Book excerpt: On July 7, 2008, a 58-year-old male volunteer fire fighter (the victim) was fatally injured after the engine he was driving left the roadway and overturned several times. The victim was en route to a vehicle fire and was approximately 1.4 miles from the fire station when the incident occurred. The engine left the roadway shoulder after misjudging the clearance to a large farm tractor stopped in the oncoming lane. The victim was not wearing a seat belt and was ejected from the vehicle. Another fire fighter onboard the apparatus crawled from the wreckage and began administering first aid to the seriously injured victim. The victim was airlifted to a medical center and was later pronounced dead. It was later determined that the victim was driving with a blood alcohol level in excess of the legal limit.

Book excerpt: On March 19, 2003, a 53-year-old male volunteer captain (the victim) was killed while enroute to a hazardous materials (hazmat) training exercise. The victim was riding in Engine 91 (E91) on the front passenger side when it left the roadway and careened into a ditch. After traveling over a hundred feet in the ditch, the engine struck a tree on the front passenger side of the vehicle. The victim was pinned in his seat by the wreckage, and, although extrication was attempted shortly thereafter, the victim died of his injuries while inside the apparatus. NIOSH investigators concluded that, to minimize the risk of similar occurrences, fire departments should develop and enforce standard operating procedures (SOPs) on the safe and prudent operation of emergency vehicles; ensure that drivers of fire department vehicles receive driver training at least twice a year; develop a pre-incident planning program to include deployment of resources.

The victim left his residence in his privately owned vehicle (POV). It is believed that he was heading directly to the fireground, and, as he approached the fireground, he saw that no apparatus had arrived on the scene. He was proceeding to the fire station to obtain the apparatus when his POV collided with a tandem dump truck that was turning onto the road he was traveling.

The slip-on unit detached from the truck bed and landed near the front of the truck. Fire department personnel arrived quickly on the scene and began preparing the truck to remove the Victim who was alert and communicating. However, once extricated and transported to a local hospital he succumbed to his injuries.

His vehicle left the road, struck two mailboxes, traveled up an embankment, and overturned several times. The victim was ejected through the sunroof and was found lying unresponsive on the road. He was transported via ambulance to a local hospital and was life-flighted shortly thereafter to a regional trauma center where he was pronounced dead the next morning.

Several fire fighters noticed an Assistant Chief's pickup truck blocking the bay door for one of their tankers. After receiving permission, a fire fighter wearing his bunker pants and wet, rubber fire boots, backed the pickup truck at an angle to the adjacent bay, and was pulling forward when it accelerated suddenly. The pickup truck struck the bay door causing it to cave inward pinning the victim against a parked tanker.

The victim immediately got out of the apparatus and positioned himself behind the apparatus on the driver-side to direct the driver in backing the brush truck up a steep incline in the gravel roadway to get closer to the burning structure. The victim tripped or fell and was struck by the backing brush truck. The second due fire apparatus arrived on-scene just as the incident occurred and crew members helped with emergency medical efforts. There were a number of bystanders in the area but there were no known eye-witnesses to the incident. The incident occurred at night in a poorly-lighted rural area.

Book excerpt: On February 13, 2012, a 21-year-old male volunteer fire fighter (victim) died after falling from the tailboard of a fire department tanker (Tanker 14). The victim, acting as a spotter and using the driver's side mirror, had successfully guided the driver in backing the tanker to position its tailboard near the dump tank. The driver of Tanker 14 stayed in the driver's seat and watched the water gauge indicator lights on the pump panel through his side mirror. The victim, located on the tailboard, operated the dump valve to fill the folding tank. When the driver saw the tank-empty light flash, he left the fire scene to go to the water source to refill the tanker. Unknown to the Tanker 14 driver, another tanker (Tanker 9) had inadvertently dropped approximately 1,500 gallons of water on the roadway while also responding to the incident. Tanker 9 had reported the inadvertent drop to their dispatcher, but Tanker 14 had not heard this communication. As Tanker 14 traveled this same roadway en route to the water source, it hit a patch of black ice that had resulted from the inadvertent water drop. The Tanker 14 driver lost control and the tanker spun around a number of times before impacting a berm on the shoulder of the roadway. The Tanker 14 driver was injured but was able to radio for help and crawl out of the passenger side door. The driver then saw the victim lying unresponsive in the roadway. A rescue unit from the fire scene responded to the crash along with other units and emergency medical aid was performed. The victim was taken to a local hospital where he died from his injuries.

The vehicle left the road, traveled down an embankment and struck two trees before coming to a rest on the roadway below. The engine's auxiliary braking system (engine brake) was engaged at the time of the incident. The victim was found lying unresponsive under the running board on the passenger's side of the vehicle. The victim was found to have lacerations around his mid-section and no pulse.
