Book excerpt: Despite progresses made over the last several decades, the prevalence of child malnutrition re-mains alarmingly high. About 149 million children under the age of five years old were stunted (too short for their age) in 2018, of which 55% and 39% lived in Asia and Africa, respectively. Malnourished children, especially stunted ones, may never achieve their full cognitive and non-cognitive potential with implications for their educational and labor market performance among other things. Malnutrition results from several interlinked factors operating at child, parental, household, and landscape level such as inadequate maternal nutrition before and during pregnancy and at the time of lactation, suboptimal breastfeeding practices, lack of nutritious complementary foods, and unhealthy living environments. This study analyzes the correlates of child undernutrition in rural Africa South of the Sahara (SSA) – a region with the least progress in tackling undernutrition.

Although progress continues to be made in reducing child stunting, over 22 percent of children under five years of age are still affected. Other forms of malnutrition are also growing: adult obesity continues to increase in countries irrespective of their income levels, and many countries are coping with multiple forms of malnutrition at the same time – overweight and obesity, as well as anaemia in women, and child stunting and wasting.

Book excerpt: Updates for many countries have made it possible to estimate hunger in the world with greater accuracy this year. In particular, newly accessible data enabled the revision of the entire series of undernourishment estimates for China back to 2000, resulting in a substantial downward shift of the series of the number of undernourished in the world. Nevertheless, the revision confirms the trend reported in past editions: the number of people affected by hunger globally has been slowly on the rise since 2014. The report also shows that the burden of malnutrition in all its forms continues to be a challenge. There has been some progress for child stunting, low birthweight and exclusive breastfeeding, but at a pace that is still too slow. Childhood overweight is not improving and adult obesity is on the rise in all regions. The report complements the usual assessment of food security and nutrition with projections of what the world may look like in 2030, if trends of the last decade continue. Projections show that the world is not on track to achieve Zero Hunger by 2030 and, despite some progress, most indicators are also not on track to meet global nutrition targets. The food security and nutritional status of the most vulnerable population groups is likely to deteriorate further due to the health and socio economic impacts of the COVID-19 pandemic. The report puts a spotlight on diet quality as a critical link between food security and nutrition. Meeting SDG 2 targets will only be possible if people have enough food to eat and if what they are eating is nutritious and affordable. The report also introduces new analysis of the cost and affordability of healthy diets around the world, by region and in different development contexts. It presents valuations of the health and climate-change costs associated with current food consumption patterns, as well as the potential cost savings if food consumption patterns were to shift towards healthy diets that include sustainability considerations. The report then concludes with a discussion of the policies and strategies to transform food systems to ensure affordable healthy diets, as part of the required efforts to end both hunger and all forms of malnutrition.

The 2015 Global Hunger Index (GHI) shows that the level of hunger in developing countries as a group has fallen by 27 percent. Yet the state of hunger in the world remains serious. This marks the tenth year that IFPRI has assessed global hunger using this multidimensional measure. This report’s GHI scores are based on a new, improved formula that replaces the child underweight indicator of previous years with child stunting and child wasting. This change reflects the latest thinking on the most suitable indicators for child undernutrition, one of three dimensions of hunger reflected in the GHI formula. Across regions and countries, GHI scores vary considerably. Regionally, the highest GHI scores, and therefore the highest hunger levels, are still found in Africa south of the Sahara and South Asia. Despite achieving the largest absolute improvements since 2000, these two regions still suffer from serious levels of hunger. Levels of hunger are alarming or serious in 52 countries. Most of the eight countries with alarming GHI scores are in Africa south of the Sahara. While no countries are classified in the extremely alarming category this year, this high level of hunger could still exist. South Asia saw the steepest absolute decline in GHI scores since 1990. Progress in addressing child underweight was the main factor behind the improved GHI score for the region since 1990.

Because of variations in survey tools attempts to compare the results of one survey with those of another have been fraught with diffi culties. Solving survey comparability problems is crucial if we are to establish accurate trends over time within a country and compare data between countries. For this reason the WHO/UNICEF Joint Monitoring Programme for Water Supply and Sanitation developed a set of harmonized survey questions that resolves the comparability problems of the past. Including the harmonized questions in national surveys and censuses will help countries gain more systematic information on the drinking water and sanitation needs of thei population. With popular use these harmonized questions will make data across international and national survey programmes more comparable which in turn will result in more accurate country regional and global estimates of unmet drinking water and sanitation needs. The core harmonized questions presented in this guide have been adopted by the Demographic and Health Surveys the Multiple Indicator Cluster Survey and the World Health Survey. The purpose of this guide is to encourage even more widespread use of these harmonized questions because accurate information is a valuable tool that can be used to assess progress towards international and national goals and targets. It also allows decision-makers and stakeholders to make evidence-based choices and direct efforts to where they are needed as well as promotes increased investment in the sector.

However, evaluation of the effect of polygyny on child nutrition has been hindered by the complexity of the relationship. This paper investigates the effect of polygyny on anthropometric outcomes while recognizing that unobservable household characteristics may simultaneously influence both the decision to form a polygynous union and the ability of the household to adequately nourish children. Polygyny can affect children’s nutrition through increased family size, early marriage, and the level of household investment in child health. In this paper, we apply an instrumental variable approach based on the occurrence of same sex siblings in a woman’s first two births to generate exogenous variation in polygyny. Using data from the 2008 and 2013 Nigeria Demographic and Health Surveys, we find a detrimental effect of polygyny on child undernutrition, with a greater effect in poorer households and those resident in more urban locations.
