Book excerpt: The realities and misconceptions of long-term care and the challenges it presents for the ethics of autonomy are analyzed in this perceptive work. While defending the concept of autonomy, the author argues that the standard view of autonomy as non-interference and independence has only a limited applicability for long-term care. He explains that autonomy should be understood as a comprehensiveness that defines the overall course of a person's life rather than as a way of responding to an isolated situation. Agich distinguishes actual and ideal autonomy and argues that actual autonomy is better revealed in the everyday experiences of long-term care than in dramatic, conflict-ridden paradigm situations such as decisions to institutionalize, to initiate aggressive treatments, or to withhold or to withdraw life-sustaining treatments. Through a phenomenological analysis of long-term care, he develops an ethical framework for it by showing how autonomy is actually manifest in certain structural features of the social world of long-term care. Throughout this timely work, the rich sociological and anthropological literature on aging and long-term care is referenced and the practical ethical questions of promoting and enhancing the exercise of autonomy are addressed.

These facts of life and of death give ample grounds for characterizing much of the human condition as unfortunate. A core philosophical question is whether the circumstances are in addition unfair or unjust in the sense of justifying claims on the resources, time, and abilities of others. The temptation to use the languages of rights and of justice is und- standable. Faced with pain, disability, and death, it seems natural to complain that "someone should do something", "this is unfair", or "it just isn't fight that people should suffer this way". Yet it is one thing to complain about the unfairness of another's actions, and another thing to complain about the unfairness of biological or physical processes. If no one is to blame for one's illness, disability, or death, in what sense are one's unfortunate circumstances unfair or unjust? How can claims against others for aid and support arise if no one has caused the unfortunate state of affairs? To justify the languages of fights to health care or justice in health care requires showing why particular unfortunate circumstances are also unfair, in the sense of demanding the labors of others. It requires understanding as well the limits of property claims. After all, claims regarding justice in health care or about fights to health care limit the property fights of those whose resources will be used to provide care.

Colleagues daily spoke of the moral and social problems of children's health care. Our offices were near the examining rooms where children had their bone-marrow procedures done. Since this is a painful test, we often heard them cry. The hospital floor where the sickest children stayed was also nearby. The physicians, nurses, and social workers believed that children's health care needs were not being met and that more could and should be done. Fewer resources are available for a child than for an adult with a comparable illness, they said. These experiences prompted us to prepare this volume and to ask whether children do get their fair share of the health care dollar. Since the question "What kind of health care do we owe to our children?" is complex, responses should be rooted in many disciplines. These include philosophy, law, public policy and, of course, the health professions. Representing all of these disciplines, contributors to this volume reflect on moral and social issues in children's health care. The last hundred years have brought great changes in health care tor children. The specialty of pediatrics developed during this period, and with it, a new group of advocates for children's health care. Women's suffrage gave a political boost to the recognition of children's special health needs.
