Book excerpt: Évaluation de la contamination chimique lors de l'utilisation de médicaments dangereux : surveillance environnementale et biologique. L'utilisation de médicaments antinéoplasiques expose les professionnels de santé à un risque pour leur santé. Le premier objectif de ce travail était d'évaluer la présence de contaminations environnementales lors de l'utilisation de médicaments antinéoplasiques dans 93 hôpitaux canadiens. Des prélèvements de surfaces ont été réalisés et analysés par chromatographie liquide ultraperformante couplée à la spectrométrie de masse (UPLC-MS/MS) à la recherche de 9 médicaments (cyclophosphamide, ifosfamide, méthotrexate, gemcitabine, 5-fluorouracile, irinotécan, docétaxel, paclitaxel, vinorelbine). Des traces de 8 médicaments ont été trouvées dans les pharmacies et hôpitaux de jour d'oncologie. Les sites les plus contaminés étaient la grille frontale du poste de sécurité microbiologique (PSM) (81,5 %), l'accoudoir d'un fauteuil de traitement (75,8 %) et le plancher devant le PSM (65,2 %). Les professionnels en contact de ces médicaments y sont donc quotidiennement exposés. C'est pourquoi, le second objectif a été d'évaluer la contamination biologique des travailleurs. Une collecte urinaire sur une durée de 24 heures a donc été réalisée chez 18 travailleurs exposés puis analysée par UPLC-MS/MS à la recherche de cyclophosphamide, ifosfamide, méthotrexate et alpha-fluoro-béta-alanine, métabolite du 5-fluorouracile. Tous les échantillons ont eu des concentrations indétectables pour les 4 médicaments recherchés. Ces résultats ne sont que le reflet d'un instant t de l'environnement de travail et de la contamination des travailleurs. Il faut rappeler aux professionnels de santé que le port des équipements de protection individuel est le moyen de protection le plus important et efficace.

The 2018 TZG supports building by countries of the resilience and capacity to address emerging and endemic zoonotic diseases such as avian influenza, rabies, Ebola, and Rift Valley fever, as well as food-borne diseases and antimicrobial resistance, and to minimize their impacts on health, livelihoods, and economies. It additionally supports country efforts to implement WHO International Health Regulations (2005) and OIE international standards, to address gaps identified through external and internal health system evaluations, and to achieve targets of the Sustainable Development Goals. The 2018 TZG provides relevant country ministries and agencies with lessons learned and good practices identified from country-level experiences in taking OH approaches for preparedness, prevention, detection and response to zoonotic disease threats, and provides guidance on multisectoral communication, coordination, and collaboration. It informs on regional and country-level OH activities and relevant unisectoral and multisectoral tools available for countries to use.

This process of exploration has taken place through three workshops, each dealing with different time periods. The workshop concerned with the period 1850-1914, which corresponds roughly to the so-called Second Industrial Revolution, was held in Maastricht, The Netherlands, on 23-25 March 1995. This volume is the outcome of that workshop. The other workshops dealing with European chemical industry were held in Liege in 1994, covering the First Industrial Revolution period, 1789-1850, and Strasbourg in 1996, covering the period between the two World Wars.
