La conception de l'intervention indirecte évolue et se construit aujourd'hui autour du modèle du partenariat, qui se veut valorisante pour les parents. Cette vision du soin reste toutefois minoritaire, et il apparaît par ailleurs que les parents n'ont pas toujours conscience du rôle qu'ils peuvent jouer dans la prise en soins de leur enfant. Nous avons donc cherché, par notre étude, à savoir quelles sont les attentes et les représentations des parents concernant leur rôle dans le suivi orthophonique de leur enfant, et comment il peut être valorisant. Pour cela, nous avons mené des entretiens semi-dirigés auprès de parents d'enfants en soins orthophoniques pour troubles du langage oral, à deux temps des prises en soins. Les données recueillies ont permis de mettre en évidence que les parents s'attribuent un rôle primordial dans le développement langagier de leur enfant, mais qu'être « partenaire de soins » d'un professionnel de santé ne fait pas encore partie de leur champ des possibles. Ils identifient l'intervention orthophonique comme valorisante à plusieurs égards : appropriation de connaissances, régression des troubles de l'enfant, soutien du suivi.

L'orthophoniste est par conséquent directement en relation avec les parents pour cette prise en charge dans laquelle ils ont un rôle primordial. Au sein de cette relation soignant-soigné, les deux acteurs en présence ont des conceptions du trouble et une culture différentes. Néanmoins, le soignant qu'est l'orthophoniste a un projet de soins pour le soigné, projet que les parents acceptent plus ou moins facilement. Une évolution de leur point de vue est donc souvent nécessaire pour le bon déroulement de l'accompagnement parental. Pour étudier cette évolution sous un angle sociologique, nous avons fait enregistrer six familles pendant leurs deux premiers entretiens. Ainsi, nous avons pu observer que selon les attentes professionnelles la plupart des parents évoluent vers un modèle de participation mutuelle pour une meilleure collaboration dans le soin : cette évolution est due à la confrontation des pratiques de soins des parents aux attentes professionnelles. L'interaction entre ces deux cultures paraît jouer également un rôle dans l'évolution des parents.

Afin de limiter les impacts négatifs de cette problématique sur les enfants et leur famille, nous proposons d'intervenir de façon préventive. Nous avons mis en place une rencontre de prévention avec les parents en recherche d'orthophoniste afin de les conseiller et de les informer sur le langage écrit. Nous leur présentons également des aménagements et activités pour faciliter l'acquisition de la lecture. A plus long terme, nous visons une amélioration du niveau de lecture et un investissement plus important de la famille en cas de prise en charge orthophonique. Au terme de notre action, nous constatons que les parents sont satisfaits d'avoir participé. Certains aménagements ont été instaurés au quotidien, tandis que les activités ciblées peinent à se mettre en place. Notre action confirme également l'importance d'une réflexion sur comment les orthophonistes, malgré les listes d'attente, pourraient accueillir et accompagner ces familles dans un cadre préventif.

Notre étude vise à établir un état des lieux de la pratique de l'accompagnement parental en libéral en France, à étudier les variables d'âge et de diagnostic orthophonique de l'enfant pouvant modifier son application et à recenser les rôles et les attentes des parents. Afin de mener à bien cette enquête, nous avons opté pour une méthodologie qualitative et quantitative combinant la passation de huit entretiens semi-directifs et la distribution de cent quatorze questionnaires autoadministrés, auprès de parents d'enfant bénéficiant d'une intervention orthophonique en libéral. Les résultats suggèrent la prédominance de l'axe d'information associée à l'émergence d'un accompagnement, davantage basé sur la formation, se traduisant dans les modalités mises en place ainsi que dans les rôles que s'attribuent les parents. Les parents semblent globalement satisfaits de l'accompagnement parental proposé actuellement, notamment au niveau relationnel. Toutefois, une part significative d'entre eux souhaiterait se voir impliquée de façon plus active.

Pour les exploiter, l'enfant a besoin de recevoir un modèle langagier correct, d'avoir un retour lorsqu'il s'exprime et donc surtout, de dialoguer.A ce titre, ses parents ont un rôle essentiel. Pourtant, dans certaines familles, les parents ne stimulent pas, peu ou mal leur enfant. Certains accumulent ainsi un retard langagier qui requiert une prise en charge orthophonique. Ce mémoire s'inscrit dans le cadre de la prévention en orthophonie. Il veut participer à l'amélioration de la prise en charge des enfants peu ou mal stimulés, en proposant des pistes d'action, de partenariat avec les parents, afin de les impliquer davantage dans le développement langagier et la rééducation de leur enfant. En premier lieu, nous avons voulu savoir, à travers une enquête, comment est vécue la rééducation par les orthophonistes et par ces parents peu stimulants, afin de recenser les avis, les besoins et les attentes de chacun. Cet état des lieux permet de connaître et donc de mieux comprendre les obstacles, difficultés et spécificités liés à la prise en charge de ces enfants et donc d'essayer de l'améliorer. Notre démarche fait apparaître que : les difficultés des orthophonistes face à ce type de prise en charge se situent surtout au niveau du manque d'implication des parents. Beaucoup cherchent à trouver une solution à ce sujet parce qu'elles sont conscientes qu'un investissement plus important de ces derniers permettrait d'améliorer l'efficacité de la rééducation de l'enfant ; les parents sont globalement satisfaits de la prise en charge qui est proposée à leur enfant. Cependant, certains semblent vouloir améliorer leurs connaissances langagières et d'autres disent être intéressés par des actions visant à les intégrer davantage dans la rééducation de leur enfant. En second lieu, des propositions de partenariat avec les parents, propositions adaptées aux attentes de chacun, ont été soumises aux orthophonistes impliquées dans le projet. Il s'agit donc ici d'un essai d'implication, de sensibilisation parentale, adapté aux contraintes réelles rencontrées par les orthophonistes et parents de quinze cabinets. Ces pistes, si elles sont réalisables et réalisées ne peuvent qu'être bénéfiques à la progression de l'enfant, à la qualité de la relation orthophoniste-parents et à l'amélioration des connaissances des parents qui profiteront aux autres enfants de la famille.

Book excerpt: Although speech-language pathologists are expected to be able to administer and interpret oral examinations, there are currently no screening tests available that provide careful administration instructions and data for intra-examiner and inter-examiner reliability. The Oral Speech Mechanism Screening Examination (OSMSE) is designed primarily for use by clinical speech-language pathologists. The examination could also serve, however, as a useful tool for oral myofunctional therapists, as well as physical therapists, dentists, and orthodontists interested specifically in speech. It is reliable, relatively easy and quick to administer, and appropriate for children and adults in either diagnostic or therapy settings. The OSMSE is intended to assess those anatomical structures and physiological functions that are most often considered to be potentially related to speech or language disorders. Structural and/or functional judgments included are organized on the OSMSE test form under categories of: lips, tongue, jaw, teeth, hard palate, soft palate, pharynx, breathing, and diadochokinesis. Results of a preliminary study with normal speaking subjects indicated that the OSMSE had satisfactory intra- and inter-examiner reliability after minimal training. The time required to administer the examination typically ranged from 5 to 10 minutes. (Author/GK)
