of VA women therapists? The current study found that all participants experienced the VA culture as impacted by military culture, both in structure of administration and in interpersonal client interactions. Additionally, participants faced challenges related to the integration of military culture, particularly rigid hierarchy. Participants also experienced challenges related to working in male-dominated settings, another factor related to military culture. Further, all participants experienced at least one instance of sexual harassment, though not all participants identified these experiences as sexual harassment. The emerging theory suggests that women's experiences of sexual harassment influence their labeling of an event as sexual harassment, their own responses to the harassing experience, and the institution's response to the harassing incident. The latter three factors mutually influence one another and may impact continued experiences of harassment in the future. Limits and implications are discussed. Book excerpt: Despite the implementation of the universal screener by the Department of Veteran Affairs (VA) to identify military sexual trauma (MST) many female veterans continue to experience sexual assaults. To better understand where the disparity exists, a literature review was conducted to review studies that discuss and provide information on the mental and physical health risks and outcomes of MST among female veterans. Four electronic databases were searched using the key words: military, veteran, MST, cognitive processing therapy studies, females, military sexual trauma, sexual assault, sexual abuse, sexual violence, military culture, treatment, Cognitive Processing Therapy (CPT), Cultural Spillover Theory, Posttraumatic Stress Disorder (PTSD), Depressive Disorder (DD) and Substance Abuse Disorder (SUD). Of the 1,079 documents found, 16 studies met inclusion criteria. The majority examined CPT as a successful method of intervention and treatment for veterans who had experienced PTSD, depression, and SUD linked to MST. Limitations were present as a few studies offered a larger sample size contributing to generalization of the study. Findings have implications for social workers who may provide counseling for veterans as they may lack specific military education and the knowledge on how to address veteran related issues.

Book excerpt: Military sexual trauma (MST) occurs at devastating rates to service members, by service members, and is widely misunderstood, qualitatively understudied, and reporting may be procedurally biased. The purpose of this study was to phenomenologically explore the lived experiences of female veteran MST survivors with their leadership (chain of command/supervisors) and understand how military culture effects these individuals. A feminist-theory conceptual framework was applied to contextualize hegemonic hypermasculine military culture and the divide and damage it may cause to female service members before MST, when surviving an MST event, and when surviving post-MST fallout. This study included 10 participants who were female, experienced MST, and served in the military for at least 90 days. Through qualitative inquiry, participants were interviewed by telephone between July 2019 and September 2019. Each interview was recorded, transcribed, and analyzed using interpretive phenomenological analysis. Data showed participants who did not report their MST to their leadership failed to do so due to fear of reprisal in situations ranging from being threatened with maltreatment to retaliation/reprisal, the supervisor being the perpetrator, and ostracism. Those who did report their MST had negative experiences, including victim blaming by their leadership, shaming, taking no action, betrayal, retaliation, and ostracism. Participants tolerated sexual harassment as the norm or, as one participant noted, "My other duty station, it was great. There was sexual harassment, but again, it's not as bad as the other thing." Military culture post-MST included a lack of support/mental health counseling, having to pretend it did not happen and continue to work (and live) with the perpetrator, often as their supervisor, and perpetrators with no consequences. Participants reported effects of MST including suicide attempts, dissociation, depression, anxiety, trust and relational issues, emotion and mood dysregulation, substance abuse, low self-worth, social isolation, intense fear, health problems, loss of career and employment potential, avoidance, and intense shame. Female service members face extreme difficulty with reporting MST, that may be exacerbated by policy and military culture, forming a procedurally flawed and damaging system. Several recommendations are presented for future practice and research.

Book excerpt: The primary objective of this pilot study was to understand the military experiences of OEF/OIF women veterans. Seven women veterans described accounts of sexual harassment and sexual assault, also known in the Veteran Health Administration (VHA) context as Military Sexual Trauma (MST). The prevalence and dialogue of MST both explicitly and implicitly throughout all the interviews justified examining MST on its own. As an alternative to tracking new cases of MST, this thesis provides an examination of the rhetoric of betrayal and suggests that objective knowledge of MST does not exist apart from such social conditions and one's interpretations of them. Betrayal emerged as the way in which women veterans understood and made meaning of their MST experiences during the claims-making process. Women veterans incorporated strategies to manage the sexual harassment and sexual assault they experienced while in the military environment, since reporting MST was actively discouraged. Findings from this study suggest that the way we approach and understand MST as a social problem needs to be reconsidered and further examined.

Veteran Administration (VA) healthcare services may not be suited to their needs as women, and civilian services are not well suited to manage their needs as Veterans. Therefore, this dissertation study used constructivist Grounded Theory methods to explore the experiences of women Veterans when accessing mental health outpatient services, their decision-making process when make the choice to enter mental health service, and aspects of the experience that are important or meaningful to them. Twelve women Veterans revealed meaningful, personal stories on their experiences of trauma and their use of mental health outpatient services. While addressing each of these factors, what emerged from the data was a broader Grounded Theory Process model of how women Veterans process trauma, and the categories of Trauma, Transitions, Identity and Structure. Women Veterans who participated in the study used mental health outpatient services to assist in reestablishing identity after trauma and to propel forward in their lives. This research provides key insight into how women Veterans make healthcare related choices and process traumatic events, like military sexual trauma (MST). This has implications for research, practice, and policy to improve the provision of care for women Veterans.
