Book excerpt: On September 3, 2011, a 22-year-old male volunteer fire fighter (the Victim) was fatally injured when the fire department service truck (S1), he was driving en-route to a medical assistance call crashed. The Victim was traveling alone on a narrow, unmarked, asphalt road that had a bend to the right at the bottom of a slight grade. While attempting to negotiate the bend, the Victim lost control of the truck which was equipped with a slip-on unit, ran off the left side of the roadway and landed in a ditch embankment against a tree. The slip-on unit detached from the truck bed and landed near the front of the truck. Fire department personnel arrived quickly on the scene and began preparing the truck to remove the Victim who was alert and communicating. However, once extricated and transported to a local hospital he succumbed to his injuries.

After entering the ditch, the medic unit overturned again, struck a barn and then overturned two more times in a field. The victim was ejected and found unresponsive. She was transported to a local hospital where she was later pronounced dead.

The victim was traveling toward the fire station on a county road when, after cresting a small hill, his POV hit an icy section of road. His vehicle left the road, struck two mailboxes, traveled up an embankment, and overturned several times. The victim was ejected through the sunroof and was found lying unresponsive on the road. He was transported via ambulance to a local hospital and was life-flighted shortly thereafter to a regional trauma center where he was pronounced dead the next morning.

He overcompensated, and the engine traveled across both lanes, left the road, and struck a large tree. The passenger was ejected from the cab of the engine and landed approximately 30 feet away. At 1537 hours, Central Dispatch requested all volunteers to respond to the scene to assist in the extrication of the victim. The victim was trapped in the engine and was removed approximately 1 hours later. The victim and the passenger were taken by ambulance to an area hospital. The passenger was hospitalized for 5 days, and the victim died 3 days after the incident.

It is believed that he was heading directly to the fireground, and, as he approached the fireground, he saw that no apparatus had arrived on the scene. He was proceeding to the fire station to obtain the apparatus when his POV collided with a tandem dump truck that was turning onto the road he was traveling.

After traveling over a hundred feet in the ditch, the engine struck a tree on the front passenger side of the vehicle. The victim was pinned in his seat by the wreckage, and, although extrication was attempted shortly thereafter, the victim died of his injuries while inside the apparatus. NIOSH investigators concluded that, to minimize the risk of similar occurrences, fire departments should develop and enforce standard operating procedures (SOPs) on the safe and prudent operation of emergency vehicles; ensure that drivers of fire department vehicles receive driver training at least twice a year; develop a pre-incident planning program to include deployment of resources.

After establishing and repositioning the TIMA, the victim positioned himself in the left lane ahead of the TIMA to direct oncoming motorists to the right lane. The victim was struck while standing on the left shoulder/left lane when a motorist swerved to the left from the right lane of travel to avoid hitting a vehicle in the right lane. The victim was pronounced dead on the scene.
