Book excerpt: Since 2007 the World Health Organization (WHO) and the Joint United Nations Programme on HIV/AIDS (UNAIDS) have recommended voluntary medical male circumcision (VMMC) as an important strategy for the prevention of heterosexually acquired HIV in men in settings where the prevalence of heterosexually transmitted HIV is high. Over 25 million men and adolescent boys in East and Southern Africa have been reached with VMMC services. These new guidelines update earlier WHO recommendations to maximize the HIV prevention impact of safe VMMC services and aim to guide the transition to the sustained provision of interventions with a focus on the health and well-being of both adolescent boys and men.

His research and consulting fields include HIV and AIDS prevention, new HIV prevention technologies, HIV risk behavior, and group behavior. Clinical trials on HIV and AIDS prevention face challenges associated with infection of participants with HIV and sexually transmitted infections. Participants also experience adverse events associated with voluntary medical male circumcision and the use of microbicides and vaccines in clinical trials. The results obtained from clinical trials investigating the efficacy of microbicides and vaccines in preventing HIV infection are disappointing, but there is hope that one day a breakthrough could be made. Clinical trials on voluntary medical male circumcision and preexposure prophylaxis have shown positive results that warranted approval by the US Food and Drug Administration (FDA) and the World Health Organization (WHO). The major challenge experienced in new HIV prevention clinical trials that are underway and in approved interventions that are being scaled up apparently relates to HIV incidence that threatens the social value and scientific validity of the interventions. Paradoxically, the clinical trials and scale-up interventions have become the new sources of HIV infection although the hypothesized benefit to be realized in the end is conceptualized as outweighing the current risks involved.

The most important argument against circumcision is the permanent change of anatomy, histology and function of the penis, with potential complications, with rates reported to be low in developed countries, whereas it may be up to 85% when circumcision is carried out by traditional circumcisers, rather than by medically trained professionals in developing countries. In some studies, reporting the complications of circumcision, primary haemorrhage was the most common (52%), whereas infection, meatal stenosis, incomplete circumcision, penile oedema, glandular injury, penile adhesions, iatrogenic hypospadias and urethral injuries were also detected at different rates. At times minor complications after circumcision which cannot be avoided even when the procedure is undertaken by specialized pediatric surgeons or urologist, in a properly equipped centres; specially if the child or his penis is congenitally abnormal, the obvious examples are, circumcising a child with an excessive suprapubic fat or a child with webbed penis or microphallus . This title aims to minimize complications of MC and to compete against its serious impact on men's' health, it will educate and teach physicians about potential complications and how they could manage it early on and avoid further patient problems This title will shed some light over the common as well the uncommon complications, which usually raise a debate about its management. There are different sets to classify MC complications: Either early, or late, minor or major, local or systemic, rare or common. Both rare and common complications will be discussed with comprehensive photos and illustrations for each complication and its operative remedy.

The book illustrates the devastating health impacts of the epidemic, with life expectancy in some countries falling to the lowest levels observed anywhere, and the remarkable success of the global HIV/AIDS response in reversing such extreme outcomes. Concerns about the implications of HIV/AIDS for economic development have played a role in motivating the global HIV/AIDS response. However, evidence on the impacts of HIV/AIDS on economic growth or poverty is weak, and the magnitude and relevance of such economic effects appears trivial compared to the consequences for life and health. Because of the success in extending access to treatment globally, HIV/AIDS has effectively transitioned into a chronic disease. This means that HIV/AIDS absorbs not only a substantial chunk of current global and national financial resources, but that these spending needs are projected to persist over decades. The costs of the HIV/AIDS response thus resemble a long-term financial liability, shaped by past and current policies. Relatedly, the calculus of cost-effectiveness of HIV/AIDS interventions has changed. People who become infected with HIV can now expect to not die because of AIDS; at the same time, each HIV infection results in medical needs and expenditures extending over decades. The book presents a framework for integrating these financial consequences and the transmission dynamics of HIV in the analysis of cost-effectiveness of HIV/AIDS interventions and in the design of HIV/AIDS programs.
