At the request of Congress, the Food and Drug Administration (FDA) commissioned a study to examine the current practice of mammography and breast cancer detection, with a focus on the FDA's oversight via the Mammography Quality Standards Act (MQSA), to identify areas in need of improvement. Enacted in 1993, MQSA provides a general framework for ensuring national quality standards in facilities performing screening mammography, requires that each mammography facility be accredited and certified, and mandates that facilities will undergo annual inspections. This book recommends strategies for achieving continued progress in assuring mammography quality, including changes to MQSA regulation, as well as approaches that do not fall within the purview of MQSA. Specifically, this book provides recommendations aimed at improving mammography interpretation; revising MQSA regulations, inspections, and enforcement; ensuring an adequate workforce for breast cancer screening and diagnosis; and improving breast imaging quality beyond mammography.

Book excerpt: Breast cancer screening programs offer the greatest promise for early detection but rates of participation in free screening programs have been disappointing for challenged populations. The purpose of the current study is to elaborate the beliefs and culturally embedded meanings that a population of low income, uninsured African American women hold toward breast cancer and breast cancer screening. During Year 01, the intermediary systematically attempted to contact 685 potentially eligible women from a - population data base from the Breast and Cervical Health Program in Washington state. Of these, 318 women had non-functioning phone contact numbers. Another 175 women were successfully contacted by the study intermediary, 91 of whom were screened out as ineligible and 26 of whom refused participation. A total of 64 eligible women agreed to be contacted by a study interviewer, 9 of whom declined or were determined to be ineligible by the interviewer. Thirty-one (31) women were enrolled and completed the case intensive interview and an additional 24 eligible women agreed to be contacted by a study team interviewer and are pending enrollment. Quality monitoring has been instituted for data generation and transcription. Initial inductive coding has yielded findings not previously documented in the research literature.

Book excerpt: The IOM's National Cancer Policy Board estimated in 2003 that even modest efforts to implement known tactics for cancer prevention and early detection could result in up to a 29 percent drop in cancer deaths in about 20 years. The IOM's National Cancer Policy Forum, which succeeded the Board after it was disbanded in 2005, continued the Board's work to outline ways to increase screening in the U.S. On February 25 and 26, 2008, the Forum convened a workshop to discuss screening for colorectal cancer. Colorectal cancer screening remains low, despite strong evidence that screening prevents deaths. With the aim to make recommended colorectal cancer screening more widespread, the workshop discussed steps to be taken at the clinic, community, and health system levels. Workshop speakers, representing a broad spectrum of leaders in the field, identified major barriers to increased screening and described strategies to overcome these obstacles. This workshop summary highlights the information presented, as well as the subsequent discussion about actions needed to increase colorectal screening and, ultimately, to prevent more colorectal cancer deaths.
