Book excerpt: Over 3 million U.S. military personnel were sent to Southeast Asia to fight in the Vietnam War. Since the end of the Vietnam War, veterans have reported numerous health effects. Herbicides used in Vietnam, in particular Agent Orange have been associated with a variety of cancers and other long term health problems from Parkinson's disease and type 2 diabetes to heart disease. Prior to 1997 laws safeguarded all service men and women deployed to Vietnam including members of the Blue Navy. Since then, the Department of Veteran Affairs (VA) has established that Vietnam veterans are automatically eligible for disability benefits should they develop any disease associated with Agent Orange exposure, however, veterans who served on deep sea vessels in Vietnam are not included. These "Blue Water Navy" veterans must prove they were exposed to Agent Orange before they can claim benefits. At the request of the VA, the Institute of Medicine (IOM) examined whether Blue Water Navy veterans had similar exposures to Agent Orange as other Vietnam veterans. Blue Water Navy Vietnam Veterans and Agent Orange Exposure comprehensively examines whether Vietnam veterans in the Blue Water Navy experienced exposures to herbicides and their contaminants by reviewing historical reports, relevant legislation, key personnel insights, and chemical analysis to resolve current debate on this issue.

Book excerpt: The Vietnam War interrupted the lives of the men and women who served and affected the direction of their lives in a multitude of ways. This study examines disparities in health, cognition, and health literacy among Vietnam-era military Veterans during their late middle-adulthood years, ages 50 to 64, guided by the paradigms of life-course theory. The study attempts to remove selection bias inherent in analyzing Veteran populations by limiting the sample to Veterans. The sample was limited to those who completed high school or its equivalency, generally a requirement for military induction. Little literature exists that examines this age period in general, let alone for Veterans. The data used to help fill this gap are from the 2003 National Assessment of Adult Literacy, Health Literacy Component, and the RAND harmonized Health and Retirement Study, version N, and the individual HRS longitudinal files from 1996 through 2010. The main conclusion is that education is essential in these each three analyses in that health literacy levels, early cognitive dysfunction (CD), and self-report of health/access to health are all associated with educational attainment, e.g., education increases health literacy, reduces the odds of early CD, and increases odds of reporting good SRH. Other conclusions are: (1) Veterans in the 50 to 64-year age group have lower Mean Health Literacy Scores than Non-Veterans, (2) Veterans and civilians 65 and older have inadequate health literacy unless health is self-reported as very good or excellent, (3) CD can be detected prior to age 65 using the four questions in the HRS survey, (4) Vietnam Veterans do not have higher odds of transitioning into early CD when compared to those who did not serve, (5) Good/very good/excellent self-report of health is associated with decreased odds of transitioning into early CD, (6) No association was found between Veteran status and self-report of health (SRH) when controlling for the various indicators of socio-economic position (wealth, income, or health insurance), (7) Race was not significantly associated with good SRH in the Veteran population, and (8) Income was significantly associated with increased odds of reporting good health for Blacks, while wealth did the same for Whites. These results suggest that practitioners should be aware of potential issues relating the low health literacy levels, cognitive dysfunction, and the effects of socio-economic position and racial disparities in treating Vietnam Veterans.

Book excerpt: After the Vietnam War, Congress wanted information about the psychological effects of the war on Vietnam veterans to inform the need for PTSD services at VA. Consequently, in 1983, Congress mandated that VA provide for the conduct of a study on PTSD and related postwar psychological problems among Vietnam veterans. VA contracted with an external entity, the Research Triangle Institute, to conduct the National Vietnam Veterans Readjustment Study (NVVRS). This cross-sectional study determined the incidence and prevalence of PTSD among Vietnam veterans and Vietnam-era veterans. PTSD is an ongoing concern for Vietnam veterans, and today, Vietnam-era veterans still constitute the largest group of veterans receiving VA care for PTSD. Congress and others have been concerned about the continued prevalence of PTSD and VA's capacity to meet the needs of Vietnam veterans. In section 212 of the Veterans Benefits and Health Care Improvement Act of 2000, Congress required that VA contract with an appropriate entity to conduct a follow-up study to the NVVRS.

During the last 30 years, Agent Orange legislation has established and updated the health and disability benefits of Vietnam veterans exposed to herbicides. The Veterans' Health Care, Training and Small Business Loan Act (P.L. 97-72) elevated Vietnam veterans' priority status for health care at Department of Veterans Affairs facilities by recognizing a veteran's own report of exposure as sufficient proof to receive medical care unless there was evidence to the contrary. The Veterans' Health Care Eligibility Reform Act of 1996 (P.L. 104-262) completely restructured VA medical care eligibility requirements for all veterans. Under P.L. 104-262, a veteran does not have to demonstrate a link between a certain health condition and exposure to Agent Orange; instead, medical care is provided unless the VA has determined that the condition did not result from exposure to Agent Orange or the condition has been identified by the Institute of Medicine (IOM) as having "limited/suggestive" evidence of no association between the occurrence of the disease and exposure to a herbicide. The Veterans' Dioxin and Radiation Exposure Compensation Standards Act of 1984 (P.L. 98-542) required the VA to develop regulations for disability compensation to Vietnam veterans exposed to Agent Orange. In 1991, the Agent Orange Act (P.L. 102-4) established for the first time a presumption of service connection for diseases associated with herbicide exposure. P.L. 102-4 authorized the VA to contract with the IOM to conduct a scientific review of the evidence linking certain medical conditions to herbicide exposure. Under this law, the VA is required to review the reports of the IOM and issue regulations, establishing a presumption of service connection for any disease for which there is scientific evidence of a positive association with herbicide exposure. Navy veterans of the Vietnam Era (those who served in Vietnam between January 9, 1962, and May 7, 1975), who served offshore and were never physically present on Vietnamese soil, have been contesting the presumption of service connection for Agent Orange disability benefits. In 2006, the U.S. Court of Appeals for Veterans Claims (CAVC) ruled in Haas v. Nicholson that navy veterans who served offshore during the Vietnam Era were entitled to a presumption of exposure to Agent Orange. However, in May 2008, the U.S. Court of Appeals for the Federal Circuit reversed the prior CAVC ruling. The Agent Orange Equity Act of 2008 (H.R. 6562), which has seen no legislative action, would clarify service in Vietnam to include inland waterways, waters offshore, and, airspace above Vietnam. This report will be updated as events warrant.
