This results in several, simultaneous epidemics worldwide with high numbers of cases and deaths. With the increase in global transport and communications, as well as urbanization and overcrowded conditions, epidemics due to the new influenza virus are likely to be established quickly around the world. Governments and their partners need to develop strategies and programmes to prepare for a pandemic. The objective of pandemic planning is to allow countries to be prepared to recognize and manage an influenza pandemic. Planning may help to reduce transmission of the pandemic strain virus, to decrease cases, hospitalizations and deaths, to maintain essential services and to reduce the economic and social impact of a pandemic.

Book excerpt: The WHO global influenza preparedness plan has been prepared to assist WHO Member States and those responsible for public health, medical and emergency preparedness to respond to threats and occurrences of pandemic influenza. It updates and significantly revises the Influenza pandemic plan. The role of WHO and guidelines for national and regional planning published by WHO in 1999. This new plan addresses the possibility of a prolonged existence of an influenza virus of pandemic potential, such as the H5N1 influenza virus subtype in poultry flocks in Asia, which persisted from 2003 onwards. It also makes provision for the possibility of simultaneous occurrence of events with pandemic potential with different threat levels in different countries, as was the case in 2004 with poultry outbreaks of H7N3 in Canada and H5N1in Asia. The phases of increasing public health risk associated with the emergence of a new influenza virus subtype that may pose a pandemic threat have been redefined, actions for national authorities are recommended, and measures to be taken by WHO during each phase outlined. This should result in greater predictability of the measures to be taken by the various partners involved, including WHO, during the different phases of the pandemic, and should improve international coordination and transparency in recommended national measures. Guidance is also provided to national authorities for developing their own pandemic plans in line with these phases. This document should be used in conjunction with the WHO checklist for influenza pandemic preparedness planning published by the World Health Organization in 2005. Contents - Executive summary 1. Book excerpt: The PIP Framework is a broad-based partnership to improve global pandemic influenza preparedness and response. The Framework, which grew out of the re-emergence of A(H5N1) influenza in 2004, was adopted by WHO’s 194 Member States at the World Health Assembly on 24 May 2011. Section 6.14.3 of the Framework establishes an annual Partnership Contribution (“PC”) to be paid by influenza vaccine, diagnostic and pharmaceutical manufacturers using the WHO Global Influenza Surveillance and Response System (GISRS). Resources contributed are to be used to strengthen pandemic influenza preparedness and response. WHO has published two prior high-level implementation plans to outline the use of the PC preparedness funds. In order to continue the strategic use of the PC funds, this document will build upon the gains made in the implementation of the previous two plans, take into account lessons from COVID-19 and take into consideration the evolving global landscape for pandemic and epidemic preparedness. The document will enable stakeholders to see at a high level, the approach for use of PC funds and priority areas for capacity-building for the period 2024-2030. The document will describe the current context, layout a results hierarchy and define the project management processes that are key for successful implementation.

The workshop summary, The Threat of Pandemic Influenza: Are We Ready? addresses these urgent concerns. The report describes what steps the United States and other countries have taken thus far to prepare for the next outbreak of "killer flu." It also looks at gaps in readiness, including hospitals' inability to absorb a surge of patients and many nations' incapacity to monitor and detect flu outbreaks. The report points to the need for international agreements to share flu vaccine and antiviral stockpiles to ensure that the 88 percent of nations that cannot manufacture or stockpile these products have access to them. It chronicles the toll of the H5N1 strain of avian flu currently circulating among poultry in many parts of Asia, which now accounts for the culling of millions of birds and the death of at least 50 persons. And it compares the costs of preparations with the costs of illness and death that could arise during an outbreak.

It brings together Member States, industry, other stakeholders and WHO to implement a global approach to pandemic influenza preparedness and response. The Framework includes a benefit-sharing mechanism called the Partnership Contribution (PC). The PC is collected as an annual cash contribution from influenza vaccine, diagnostic, and pharmaceutical manufacturers that use the WHO Global Influenza Surveillance and Response System (GISRS). Funds are allocated for: (a) pandemic preparedness capacity building; (b) response activities during the time of an influenza pandemic; and (c) PIP Secretariat for the management and implementation of the Framework. This report presents overall success metrics and infographics to illustrate progress in PIP Framework implementation. A progress report is published four times a biennium, and covers technical and financial implementation for the PIP PC High-Level Implementation Plan II (HLIP II), as well as the PIP Secretariat. Milestones are reported every six months and indicators are reported yearly. All data are presented cumulatively from the beginning of each biennium, in this case, 1 January 2020.
