We reviewed the existing data regarding the epidemiology and management of TB in pregnant and postpartum women. We also compared the performance of an interferon-gamma release assay (IGRA) with the tuberculin skin test (TST) in the diagnosis of latent TB infection (LTBI) in pregnant and postpartum women in India. MANUSCRIPT I - Tuberculosis in pregnant and postpartum women: Epidemiology, Management and Research Gaps. Background. Tuberculosis is most common during a woman's reproductive years and is a major cause of maternal-child mortality. National guidelines for screening and management vary widely, however, due to insufficient data. Methodology. We conducted a review of the literature including online databases and clinical guidelines. Results. We review the global burden of TB in women of childbearing age, the effect of the pregnancy and the postpartum period on TB, how to screen and diagnose pregnant women for active and latent TB, the management of active and latent TB in pregnancy and postpartum, including the safety of TB medication, and infant outcomes. We also include data on HIV/TB co-infection and multi-drug resistant TB (MDR-TB). Discussion. The burden of TB is high in women of childbearing age, but data on the management of TB in pregnant and postpartum women are lacking as this vulnerable population is often excluded from clinical trials. In order to decrease maternal mortality from TB, more focused attention and research needs to be paid to TB in women. MANUSCRIPT II - Screening for latent TB infection in pregnant and postpartum women: A comparison of an interferon-gamma release assay (IGRA) with tuberculin skin testing (TST) in Pune, India. Background. Immune changes of pregnancy can allow latent TB infection (LTBI) to reactivate to active TB disease. The widely used TST lacks sufficient sensitivity and specificity for LTBI. The newer IGRA may be better for LTBI screening, but its performance in pregnant/postpartum women and in TB-endemic countries is not well established. Methods. We conducted a cross-sectional study of HIV-negative women seeking perinatal care at a government hospital in Pune, India. Sociodemographic, medical and TB symptom data were collected. Women without symptoms of active TB received TST and IGRA testing. Results. Of the 154 women enrolled from the antenatal clinic (ANC), there was 74% agreement (kappa=0.4) between the TST and IGRA compared to 69% agreement (kappa= 0.26) among 148 women from the delivery ward. Of the 140 (92%) with TST readings from the ANC, 18% (95% CI 12-25%) were TST positive versus 11% (95% CI 12-25%) of 142 (96%) women with TST readings from the delivery ward. In both the ANC and delivery ward, 32% (ANC: 95% CI 26-42%, Delivery: 95% CI 25-41%) of enrolled women were IGRA positive. Thirty (22%) women from the ANC had discordant results (25 TST negative/IGRA positive, 5 TST positive/IGRA negative) versus 35 (24%) women from the delivery cohort (33 TST negative/IGRA positive, 2 TST positive/IGRA negative). Discussion. Agreement between the TST and IGRA was fair in the ANC but poor in the delivery group. Stage of pregnancy appeared to affect the reliability of TST performance but not of IGRA performance, suggesting that the IGRA is a better screening tool for LTBI in pregnant and postpartum women from TB-endemic countries.

Of the 9 million annual TB cases, about 1 million (11%) occur in children (under 15 years of age). Of these childhood cases, 75% occur annually in 22 high-burden countries that together account for 80% of the world's estimated incident cases. In countries worldwide, the reported percentage of all TB cases occurring in children varies from 3% to more than 25%. The Stop TB Strategy, which builds on the DOTS strategy developed by the World Health Organization (WHO) and the International Union Against TB and Lung Disease, has a critical role in reducing the worldwide burden of disease and thus in protecting children from infection and disease. The management of children with TB should be in line with the Stop TB Strategy, taking into consideration the particular epidemiology and clinical presentation of TB in children. These consensus guidelines were produced to help the National Tuberculosis Programmes on the management of tuberculosis in children.

Module 2: screening – systematic screening for tuberculosis disease is an updated and consolidated summary of WHO recommendations on systematic screening for tuberculosis (TB) disease, containing 17 recommendations for populations in which TB screening should be conducted and tools to be used for TB screening. TB screening is strongly recommendations for household and close contacts of individuals with TB, people living with HIV, miners exposed to silica dust, and prisoners. In addition, screening is conditionally recommended for people with risk factors for TB attending health care, and for communities with risk factors for TB and limited access to care (e.g. homeless, urban poor, refugees, migrants). General population screening is recommended in high-burden settings (0.5% prevalence or higher). Symptoms, chest radiography (CXR), and molecular WHO-recommended rapid diagnostic tests for TB are recommended as screening tools for all adults eligible for screening. Computer-aided detection programmes are recommended as alternatives to human interpretation of CXR in settings where trained personnel are scarce. For people living with HIV, C-reactive protein is also a good screening tool. This guideline document is accompanied by an operational handbook, the WHO operational handbook on tuberculosis. Module 2: screening – systematic screening for tuberculosis disease, that presents principles of screening, steps in planning and implementing a screening programme, and algorithm options for screening different populations.
