Using this analysis, better targeted prevention strategies can be developed in keeping with the USFA's goal to reduce firefighter deaths 25 percent by 2005. In contrast to the annual USFA firefighter fatality reports, this analysis allowed for comparisons over time to determine any changes in firefighter mortality, with a depth of scrutiny not present in earlier analyses.

Central Dispatch notified the fire department at 0656 hours of a kitchen fire, and at 0659 hours the driver of Engine 81 notified Central Dispatch that he was leaving the station to respond to the alarm. The victim left his residence in his privately owned vehicle (POV). It is believed that he was heading directly to the fireground, and, as he approached the fireground, he saw that no apparatus had arrived on the scene. He was proceeding to the fire station to obtain the apparatus when his POV collided with a tandem dump truck that was turning onto the road he was traveling.

Book excerpt: On March 19, 2003, a 53-year-old male volunteer captain (the victim) was killed while enroute to a hazardous materials (hazmat) training exercise. The victim was riding in Engine 91 (E91) on the front passenger side when it left the roadway and careened into a ditch. After traveling over a hundred feet in the ditch, the engine struck a tree on the front passenger side of the vehicle. The victim was pinned in his seat by the wreckage, and, although extrication was attempted shortly thereafter, the victim died of his injuries while inside the apparatus. NIOSH investigators concluded that, to minimize the risk of similar occurrences, fire departments should develop and enforce standard operating procedures (SOPs) on the safe and prudent operation of emergency vehicles; ensure that drivers of fire department vehicles receive driver training at least twice a year; develop a pre-incident planning program to include deployment of resources.

The Victim was traveling alone on a narrow, unmarked, asphalt road that had a bend to the right at the bottom of a slight grade. While attempting to negotiate the bend, the Victim lost control of the truck which was equipped with a slip-on unit, ran off the left side of the roadway and landed in a ditch embankment against a tree. The slip-on unit detached from the truck bed and landed near the front of the truck. Fire department personnel arrived quickly on the scene and began preparing the truck to remove the Victim who was alert and communicating. However, once extricated and transported to a local hospital he succumbed to his injuries.

The crash occurred when the engine entered an intersection with a red light and was struck by a sport utility vehicle. The engine rolled over and both victims were ejected. Victim 1 was transported to a local hospital and pronounced dead. Victim 2 was pronounced dead at the scene. Both victims were reported to not be wearing their seat belts.

Book excerpt: On April 15, 2008, at approximately 1535 hours, a 30-year-old volunteer chief and a 38-year-old volunteer fire fighter died while driving their apparatus through thick black smoke onto a bridge that had collapsed from fire damage caused by a wildfire. They were responding as mutual aid to the wildfire in a neighboring community. Key contributing factors identified in this investigation include: excessive speed for reduced visibility/smoke conditions, lack of traffic control, lack of coordination between responding agencies and departments, and inadequate driver and multi-agency response training. NIOSH investigators concluded that, to minimize the risk of similar occurrences, fire departments should: (1) ensure that fire fighters receive essential training on the emergencies that they will respond to and how to respond safely. Additionally, fire departments, municipalities, and authorities having jurisdiction should: (2) establish pre-incident plans regarding traffic control for emergency service incidents and pre-incident agreements with public safety agencies, traffic management organizations, and private sector responders; (3) train on utilizing the national incident management system to effectively respond to and manage multi-agency incidents; and (4) be aware of programs that provide assistance in obtaining alternative funding, such as grant funding, to replace or purchase fire fighting equipment.

The victim was traveling toward the fire station on a county road when, after cresting a small hill, his POV hit an icy section of road. His vehicle left the road, struck two mailboxes, traveled up an embankment, and overturned several times. The victim was ejected through the sunroof and was found lying unresponsive on the road. He was transported via ambulance to a local hospital and was life-flighted shortly thereafter to a regional trauma center where he was pronounced dead the next morning.

Book excerpt: On April 15, 2016, a 37-year-old male career fire fighter/paramedic was killed and a 19-year-old male volunteer fire fighter was seriously wounded when they were shot after a combination fire department was dispatched for a check on the welfare of a citizen. The fire fighters were on the front porch attempting to gain entry into the single family dwelling when they were shot by the resident. At approximately 1930 hours, the county 911 center received a call from a civilian who reported that he was outside of his brother’s house and his brother was not answering his phone calls or knocks on the front door. The caller reported that he had spoken with his brother earlier in the day and that his brother’s vehicle was parked in the driveway in front of his house. He further stated that his brother had known medical issues. He requested assistance in gaining entry into his brother’s house. Rescue Engine 827 with six volunteer fire fighters and Paramedic Ambulance 823 with two career fire fighter/paramedics were dispatched at 19:35 hours. After arriving on scene, the fire fighters met the homeowner’s brother in the driveway and observed that all visible windows were covered. The fire fighters knocked on the front door, announced their presence several times and checked for an open door, however they did not perform a 360-degree walk around. After again announcing their presence, the fire fighters began to force open the front doors. Forcing both doors took 5-8 minutes with multiple strikes from three fire fighters using a halogen tool, axe and a sledge. The fire fighters forced the metal outer door but had trouble forcing the inner wooden door and ended up knocking a lower panel out of the wooden door and reaching through the hole to open the door from the inside. Four fire fighters, two medics and the homeowner’s brother were standing on the small front porch and the steps in front of the door. As the door was opened the homeowner’s brother entered. The homeowner fired a pistol multiple times through the open doorway striking his brother, the fire fighter/paramedic and a volunteer fire fighter.
