Further, because many pregnant women are not tested for HIV and therefore do not receive treatment, the number of children born with HIV is still unacceptably high. What can we do to eliminate this tragic and costly inheritance? In response to a congressional request, this book evaluates the extent to which state efforts have been effective in reducing the perinatal transmission of HIV. The committee recommends that testing HIV be a routine part of prenatal care, and that health care providers notify women that HIV testing is part of the usual array of prenatal tests and that they have an opportunity to refuse the HIV test. This approach could help both reduce the number of pediatric AIDS cases and improve treatment for mothers with AIDS. Reducing the Odds will be of special interest to federal, state, and local health policymakers, prenatal care providers, maternal and child health specialists, public health practitioners, and advocates for HIV/AIDS patients. There has been a dramatic global increase in the rates of women living with HIV/AIDS. Among young women, especially in developing countries, infection rates are rapidly increasing. Many of these women are also mothers with young infants. When a woman is labeled as having HIV, she is treated with suspicion and her morality is being questioned. Previous research has suggested that women living with HIV/AIDS can be affected by delay in diagnosis, inferior access to health care services, internalized stigma and a poor utilization of health services. This makes it extremely difficult for women to take care of their own health needs. Women are also reluctant to disclose their HIV-positive status as they fear this may result in physical feelings of shame, social ostracism, violence, or expulsion from home. Women living with HIV/AIDS who are also mothers carry a particularly heavy burden of being HIV-infected. This unique book attempts to put together results from empirical research and focuses on issues relevant to women, motherhood and living with HIV/AIDS which have occurred to individual women in different parts of the globe. Achieving and maintaining elimination requires strong political and public health commitment. Strengthened, resilient health systems improve a broad range of services and outcomes while similarities in prevention interventions add to the benefit of an integrated approach. Validation is an attestation that a country has successfully met standard criteria for elimination, or for being at one of the 3 levels of achievement on the ‘Path to Elimination’ while delivering quality services for women, girls and their children, through the life-course, respecting human rights and ensuring gender equality and community engagement. It requires systems that comprehensively identify and monitor new infections and infant outcomes. Establishment of criteria for validation began in 2007 with global consultations while lessons learnt advised publication of 2 editions of global guidance on criteria and processes for validation: elimination of mother-to-child transmission of HIV and syphilis (the ‘Orange Book’). This document, the third version, adds on EMTCT of HBV, bringing together a package of interventions and metrics to support integrated management and monitoring of vertical transmission across a wide range of epidemiological and programmatic contexts.

Whereas significant progress has been made in reduction of mother to child transmission of HIV in developed countries, the situation remains desperate in developing countries. Progress has been hampered by shortage of staff, facilities, limited access to voluntary counselling and testing and lack of support for women by their partners and communities. The challenge is to increase voluntary counselling and testing uptake during antenatal care. Onandjokwe district in Northern Namibia is currently introducing the Prevention of Mother to Child Transmission Program (PMTCT). It has been found the previous PMTCT programs have failed because they adopted a top down approach where there was no community consultation. This study was conducted to explore the community perceptions, knowledge and attitudes regarding mother to child transmission of HIV through focus group discussions and in-depth interviews of key community members.

As a mother, she was deeply moved by the fate of many HIV-infected women and more than that, the short life of many children to whom HIV was transmitted by their mothers. All her efforts led her to this book, which presents the situation of HIV transmission from mothers to children in Vietnam, including the effect of health care system, culture with traditional norms and values, and personal difficulties of HIV-infected women. Medical intervention alone is insufficient to assist women in the comprehensive way accessing PMTCT services, given their complex needs in a society in which HIV is highly stigmatized and considered a "social evil".
