Book excerpt: In the past half century since the Vogts stimulated investigations on the anatomi cal relationships between cortex and subcortical structures, a voluminous literature has developed on the thalamus. Although much of this writing has been based upon studies of sub-human primates and carnivora, the prime goal of most investi gators has been to elucidate clinical experience. And indeed, these researches have enlightened the neurologist. From the beginning of anatomical studies of the thalamus, anatomists have sought a meaningful parcellation of the thalamus on the basis of both topography and cellular morphology. However, as the internal organization of the thalamus changes greatly in phylogeny, a topographical designation of thalamic divisions in different species loses its significance. Hence, the detailed descriptions of the nuclear configuration of carnivora, which appeared from the laboratories of Huber and Crosby, could not be readily applied to the thalamus of monkey or man. This has led to the introduction of different and more or less specific terminologies for each species rendering attempts at homologies difficult, if not impossible. Recogniz ing the changes in conceptualization of the thalamus and the advances in neuro anatomical knowledge, Dr. Van Buren has tackled the formidable tasks of bringing the thalamic nomenclature up to date. He has based his parcellation of the thala mus upon the histological morphology of the neuronal components, which are modified, to some extent at least, by the fiber tracts traversing the area.

The brain stem and cerebellum are dealt with here for the same purpose as was the brain in the previous work, i.e., to reach, step by step, knowledge that is comprehensive enough for an understanding of an atlas of sections and its clinical use. Following a brief survey of the methods used, the first chapter describes the brain stem and cerebellum surfaces as well as their location in the posterior cranial fossa. The second and the third chapter, respectively, describe the brain stem and cerebellum structures followed by brief surveys of their functions, enabling the reader to obtain an introductory view of the role of both the nuclei and fasciculi. The fourth chapter studies the brain stem vascular network in detail. Thus, this chapter sums up the results of research on brainstem superficial blood vessels and their intra nervous territories that were already presented in two previous works [79, 80]. By contrast, presentation of the cerebellar vascularization follows the previous literature.

The Atlas is based on the Handbuch der Neurochirurgie, Vols. I and III (Springer 1956, 1959) but, whereas this is a comprehensive reference work, the present book is intended to give the practicing neurosurgeon, neuroradiolo gist, neuropathologist and neurologist the concise information they need for diagnostic purposes concerning the aspect, site, and ma lignancy of tumors and other space-occupying lesions in the brain. The schematic diagrams showing the sites of predilection of these tumors, as well as a piOgnosis based on the degree of malignancy, will be most useful here. The early chapters discuss the general rules governing displace ments due to space-occupying lesions and the manifestations of brain herniations. Other neurosurgical conditions, such as localized inflammatory processes, edema and obstructive hydrocephalus, are dealt with in brief chaptets; in this case I have chosen to show some of the rarer conditions rather than all the common lesions. In spite of probable future changes in terminology and classification, we have retained the classification used in the Atlas of Histology of Brain Tumors.
