A marked improvement in child malnutrition and infant mortality has also been experienced recently. But in spite of input and outcome improvements, policy makers are facing challenges, such as: a) reducing the health status gap between the poor and that of the non-poor; b) increasing resources for the provision of health care to the poor; and, c) increasing the efficiency in the use of these resources. The report reviews the health sector financing and delivery systems, the progress made thus far in health outcomes, and health reforms, both in financing of services and the provision of health. It further recommends best practices for strengthening management, planning, and information systems within the Ministry of Health. Conclusions and recommendations emerge from this report, conveying that much can be gained by sustaining and deepening those reforms directed at health improvements for the poor.

This report is meant to help the Government continue to develop its agenda to improve health care for the poor. Its focus is on poverty, and particularly on ways to improve the Ministry of Health's primary health subprogram, rather than on sector-wide reforms, to increase efficiency. The report emphasizes that there is much to be gained by sustaining and deepening the reforms directed toward improving health care for the poor while using the shift in focus and responding to the key challenges. Book excerpt: Peru is an upper middle-income country that has experienced fast economic growth (average of 6.9 percent per year from 2004 to 2013, according World Developing Indicators, WDI) combined with a reduction in poverty and inequality over the past decade. Economic growth was led by exports and domestic demand, generating an increase in private investment, attracting foreign capital, and strengthening public finances. The population living in poverty and extreme poverty fell from 58.7 percent and 16.4 percent in 2004 to 25.8 percent and 6 percent in 2012, respectively (INEI 2014a). Inequality has also decreased, with the Gini index declining from 0.503 in 2004 to 0.48.1 in 2010 (WDI).

The breadth and depth of data must be relevant and applicable to decision making at all levels of the health system, both to respond to national policy needs as well as comparing Peru in a regional and global context. This report assesses the status and availability of Peru's routinely reported health sector data and statistics as well as the underlying data infrastructure. The report provides a set of recommendations to improve the health information system and to strengthen data coverage and comparability as Peru's health care system develops.

Book excerpt: Over the last two decades, the Peruvian government has made great efforts to improve access to health care by significantly augmenting the coverage of the non-contributory public health care system Seguro Integral de Salud (SIS). This expansion has a positive impact on welfare and public health indicators, as it limits the risk of catastrophic health-related costs for previously uninsured individuals and allows for the appropriate treatment of illnesses. However, it also entails some unintended consequences for informality, tax revenues, and GDP, since a few formal agents are paying for a service that the majority of (informal) agents receive for free. In this paper, we use a general equilibrium model calibrated for Peru to simulate the expansion of SIS to quantify the unintended effects. We find that overall welfare increases, but informality rises by 2.7 percent, while tax revenues and output decrease by roughly 0.1 percent. Given the extent of the expansion in eligibility, the economic relevance of these results seems negligible. However, this occurs because the expansion of coverage was mostly funded by reducing the spending per-insured person. In fact, we find larger costs if public spending is increased to improve the quality of service given universal coverage.

Book excerpt: Large amounts of development assistance in the form of infrastructure and social programs are annually made available to communities across Peru due the presence of extractive industry companies. These investments however do not always achieve the social development impact anticipated. As one company pointed out: our company has invested millions of dollars in social programs in our neighboring communities, however the indicators for child malnutrition and maternal health have seen no noticeable improvement . Communities base their support or rejection for extractive industry operations by weighing up the benefits against the risks; where the risk-benefit balance sheet does not look positive, conflict is likely. Companies, and the governments who benefit from their tax revenues, therefore work hard to reduce risks and increase benefits for communities: but this report confirms an overlooked aspect of the development outcomes: men are capturing more of the benefits and these are not necessarily reaching the wider family; whereas it is the women and children who experience more of the risks that arise from the presence of extractive industry projects. The impact on women could be significantly improved by taking simple steps with little additional effort or cost disaggregating data by sex, strengthening consultations with women, investigating issues of risk to women, improving women s access to benefits. Additionally this report challenges the development industry (those non-governmental organizations, community-based organizations, universities, research centers, think tanks, funders, foundations and training organizations for whom the development of theories and practices to alleviate poverty is a primary objective.) to work more closely with local government and companies, sharing its knowledge on thematic issues such as gender (and other areas such as Rights-Based Approaches or Community-Driven Development) and on programs (such as maternal health, bi-lingual education, or productive chain development) to achieve what the NGOs, civil society and local government have within their mandates to deliver, and what the EI sector seeks.
