Book excerpt: On July 10, 2018, a 34-year old paid-on-call/volunteer fire Captain died, and another firefighter was injured in a building explosion while responding to a report of a natural gas leak. At 1820 hours the local combination fire department was dispatched for a report of gas odor at an intersection approximately 1 block away from the fire station. The initial arriving crew immediately began evacuating the surrounding buildings. Meeting resistance from civilians who were unwilling to evacuate businesses in the area, the Incident Commander called for additional personnel to respond to the station. A Captain with the fire department arrived following the staffing request, along with several other firefighters. After checking in with the Incident Commander, the Captain, along with firefighters from the initial responding crew, entered a building near the suspected leak to control utilities in the basement. As the crew was exiting, the building exploded. One firefighter walked away from the blast with minor injuries. Firefighters on-scene searched for the Captain and the other firefighter in the pile of rubble from the building. The injured firefighter was found buried up to his face, conscious and moving. He was transported to the hospital in critical condition having sustained multiple fractures to his jaw and head. The Captain was found face down in the rubble. Crews worked to extract the Captain, loaded him into a waiting Advanced Life Support (ALS) staffed ambulance, and performed cardiopulmonary resuscitation until they reached the hospital where he later passed away from his injuries.

Book excerpt: On March 4, 2012, a 34-year-old male volunteer lieutenant (the victim) lost his life at a theatre fire after the roof collapsed, trapping him within the theatre. At approximately 1215 hours, an on-duty patrol officer (also chief of the victim's fire department) radioed dispatch for a structure fire (flames visible). The 1st due fire department arrived on scene, set up operations on the A-side of the structure, and directed the incoming mutual aid department (victim's department) to the rear of the structure. No fire was visible from the rear. Both departments attacked the theatre fire from opposite sides (A-side and C-side) of the structure establishing their own incident commander/officer in charge, fireground operations, and accountability systems. The 1st due fire department initially fought the fire defensively from the A-side, while the victim and two additional fire fighters (FF1 and FF2) entered through the C-side, advancing a hoseline until they met A-side fire fighters near the theatre's lobby (area of origin). The 1st due fire department eventually placed an elevated master stream into operation, directing it into the lobby and then onto the roof while fire fighters were operating inside. Roof conditions deteriorated until the roof collapsed into the structure trapping the victim, FF1, and FF2. FF1 and FF2 recalled speaking with the victim immediately following the collapse, but nothing was heard from the victim following the activation of a personal alert safety system device (PASS). All three were eventually located, removed from the structure, and transported to a local hospital, but the victim had already succumbed to his injuries.

The victim and fire department members had just returned from a structure fire and were in the process of refilling tankers when the victim went inside the fire station to write the fire report. Several fire fighters noticed an Assistant Chief's pickup truck blocking the bay door for one of their tankers. After receiving permission, a fire fighter wearing his bunker pants and wet, rubber fire boots, backed the pickup truck at an angle to the adjacent bay, and was pulling forward when it accelerated suddenly. The pickup truck struck the bay door causing it to cave inward pinning the victim against a parked tanker.

Book excerpt: On December 8, 2011, a 43-year-old male career fire fighter received fatal injuries when he was trapped under falling debris during a partial collapse at the rear of a three-story residential structure. The victim was part of a rescue company that was conducting a secondary interior search for a reported missing resident. The secondary search was initiated approximately 30 minutes after the crews had arrived on-scene and approximately 10 minutes after fire fighters evacuated the building due to deteriorating conditions within the burning structure. The secondary search was initiated after the missing civilian's roommate persisted in telling fire fighters that his friend was still inside, and most likely in a rear, second-floor bedroom. The collapse trapped the victim under debris on the first floor while the injured fire fighter rode the second floor down to the basement. A total of 11 fire fighters were inside the structure at the time of the collapse. Rescue operations took approximately 50 minutes to free the victim who was unresponsive. Extensive shoring was required within the unstable collapse area and crews had to breach the brick cellar wall to reach the injured fire fighter. Following the extrication efforts, fire fighters continued to search for the missing civilian. It was later determined that the missing civilian was not inside the structure at the time of the collapse. The civilian had left prior to the arrival of the fire department.
