Dans le cadre d’une grossesse à bas risque, la parturiente peut décider d’être suivie par son médecin généraliste. Celui-ci possède une formation théorique sous forme de cours magistraux et pratique, acquise lors de ses stages. Il possède une place importante dans le suivi de la femme-enceinte et dans son éducation. Notre travail s’intéresse à évaluer un nouvel outil multimédia, une vidéo portant sur la grossesse et le travail physiologique, dans la formation initiale et continue du médecin généraliste, ainsi que dans l’éducation des femmes enceintes. Méthode : il s’agit d’une étude qualitative par entretien individuel semi-dirigé auprès des médecins généralistes des Hauts de France pratiquant le suivi de grossesse, recrutés par la méthode de proche en proche. Les entretiens ont été enregistrés puis retranscrits mot à mot en verbatim. Une triangulation des données et une analyse thématique via le logiciel N-Vivo ont été réalisées. Résultats : douze entretiens ont été réalisés de novembre 2018 à septembre 2019. Les médecins s’accordent sur la complexité de l’obstétrique, avec la nécessité de continuellement se former. Dans cet optique, l’outil vidéo semble posséder sa place dans l’apprentissage initiale, ainsi que dans la formation continue à condition que le médecin présente un intérêt pour le sujet. Le cas de l’accouchement inopiné peut être responsable d’inquiétude. Notre vidéo semble être vu comme un outil de réassurance. La connaissance des femmes sur la grossesse semble être insuffisante et cet outil possède un réel potentiel pour pallier ce manque. Des pistes d’amélioration, ainsi que la discussion sur un mode de diffusion ont pu être exploré. Conclusion : à l’ère du multimédia, notre outil possède un réel potentiel dans le cadre de la formation des médecins, initiale ou continue, et dans l’éducation des patientes. Il semble judicieux d’évaluer notre outil auprès des femmes.

Book excerpt: Contexte : la démographie médicale actuelle place le médecin généraliste en tant qu'acteur majeur du suivi des femmes enceintes. Gestaclic est un site internet d'aide au suivi des grossesses à bas risque en médecine générale. 3 ans après son lancement nous n'avions pas d'informations sur ses répercussions. L'objectif était d'évaluer l'impact de son utilisation sur le suivi de grossesse par le médecin généraliste. Méthode : étude comparative des dossiers médicaux avant et après l'utilisation systématique de l'outil, réalisée du 15 février 2017 au 15 août 2017. Le critère de jugement principal était l'amélioration de la recherche des critères prédéfinis. Critères choisis en priorité en se basant sur les lacunes pointées par l'ENP. Résultat : 135 dossiers de patientes suivies par onze médecins généralistes en Ile-de-France ont été analysés et comparés. L'utilisation du site a eu un impact positif sur 20% des critères étudiés : la proposition de l'epp (p=0.001), la recherche de la date du dernier FCV (p=0.003), le conseil de mise à jour de la coqueluche (p=0.02), la réalisation de la palpation mammaire (p=0.003), la réalisation de la sérologie VIH (p=0.004). Conclusion : le site pourrait être un outil d'amélioration du suivi des grossesses à bas risque en médecine générale. Son évaluation sur une période plus longue avec un panel de médecins plus large serait intéressante afin de savoir si l'effet positif initialement mesuré persiste dans le temps. Une optimisation de l'architecture du site pourrait contribuer à améliorer les autres critères étudiés.

The consequence is that, often, cervical cancer is not detected until it is too late to be cured. An urgent effort is required if this situation is to be corrected. This Guide is intended to help those responsible for providing services aimed at reducing the burden posed by cervical cancer for women, communities and health systems. It focuses on the knowledge and skills needed by health care providers, at different levels of care.

However, by the late twentieth century, the increase of emerging and reemerging infectious diseases was evident in both low and high income countries. About 30 new infectious diseases have been identified in the last 20 years. Among the "new" diseases, and most importantly, the Human Immunodeficiency Virus (HIV) epidemic, with 40 million persons infected and 25 million deaths since its first description, presents one of the most significant health, societal and security challenges facing the global community. The interaction of HIV/AIDS with tuberculosis, malaria and bacterial infections have increased HIV-related morbidity and mortality, and in turn, the HIV pandemic has brought about devastating increases in tuberculosis. Understanding the population impact and the dynamics of infection diseases in the most affected region is critical to efforts to reduce the morbidity and mortality of such infections, and for decisions on where to use limited resources in the fight against infections. This book aims to contribute to these efforts by offering a demographic and epidemiological perspective on emerging and reemerging infections in sub-Saharan Africa.

Provision of sustainable pain relief within health care systems is covered in a separate chapter. Since morphine and other opioid analgesics -- required for the relief of moderate-to-severe pain in children -- are listed under the 1961 UN Single Convention on Narcotic Drugs, the new guidelines include an annex explaining the Convention's requirements for the handling and procurement of opioid analgesics for the relief of pain.

They have traditionally been viewed as repositories of information and wisdom, and are critical pillars of the community but as the HIV/AIDS pandemic destroys family systems, the elderly increasingly have to deal with the loss of their own support while absorbing the additional responsibilities of caring for their orphaned grandchildren. Aging in Sub-Saharan Africa explores ways to promote U.S. research interests and to augment the sub-Saharan governments' capacity to address the many challenges posed by population aging. Five major themes are explored in the book such as the need for a basic definition of "older person," the need for national governments to invest more in basic research and the coordination of data collection across countries, and the need for improved dialogue between local researchers and policy makers. This book makes three major recommendations: 1) the development of a research agenda 2) enhancing research opportunity and implementation and 3) the translation of research findings.

Information ranges from explanations of how specific drugs work, through the precautions to take in the presence of certain disorders, to a list of factors that influence the effectiveness of opioids. Concerning the use of opioids, readers are reminded that psychological dependence does not occur in cancer patients and that the only correct dose of morphine is the one that relieves the pain. Part two provides a guide to opioid availability. A discussion of the reasons why opioids continue to be underprescribed or difficult to obtain is followed by an explanation of the Single Convention on Narcotic Drugs.
