The fire fighter was rescuing an injured construction worker who had been hurt during the collapse of a separate cell phone tower located on the same site. He and three other fire fighters were dragging the injured maintenance worker out of the danger zone of the first collapsed tower when a second tower collapsed and struck him as he attempted to run away. He was transported to a local hospital where he was pronounced dead. The initial tower collapse occurred during scheduled maintenance to reinforce the tower.

They arrived on scene to find a camper fully involved with fire and flames impinging on an adjacent mobile home. The occupants of the camper and mobile home escaped without injury, prior to the fire department's arrival. The victims entered the mobile home through the front door with a charged 1-in hoseline. Within 5 to 10 minutes of them entering, the pump operator sounded the evacuation alarm when he noticed his tank water was low. The victims did not evacuate from the structure. Fire fighters on scene attempted to contact the victims via radio and by yelling into the mobile home. The fire chief and a fire fighter tugged on the 1-in hoseline several times with no response.

Book excerpt: On February 13, 2012, a 21-year-old male volunteer fire fighter (victim) died after falling from the tailboard of a fire department tanker (Tanker 14). The victim, acting as a spotter and using the driver's side mirror, had successfully guided the driver in backing the tanker to position its tailboard near the dump tank. The driver of Tanker 14 stayed in the driver's seat and watched the water gauge indicator lights on the pump panel through his side mirror. The victim, located on the tailboard, operated the dump valve to fill the folding tank. When the driver saw the tank-empty light flash, he left the fire scene to go to the water source to refill the tanker. Unknown to the Tanker 14 driver, another tanker (Tanker 9) had inadvertently dropped approximately 1,500 gallons of water on the roadway while also responding to the incident. Tanker 9 had reported the inadvertent drop to their dispatcher, but Tanker 14 had not heard this communication. As Tanker 14 traveled this same roadway en route to the water source, it hit a patch of black ice that had resulted from the inadvertent water drop. The Tanker 14 driver lost control and the tanker spun around a number of times before impacting a berm on the shoulder of the roadway. The Tanker 14 driver was injured but was able to radio for help and crawl out of the passenger side door. The driver then saw the victim lying unresponsive in the roadway. A rescue unit from the fire scene responded to the crash along with other units and emergency medical aid was performed. The victim was taken to a local hospital where he died from his injuries.

Book excerpt: On September 6, 2010, a 51-year-old male volunteer fire fighter (victim) died after being overcome by low oxygen and sewer gases while climbing down into a sewer manhole in an attempt to rescue a village utility worker. The utility worker had entered the manhole to investigate a reported sewer problem and was overcome by low oxygen and sewer gases. The incident occurred behind the fire station in an underground sewer line that ran under the fire station. The local utility company contacted the chief of the village's volunteer fire department and requested that a piece of fire apparatus be moved out of the station so they would not block it in while accessing a manhole. The fire chief responded to the station to move fire apparatus so it would not be blocked by the utility trucks. The victim and another fire fighter also arrived at the station to assist. A utility worker entered the manhole behind the station to clear a sewer backup and was overcome by a lack of oxygen and sewer gases and then fell unconscious inside the manhole. The victim then entered the manhole without any personal protective equipment to help the utility worker and was also overcome by the low oxygen level and sewer gases. The victim and the utility worker were later removed from the sewer manhole by fire department personnel and transported to a local hospital where they were pronounced dead. The medical examiner reported the cause of death as asphyxia due to low oxygen and exposure to sewer gases.

He overcompensated, and the engine traveled across both lanes, left the road, and struck a large tree. The passenger was ejected from the cab of the engine and landed approximately 30 feet away. At 1537 hours, Central Dispatch requested all volunteers to respond to the scene to assist in the extrication of the victim. The victim was trapped in the engine and was removed approximately 1 hours later. The victim and the passenger were taken by ambulance to an area hospital. The passenger was hospitalized for 5 days, and the victim died 3 days after the incident.

Book excerpt: On November 13, 2010 a 23-year-old career fire fighter (Victim) was struck and killed by a car while at the scene of a grass fire along a four lane, east-west, interstate highway. The highway was divided by a grassy, and in places underbrush-covered, median. A passing motorist saw the fire burning in the median and reported it to 911 who notified the local fire department. An engine (E24), and a quint (E4), were dispatched to the scene. E24 arrived ahead of E4 and parked next to the median straddling the fog line and pulling into the passing lane to provide a protected work area for the fire fighters. E4 arrived one minute later and parked west of E24 pulling toward the guardrail to provide a traffic block. At the same time, a car and a van were travelling eastbound toward the parked fire apparatus. Both vehicles were in the passing lane when the van hit the back of the car. Following the collision, the van drove on the shoulder of the passing lane and stopped before reaching E4. The car travelled onto the left shoulder of the highway passing between both apparatus and the guardrail before striking the two fire fighters. The injured fire fighter was thrown into the median by the impact and suffered critical injuries but survived. The Victim landed onto the road shoulder. Fire fighters who were on the scene initiated patient care measures immediately. The Victim was transported to a local trauma center and despite resuscitative efforts, he was pronounced dead.
