Available in PDF, EPUB and Kindle. Book excerpt: CONTEXTE : La coordination entre soins primaires et secondaires nécessite des moyens de communication interprofessionnelle. Historiquement, les lettres d'adressage, de réponse et de sortie d'hospitalisation remplissent ce rôle et restent actuellement les principaux vecteurs d'informations. Ceci est d'autant plus crucial dans les systèmes de soins avec gatekeeping où les lettres d'adressage permettent réglementairement aux patients d'aller consulter un spécialiste. L'information contenue dans ces lettres peut être évaluée selon des critères de qualité médicale, littéraire et administrative. OBJECTIFS : Evaluer la qualité du contenu des lettres d'adressages issues des médecins généralistes et établir des comparaisons entre pays et entre spécialités destinataires. METHODES : Une revue systématique de la littérature est entreprise à la recherche de collections publiées de lettres d'adressage, analysées quant à leur qualité. RESULTATS : 176 études correspondent aux critères de la revue systématique. Pour 63 d'entre elles, le texte intégral n'a pu être récupéré et pour 34, les données présentes ne sont pas exploitables. Au final, 44 études ont pu être analysées numériquement et 5 autres ont fourni des critères d'évaluation pour les lettres d'adressages. Leur contenu apparaît globalement pauvre. Les items médicaux les plus souvent présents sont le motif d'adressage (81,60 %), l'histoire de la maladie (68,71 %), les antécédents personnels (51,44 %). Toutes les études sauf 6 proviennent de systèmes de soins avec gatekeeping. CONCLUSION : II paraît pertinent d'aborder la question de la coordination et des pratiques de soins primaires sous l'angle du contenu des lettres d'adressage, mais il existe des limites à cette approche. L'exercice pourrait s'avérer intéressant dans le cadre de l'évaluation des pratiques professionnelles et de la formation médicale initiale.

Book excerpt: Introduction : Le courrier médical est le principal outil de communication entre les médecins. Des critères de rédaction indispensables à la prise en charge optimale des patients ont été définis. L'objectif de cette étude était, dans un premier temps, d'évaluer la qualité des courriers d'adressage de médecins généralistes dans le service de Médecine Générale et Interne du Centre Hospitaliser d'Epernay sur 3 mois ; puis dans un deuxième temps, d'analyser l'adhésion des médecins généralistes du bassin d'Epernay à un courrier type créé à partir des premiers résultats. Matériel et méthode : Il s'agit d'une étude rétrospective, descriptive, monocentrique réalisée du 01/09/2017 au 31/11/2017. Tous les dossiers présentant un courrier d'adressage ont été étudiés. Pour la seconde partie de l'étude, il s'agit d'une analyse descriptive de questionnaires envoyés aux médecins accompagnés du courrier type proposé, entre juin et novembre 2018. Résultats : Au total 137 courriers ont été analysés. Presque 90% étaient considérés comme lisibles. Les antécédents complets du patient étaient mentionnés dans 62,7% des cas. Le traitement était manquant dans la moitié des cas. Le contexte psychosocial du patient était majoritairement absent. L'examen clinique était décrit dans un tiers des lettres et une demande du médecin généraliste était clairement formulée dans 30,7% des courriers. Puis 68 questionnaires ont été envoyés, avec un taux de réponse de 45,6%. 28 questionnaires ont pu être analysés. 53,6% des médecins ont déclaré avoir utilisé le courrier type. 95,8% des médecins étaient satisfaits de l'exhaustivité des données intégrées dans le courrier. 87,5% trouvaient que cet outil était reproductible d'un patient à l'autre, 86,7% le recommandaient. D'après plusieurs médecins, le modèle proposé était plus adapté aux personnes âgées et/ou dépendantes. Le format papier semblait être un frein à l'utilisation du courrier proposé pour certains des médecins contactés. Conclusion : La qualité et l'exhaustivité des données transmises dans les courriers d'adressage des médecins généralistes au service de Médecine Générale et Interne d'Epernay restent hétérogènes. L'utilisation d'un courrier type commun pré rempli pourrait être une solution pour améliorer nos pratiques locales. L'intégration d'un tel outil dans les logiciels de travail pourrait permettre une adhésion optimale des médecins généralistes.

Le but de notre étude est d'analyser la « valeur informative » de ces lettres d'adressage et de s'intéresser secondairement, au délai de prise en charge, à la concordance diagnostique, ainsi qu'au taux d'hospitalisation des patients adressés aux urgences. Matériel et Méthode : Il s'agit d'une étude observationnelle réalisée aux urgences de l'hôpital de Beaujon durant l'hiver 2017-2018, incluant tous les patients munis d'une lettre d'adressage. Pour chaque lettre, nous avons établi un score de « valeur informative » (VI) allant de 0 à 17 points, correspondant à la somme des informations médicales jugées indispensables. Une lettre dite de « qualité » était définie par un score de VI supérieur ou égal à 8. Les lettres étaient classées en 4 groupes: VI faible (0 à 3), modérée (4 à 7), bonne (8 à 11) et excellente (12 à >15) afin de corréler cette valeur avec différents paramètres : les délais de prise en charge, le taux de concordance diagnostique et d'hospitalisation. Résultats : Sur les 204 courriers analysés, 70,6% des lettres étaient « de qualité ». La VI moyenne était de 9,2 (IC95%: 8,6- 9,7) et la médiane de 9. Entre la prise en charge par 1'urgentiste et le premier acte médical prescrit, le délai moyen était d'autant plus court que la VI était forte. Cependant sur le temps de prise en charge total moyen, aucune différence entre les groupes n'a été observé. Plus la lettre avait une VI importante, plus le taux de concordance diagnostique était élevé. Il ne semblait pas y avoir d'influence de la VI sur le taux d'hospitalisation. Conclusion : La majorité des lettres avait une forte VI et était donc de « qualité ». Plus la VI était forte, plus le délai entre la prise en charge par l'urgentiste et le premier acte médical prescrit était court et plus la concordance diagnostique était élevée. Cette étude devrait sensibiliser les médecins à l'importance d'une lettre à forte VI afin d'optimiser la prise en charge aux urgences.

Yet its success is matched not by encouragement and enhancement of its facilities but by United Nations financial constraints which hinder its work and, ultimately, may threaten its independence in relation to the political organs of the United Nations.

Many theories have been proposed to explain this development, but until now o on has undertaken a cross-cultural, controlled study of children in the act of drawing. "Playing with Form" is the first empirical study of children drawing in diverse cultures. Alexander Alland, Jr. spent eight months observing and filming children six cultures - Japan, Bali, Taiwan, Ponape, France, and the U.S. - as they drew. Attempting to determine the accuracy of current generalizations about the development of drawing skills as well as to understand the step-by step process of drawing, Alland amassed 240 drawings (100 of which are reproduced here) by children ranging from two to eight years old. The author uses this wealth of primary material to dispute much current thinking about children's drawing, particular theories about specific universal stages of development. While he does suggest some general rules which underlie the process of drawing, Alland argues that cultural differences reflect rules which are specific to the culture in which children' "play with form". An invaluable first step toward understanding the exact role culture plays in the development of style in children's drawing, "Playing with Form" will be of fundamental interest to anthropologists, developmental psychologists, art historians, and elementary school teachers. -- back cover
