Book excerpt: As an ethicist and medical educator, who has been involved with the creation and implementation of policy surrounding end-of-life care, non-beneficial treatment, and advance directives, I (J.M.A.) realized early on in my career that advance care planning was not taking place within or external to the clinical setting despite institutional requirements and continuing medical education on such matters. Volunteering as a clinical ethics consultant and committee member at four different health care institutions, I was privy to the difficult ethical situations that would arise at the end of life, which was approximately 80% of all of the ethical dilemmas we worked through. Many of these dilemmas, particularly those that involved family and patient conflicts of values, could have been prevented through advance care planning. Through my professional and personal experiences, collaborative research was needed to better understand concepts and practices pertaining to advance care planning. Through networking and thoughtful discussions with my interdisciplinary colleagues, some of whom had dedicated their professional lives to such issues as advance care planning, collaborative research emerged that carefully looked at advance care planning from the perspectives of health care professionals in the trenches of patient care and analyzed through the lenses of social sciences, ethics, and the humanities. Multiple studies utilizing qualitative data were successfully carried out, and one is currently underway, which emerged through the process of collaborative study and writing. Our collaborative research reveals the importance of reducing researcher bias and the value of mixed-methods data gathering and analysis.

Unfortunately, the experience of dying in the United States is often characterized by fragmented care, inadequate treatment of distressing symptoms, frequent transitions among care settings, and enormous care responsibilities for families. According to this report, the current health care system of rendering more intensive services than are necessary and desired by patients, and the lack of coordination among programs increases risks to patients and creates avoidable burdens on them and their families. Dying in America is a study of the current state of health care for persons of all ages who are nearing the end of life. Death is not a strictly medical event. Ideally, health care for those nearing the end of life harmonizes with social, psychological, and spiritual support. All people with advanced illnesses who may be approaching the end of life are entitled to access to high-quality, compassionate, evidence-based care, consistent with their wishes. Dying in America evaluates strategies to integrate care into a person- and family-centered, team-based framework, and makes recommendations to create a system that coordinates care and supports and respects the choices of patients and their families. The findings and recommendations of this report will address the needs of patients and their families and assist policy makers, clinicians and their educational and credentialing bodies, leaders of health care delivery and financing organizations, researchers, public and private funders, religious and community leaders, advocates of better care, journalists, and the public to provide the best care possible for people nearing the end of life.

Book excerpt: This case study details the process by which a mixed methods approach--a concurrent nested design--was utilized for purposes of a collaborative research project that aimed to examine the barriers of advance care planning (ACP) experienced by care managers, and their accompanying attitudes and feelings through the use of qualitative focus group data and quantitative survey data. The study involved a two-part multisite study where concurrent data collection was initially intended to be analyzed separately and then compared in the interpretation phase of the study. The purpose of this case study was to explain the importance of collaborative communication in the design of mixed methods research and to illustrate the value of exploring and adopting different approaches for analysis and interpretation of data to best answer the research question(s) and fulfill proposed aims and objectives. In adopting a concurrent nested design for this research study, following the collection of data and initial analyses, we achieved a deeper understanding of our results and their implications for future ACP discussions and practices, as well as a way to further achieve the validity and reliability of qualitative research often scrutinized for its subjectivity. Our findings provided a deeper understanding of ACP and those barriers experienced by care managers, concluding ACP discussions can be successfully led but not without further education, resources, and team-based guidance for this population.

This Institute of Medicine report examines ways to measure the impacts of IPE on collaborative practice and health and system outcomes. According to this report, it is possible to link the learning process with downstream person or population directed outcomes through thoughtful, well-designed studies of the association between IPE and collaborative behavior. Measuring the Impact of Interprofessional Education on Collaborative Practice and Patient Outcomes describes the research needed to strengthen the evidence base for IPE outcomes. Additionally, this report presents a conceptual model for evaluating IPE that could be adapted to particular settings in which it is applied. Measuring the Impact of Interprofessional Education on Collaborative Practice and Patient Outcomes addresses the current lack of broadly applicable measures of collaborative behavior and makes recommendations for resource commitments from interprofessional stakeholders, funders, and policy makers to advance the study of IPE.
