Book excerpt: Des mécanismes d'adaptation du métabolisme permettent à des femmes bien nourries, en bonne santé, disposant d'une alimentation variée de mener une grossesse normale à son terme. Au cours de la grossesse, les prescriptions de suppléments actuellement très fréquentes, diffèrent selon les praticiens de telle sorte que ces prescriptions deviennent soit automatiques, soit induites par la demande des femmes. Nous avons mené une enquête auprès de 223 femmes ayant accouché au CHU de Nantes afin d'évaluer la concordance entre les prescriptions des supplémentations et les niveaux de risque de ces femmes. Notre analyse a mis en évidence un impact limité des recommandations de la Haute Autorité de Santé d'avril 2005 sur la prescription d'acide folique (26%) ou du fer notamment par rapport à la prescription de la vitamine D (75%) où les recommandations sont mieux suivies. D'autres suppléments comme le magnésium, les compléments vitaminiques, restent très prescrits alors qu'aucun intérêt n'a été démontré en leur faveur. De même, la consultation préconceptionnelle est encore trop peu réalisée (20%) car si certaines patientes prévoient leur grossesse, elles ne savent pas qu'elles doivent changer certains comportements. Ces résultats montrent que les recommandations mériteraient une meilleure diffusion auprès des professionnels de santé ainsi que des femmes en âge de procréer pour qu'ils soient plus sensibilisés à l'importance de la nutrition et de la supplémentation éclairée pendant la grossesse.

Ceci pose la question de l'intérêt d'une supplémentation durant la grossesse voire dès la période pré-conceptionnelle. Cette thèse a pour but d'exposer les changements physiologiques liés à la grossesse dans une première partie. Puis, les besoins nutritionnels chez la femme enceinte, notamment pour les nutriments « pivots » de la grossesse : la vitamine B9, la vitamine D, l'iode, le fer et les acides gras ainsi que les recommandations données par les autorités de santé. Enfin, nous terminerons par les compléments alimentaires « grossesse » existant sur le marché, leurs intérêts, leurs risques et le rôle de conseil du pharmacien d'officine.

Il est alors primordial que les femmes, dès le désir de grossesse aient une alimentation de qualité afin de réduire les prévalences de morbidité et de mortalité maternelles et néonatales. Le but de ce travail est d' apprécier la qualité du régime alimentaire et les changements dans l' alimentation avant et pendant la grossesse. De façon spécifique, nous avons étudié la diversité alimentaire, les apports en énergie et nutriments. Néanmoins, pour quelques nutriments (acide folique, vitamine D, fer et iode, etc.) et dans certaines situations (adolescence, comorbidités, etc.) une alimentation diversifiée ne suffit pas à assurer une couverture correcte des besoins de la grossesse. La question de la lutte contre ces carences se pose alors. Doit-on supplémenter de façon systématique les femmes enceintes en vitamines ? Doit-on relancer le débat sur l' enrichissement des céréales en micronutriments ? Ce travail explore les avantages et les limites de ces moyens de lutte contre ces carences. Les compléments alimentaires pour la grossesse sont en vogue, il faut renforcer leur cadre juridique pour mieux répondre aux vrais besoins de la femme enceinte. Il faut encourager le rôle de pharmacien d' officine et établir des nouvelles recommandations diffusables à tous les professionnels da santé.
